    Notes: Background  Recently, it has been shown that UVB phototherapy may be more effective than UVA in the treatment of vitiligo. Currently, however, no studies have compared the efficacy of UVB311 nm and broad-band UVB therapy. Calcipotriol has recently been reported to be effective adjunctive treatment for vitiligo, enhancing the efficacy of 8-methoxypsoralen plus UVA (PUVA) therapy.Methods  Ten patients were enrolled in the study; nine completed the 12 months of therapy. The upper part of the body was treated twice weekly with UVB311 nm and the lower part with broad-band UVB. Calcipotriol was applied onto the vitiligo lesions of the right side of the body and placebo on the left side. Repigmentation was documented by photography, planimetry, and Vitiligo Disease Activity (VIDA) score. The quality of life was measured by the Dermatology Life Quality Index (DLQI).Results  After 7–16 weeks, six of the nine patients showed initial repigmentation on the side treated with UVB311 nm. After 6 months of treatment, none of the patients showed repigmentation on the areas treated with broad-band UVB, which prompted us to apply UVB311 nm all over the body. At the end of 12 months, two patients showed 〉 75% repigmentation, two showed 51–75%, two showed 26–50%, and three showed 0–25%. In all patients with progressive vitiligo (seven of the nine patients), disease activity was stopped. Remarkably, vitiligo lesions treated with calcipotriol initially showed delayed repigmentation compared with control areas; however, there was no therapeutic difference between calcipotriol and placebo, both in combination with UVB311 nm, by the end of the study. The DLQI score improved significantly by an average of 28%.Conclusion  UVB311 nm therapy was effective in the treatment of vitiligo, whereas broad-band UVB had no effect. Combination with calcipotriol ointment was not superior to UVB311 nm monotherapy. The quality of life significantly improved with narrow-band UVB311 nm phototherapy.

    Description / Table of Contents: Summary A 7 year old boy presented with linear and whorled nevoid hypermelanosis (LWNH). This entity, delineated in 1988, is characterized by streaked or whorled, frequently reticulate hyperpigmented lesions following the lines of Blaschko without preceding inflammation and without atrophy. The age of onset is usually within the first 2 years of life. Histologically, there is a mild basal cell hyperpigmentation with prominent melanocytes. In contrast to earlier reports, we observed some melanophages in the upper dermis. Associated abnormalities have been reported in a few patients, but were absent in our case. We present the clinical features, diagnostic criteria and differential diagnosis of this rare entity. Apparently, our patient represents the first case of LWNH in the German literature.
    Notes: Zusammenfassung Wir diagnostizierten bei einem 7jährigen Patienten eine streifen- und wirbelförmige nävoide Hypermelanose (SWNH). Die wesentlichen Kriterien dieser 1988 beschriebenen Entität sind lineare, oft retikuläre Hyperpigmentierungen entlang der Blaschko-Linien ohne entzündliches Vorstadium und ohne Atrophie der Läsionen. Die Erkrankung manifestiert sich meist innerhalb der ersten 2 Lebensjahre. Histologisch ist eine verstärkte Pigmentierung des Stratum basale mit prominenten Melanozyten zu finden. Im Gegensatz zu früheren Beobachtungen fanden sich bei unserem Patienten einige Melanophagen im oberen Korium im Sinne einer Pigmentinkontinenz. In wenigen Fällen wurden assoziierte Mißbildungen mitgeteilt, fanden sich im vorgestellten Fall jedoch nicht. Anhand dieses Fallberichtes möchten wir das klinische Bild, die diagnostischen Kriterien und die Differentialdiagnosen dieser seltenen Entität darstellen. Soweit uns bekannt, stellt unser Patient den ersten Fall einer SWNH in der deutschsprachigen Literatur dar.

