Résumé Un programme de traitement est proposé concernant les processus schizophréniques précoces à Symptomatologie pauvre à partir de «modèles» psycho-sociaux et psychodynamiques. Les patients sont sélectionnés selon leur âge, le niveau social et le niveau intellectuel dans le but d'aboutir aux meilleurs résultats socio-thérapiques, avec des groupes aussi homologues que possible. En présence de signes psychotiques manifestes, la psycho-pharmacologie sera utilisée en premier lieu en vue de réduire le type de défense psychotique et de permettre au Moi de fonctionner de nouveau selon un niveau de réalité socialement adapté. Les symptômes schizophréniques ou toute aggravation de l'état du malade seront appréhendés à travers une structure psycho-sociale et psycho-dynamique-type pouvant servir de modèle. Les symptômes cèdent la plupart du temps assez rapidement et le patient apprend ainsi à parler de ses difficultés réelles et de ses conflits internes. Un programme de réactivation est introduit dans le but d'exploiter les nouvelles possibilités d'adaptation et les fonctions du Moi qui réapparaissent avec le traitement. Il est surtout nécessaire, chez les patients présentant des bizarreries pré-psychotiques extravagantes, de les ramener à une réinsertion dans la réalité en corrigeant leur sens de l'orientation et en leur donnant des possibilités d'attitude et de comportement qui leur seront nécessaires pour réaliser une insertion sociale et familiale. La matrice d'une telle réhabilitation sociale est représentée par la communauté thérapeutique. Un travail d'équipe bien coordonné, avec un but thérapeutique connu et précis, une manipulation constante des nouvelles difficultés qui surgissent et des rivalités inévitables que cela comporte, sont déterminants pour une véritable efficacité du personnel soignant. Au niveau des patients, l'utilisation des conflicts intrapsychiques et interpersonnels dans le groupe thérapeutique, de même que les discussions et décisions concernant tous les problèmes pratiques, les tâches et les initiatives de la communauté thérapeutique, se font au travers d'une activité de groupe dite «démocratique». Les indispensables décisions à caractère autoritaire, souvent nécessaires, devront être clairement explicitées à chaque patient conterné. Après, il paraît nécessaire de procéder comme suit: 1. Le contrôle médicamenteux; 2. la réhabilitation sociale dans un milieu protégé compatible avec la gravité des troubles; 3. l'interprétation des difficultés d'adaptation et des conflits avec le milieu dans le groupe thérapeutique ou en traitement individuel. Les expériences relatées montrent d'abord, dans des cas d'espèces, que l'apprentissage d'un nouveau rôle social permet aussi la réalisation de besoins propres aux malades et, avec la stabilisation des fonctions du Moi, les troubles de la personnalité s'amoindrissent, ceci aussi bien dans des défects schizophréniques que lors de processus prépsychotiques.

Zusammenfassung Auf der Basis einiger sozialpsychologisch-psychodynamischer Modellvorstellungen wird über ein Behandlungsprogramm für frühmanifeste symptomarme Prozeßschizophrenien berichtet. Die Patienten — über die Einrichtungen wird ausführlich referiert — sind nach Alters-, Sozial- und Intelligenzniveau ausgelesen, um mit eingermaßen homologen Gruppen eine hohe soziotherapeutische Wirksamkeit zu erzielen. Wenn Zeichen einer manifesten Psychose vorhanden sind, werden mittels Psychopharmaka Angst-, Trieb- und Konfliktdynamik zunächst so weit reduziert, daß weniger psychotische Abwehr erforderlich ist und das Ich wieder auf einem einigermaßen realitätsgerechten, sozial angepaßten Niveau funktionieren kann. Die schizophrenen Symptome und jede Verschlechterung des Zustands werden dann dem Kranken gegenüber zunehmend auf dem Hintergrund sozialpsychologisch-psychodynamischer Modellvorstellungen verstanden. Die Symptome treten meist relativ rasch zurück; der Kranke lernt stattdessen, über seine realen Schwierigkeiten und inneren Konflikte zu sprechen. — Zu ihrer therapeutischen Bearbeitung tritt ein Aktivierungsprogramm. Sein Ziel ist die Förderung aller mit dem Zurücktreten der Psychose wieder