    Notizen: Summary.  We report the case of a 10.5-year-old boy with severe haemophilia A (SHA) and inhibitors who presented with superior vena cava (SVC) obstruction while on immune tolerance induction (ITI) with daily recombinant factor VIII (rFVIII) and factor eight bypassing activity (FEIBA) (75 U kg−1) twice a week. The boy had a right-sided implanted central venous catheter. Imaging revealed a large occlusive thrombus in the SVC with all upper venous system drainage occurring through the azygos and collateral veins. Despite initial success with local thrombolytic therapy using recombinant tissue plasminogen activator, the thrombus persisted. Mechanical thrombolysis and angioplasty resulted in the successful removal of the thrombus and resolution of the SVC syndrome. Unfractionated heparin was used to prevent thrombus reformation/propagation. A work-up did not reveal any underlying genetic prothrombotic risk factors.The occurrence of such a profoundly symptomatic thromboembolism (TE) in a boy with SHA with inhibitors is unusual. A combination of risk factors, including the ongoing infusion of high doses of FVIII in the context of a disappearance of inhibitors together with the infusion of clotting factors known to be potentially thrombogenic, may place haemophilic patients on ITI (immune tolerance induction) at risk for this rare, life-threatening complication. The appropriate management of TEs in such a setting is unknown.

    Notizen: Abstract. A whole blood prothrombin time/international normalized ratio (PT/INR) monitor (CoaguChek, Roche Diagnostics Corp., Indianapolis, IN) was assessed in children for its accuracy, reliability, safety, and acceptance by health care personnel and patient's families. The PT/INR values measured by the CoaguChek monitor showed an excellent correlation with PT/INR values measured by the Hospital for Sick Children (HSC) laboratory (r= 0.96) and Hamilton Civic Hospitals Research Centre (HCHRC) laboratory (r= 0.92) in clinic patients and a close correlation with PT/INR values measured by the HSC laboratory (r= 0.76) and HCHRC laboratory (r= 0.74) in patients at home. Reduced correlation in the home setting did not adversely affect clinical management. The whole blood PT/INR monitor is safe and accurate for children requiring oral anticoagulation therapy in either the outpatient clinic or home setting.

Tielrooij & F. H. L. Koppens Two-dimensional crystals such as graphene and transition-metal dichalcogenides demonstrate a range of unique and complementary optoelectronic properties. Assembling different two-dimensional materials in vertical heterostructures enables the combination of these properties in one device, thus creating multifunctional optoelectronic systems with superior performance. Here, we demonstrate that graphene/WSe2/graphene heterostructures ally the high photodetection efficiency of transition-metal dichalcogenides with a picosecond photoresponse comparable to that of graphene, thereby optimizing both speed and efficiency in a single photodetector. We follow the extraction of photoexcited carriers in these devices using time-resolved photocurrent measurements and demonstrate a photoresponse time as short as 5.5 ps, which we tune by applying a bias and by varying the transition-metal dichalcogenide layer thickness. Our study provides direct insight into the physical processes governing the detection speed and quantum efficiency of these van der Waals heterostuctures, such as out-of-plane carrier drift and recombination. The observation and understanding of ultrafast and efficient photodetection demonstrate the potential of hybrid transition-metal dichalcogenide-based heterostructures as a platform for future optoelectronic devices.