    Description / Table of Contents: Summary We describe a 55-year-old woman with an amelanotic nodular malignant melanoma of the vulva. The tumour had developed within 3 months. Physical examination showed a solid, erythematous, centrally depressed nodular tumour with a waxy appearance. It was 1.3 cm across and located on the right labium majus. The surface was eroded and showed teleangiectasias. Histopathological examination revealed a nodular malignant melanoma with predominantly spindle-shaped tumour cells, stage IV–V, and 7.2 mm thick. The primary tumour was excised but the patient died of generalized metastases 19 months later. There are very few reports on melanoma of the vulva in the dermatological literature. In addition to this case report we review the literature and discuss the clinical picture, histopathology and prognosis of this melanoma.
    Notes: Zusammenfassung Wir berichten über eine 55jährige Patientin mit einem amelanotischen nodulären malignen Melanom der Vulva, welches sich innerhalb von drei Monaten entwickelt hatte. Klinisch fand sich an der rechten großen Labie ein harter, 1,3 cm großer, kalottenförmiger, zentral eingesunkener, rötlicher Tumor von glasigem und wachsartigem Aussehen. Die Oberfläche war erodiert und wies Teleangiektasien auf. Das histopathologische Bild zeigte ein überwiegend spindelzelliges noduläres malignes Melanom, Level IV–V und Tumordicke von 7,2 mm. Neunzehn Monate nach Exzision des Primärtumors verstarb die Patientin infolge generalisierter Metastasierung. In der dermatologischen Literatur finden sich nur sehr wenige Berichte über maligne Melanome im Bereich der Vulva. Anhand dieses Fallberichtes möchten wir eine Literaturübersicht geben und das klinische und histopathologische Bild sowie die Prognose diskutieren.

    Description: This study analyses the response of the carbon and nitrogen isotopic composition of sedimentary organic matter to rapid human-induced eutrophication and meromixis as well as subsequent restoration efforts [in-lake phosphorus (P)-Precipitation, P-remediation of the well inflow and multiannual destratification] in a 46-yr sediment core sequence (1963-2009) from Fischkaltersee, a small hard-water lake (S-Germany). In addition, the sediment record was compared with detailed data on water column chemistry during almost (1977-2009) the recorded history of eutrophication and trophic recovery of the named lake. While the onset of eutrophication resulted in an abrupt positive excursion (+2.4‰), the overall reaction of δ 13 C SOM to ongoing eutrophication and meromixis as well as to permanent hypolimnion aeration and trophic recovery is a continous negative trend (-3.7‰) with the most depleted signatures (-38.8‰) present in the youngest part of the core. This negative trend was not influenced by multiannual hypolimnion aeration, which although oxygenating bottom waters (〉2 mg O 2 L –1 ), did not reverse the increasing anoxis in the sediment, as is indicated by an declining Mn/Fe ratio. Hence, we conclude that in Fischkaltersee δ 13 C SOM was controlled by photoautotrophic input only during an early phase in the eutrophication process. The signal of intensifying microbially mediated carbon cycling processes in the sediment, i.e. methanogenesis and methanotrophy, was superimposed on the primary productivity signal by crossing a certain TP threshold (approx. TP=0.04 mg L –1 ). Sedimentary δ 15 N values exhibit an overall increase (+3.4‰) in reaction to the eutrophication process, while trophic recovery produces a continous decrease in the signal (-2.7‰). Linear correlation of δ 15 N to nitrate utilisation in the epilimnion, however, is rather weak (R 2 =0.33). Comparison between sediment δ 15 N values and water column data reveals that two negative shifts in the isotopic signal (-1‰/-0.6‰) around 1980 and 1984 respectively, can be attributed to the existence of significant amounts of ammonium (up to 2 mg L –1 ) in the trophogenic zone. Coincident drops in nitrate utilisation (-20%/-53%) indicate phytoplankton assimilation of ammonium. As the artificial aeration did also not affect δ 15 N, we conclude that water column denitrification never contributed significantly to the sedimentary N isotope signal.