    Beschreibung: Perinatal stroke causes cerebral palsy and lifelong disability. Specific diseases are definable, but mechanisms are poorly understood. Evidence suggests possible associations between arterial perinatal stroke and prothrombotic disorders, but population-based, controlled, disease-specific studies are limited. Understanding thrombophilia in perinatal stroke informs pathogenesis models and clinical management. We conducted a population-based, prospective, case-control study to determine the association of specific perinatal stroke diseases with known thrombophilias. Children with idiopathic magnetic resonance imaging–classified neonatal arterial ischemic stroke (NAIS), arterial presumed perinatal ischemic stroke (APPIS), or fetal periventricular venous infarction (PVI) were recruited. Standardized thrombophilia evaluations were performed after 12 months of age on stroke cases and controls, including quantified proteins C and S, antithrombin, factors VIII/IX/XI, fibrinogen, lipoprotein(a), homocysteine, lupus anticoagulant, anticardiolipin antibodies and genotyping of factor V Leiden (FVL), factor II G20210A (FII), and methylenetetrahydrofolate reductase C677T. A total of 212 children were studied: 46 with NAIS, 34 with APPIS, 55 with PVI, and 77 controls (male, 53%; median age, 4.8 years). Of 14 parameters, no differences were observed in 12, including all common thrombophilias. Mean prothrombin time was shorter in arterial strokes ( P 〈 .001). Rates of antiphospholipid antibodies were low, comparable to those in controls, and resolved on repeat testing. FVL and FII rates were comparable to population norms. Total number of possible abnormalities did not differ between cases and controls. Our prospective, population-based, controlled, disease-specific study suggests minimal association between perinatal stroke and thrombophilia. This does not exclude the possibility of disordered coagulation at the time of stroke but suggests testing in childhood is not indicated.

    Beschreibung: The Svalbard region faces drastic environmental changes, including sea-ice loss and “Atlantification” of Arctic waters, caused primarily by climate warming. These changes result in shifts in the sea-ice-associated (sympagic) community structure, with consequences for the sympagic food web and carbon cycling. To evaluate the role of sympagic biota as a source, sink, and transmitter of carbon, we sampled pack ice and under-ice water (0–2 m) north of Svalbard in spring 2015 by sea-ice coring and under-ice trawling. We estimated biomass and primary production of ice algae and under-ice phytoplankton as well as biomass, carbon demand, and secondary production of sea-ice meiofauna (〉10 µm) and under-ice fauna (〉300 µm). Sea-ice meiofauna biomass (0.1–2.8 mg C m–2) was dominated by harpacticoid copepods (92%), nauplii (4%), and Ciliophora (3%). Under-ice fauna biomass (3.2–62.7 mg C m–2) was dominated by Calanus copepods (54%). Appendicularia contributed 23% through their high abundance at one station. Herbivorous sympagic fauna dominated the carbon demand across the study area, estimated at 2 mg C m–2 day–1 for ice algae and 4 mg C m–2 day–1 for phytoplankton. This demand was covered by the mean primary production of ice algae (11 mg C m–2 day–1) and phytoplankton (30 mg C m–2 day–1). Hence, potentially 35 mg C m–2 day–1 of algal material could sink from the sympagic realm to deeper layers. The demand of carnivorous under-ice fauna (0.3 mg C m–2 day–1) was barely covered by sympagic secondary production (0.3 mg C m–2 day–1). Our study emphasizes the importance of under-ice fauna for the carbon flux from sea ice to pelagic and benthic habitats and provides a baseline for future comparisons in the context of climate change.

    Beschreibung: Remotely-sensed Ocean color data offer a unique opportunity for studying variations of bio-optical properties which is especially valuable in the Arctic Ocean (AO) where in situ data are sparse. In this study, we re-processed the raw data from the Sea-viewing Wide Field-of-View (SeaWiFS, 1998–2010) and the MODerate resolution Imaging Spectroradiometer (MODIS, 2003–2016) ocean-color sensors to ensure compatibility with the first ocean color sensor, namely, the Coastal Zone Color Scanner (CZCS, 1979–1986). Based on a bio-regional approach, this study assesses the quality of this new homogeneous pan-Arctic Chl a dataset, which provides the longest (but non-continuous) ocean color time-series ever produced for the AO (37 years long between 1979 and 2016). We show that despite the temporal gaps between 1986 and 1998 due to the absence of ocean color satellite, the time series is suitable to establish a baseline of phytoplankton biomass for the early 1980s, before sea-ice loss accelerated in the AO. More importantly, it provides the opportunity to quantify decadal changes over the AO revealing for instance the continuous Chl a increase in the inflow shelves such as the Barents Sea since the CZCS era.