auftauchenden sozialen Anpassungsleistungen und Ich-Funktionen. Vor allem bei Kranken mit erheblichen präpsychotischen Auffälligkeiten müssen zudem von der Umwelt erheblich abweichende Wertorientierungen korrigiert und unzureichend erworbene soziale Verhaltensmuster vermittelt werden, die notwendig sind, um Sozial- und Geschlechtsrolle eines Erwachsenen einigermaßen verwirklichen zu können. Matrix dieses sozialen Lernprozesses ist neben dem Aktivierungs- und Rehabilitationsprogramm die Stationsgemeinschaft, deren Orientierung wieder wesentlich vom therapeutischen Stab getragen wird. Eine gut koordinierte Teamarbeit unter einem klaren und gemeinsamen therapeutischen Ziel, eindeutige Rollenabgrenzung und fortlaufende Bearbeitung der auftauchenden Schwierigkeiten und der unvermeidlichen Rivalitäten ist deshalb entscheidend für die Effektivität des therapeutischen Personals. — Auf der Patientenebene sind die Durcharbeitung intrapsychischer und interpersoneller Konflikte in der therapeutischen Gruppe, die Diskussion und Abstimmung aller praktischen Probleme, Aufgaben und Initiativen der Stationsgemeinschaft in einer „demokratischen“ Aktionsgruppe (therapeutic community session) und die unbedingt notwendigen Autoritätsentscheidungen des Stabs in einer persönlichen Erläuterung und Diskussion mit dem betreffenden Patienten klar zu trennen. — Nachsorge, als Fortsetzung 1. der medikamentösen Regulierung, 2. des sozialen Lernprozesses in einem der Schwere der Störung angemessenen beschützenden Milieu und 3. der Bearbeitung anpassungsbehindernder Konflikte und Schwierigkeiten in der therapeutischen Gruppe oder in Einzelbetreuung, ist in der Regel über lange Frist erforderlich. Die vorliegenden Erfahrungen zeigen zunächst an Einzelfällen, daß mit dem Hineinwachsen in eine Sozialrolle, die auch eine befriedigende Verwirklichung der eigenen Bedürfnisse ermöglicht und mit der Stabilisierung der Ich-Funktionen zugleich die Persönlichkeitsstörungen — gleichgültig, ob man sie mehr dem „Defekt“ oder einer „apräpsychotischen Egopathie“ zurechnen müßte — geringer werden.

Summary A programme of management for patients with early process schizophrenia without florid symptomatology is reported, based upon a combined social psychological and psychodynamic model. The patients are selected according to age, social characteristics and intelligence level so as to produce a homogeneous group with a high sociotherapeutic effectiveness. If there are signs of a developing manifest psychosis, drugs are used to reduce the necessity for psychotic defences, so that the ego can again function at a realistic socially-adapted level. The schizophrenic symptoms, and each relapse in condition, thus progressively become understood by the patient in terms of a social psychological and psychodynamic formulation. The symptoms recede relatively quickly and the patient learns instead to discuss his real difficulties and inner conflicts. A concomitant activity-programme has the aim of promoting socially adaptive skills and ego-functions as the psychosis recedes. Above all, the divergent value orientations and poorly acquired social behaviour patterns of prepsychotic patients must be corrected so that adult social and sex roles can to some extent be adopted. This social learning process takes place in the matrix of the ward community through the activity and rehabilitation programme guided by the therapeutic staff. Well-coordinated team-work, with clear and mutually agreed goals, unambiguous role-boundaries and continuous handling of emerging problems and inevitable rivalries, is therefore essential for the effectiveness of the therapeutic personnel. At the patient level, it is important to provide separately for working through intrapsychic and interpersonal conflicts in therapeutic groups, for democratic discussion of important practical problems, tasks and initiatives of the ward community in staff-patient meetings, and for essential staff decisions to be elucidated in discussion with the patients concerned. Aftercare is usually needed for a considerable period 1) in order to continue medication 2) so that the social learning process can continue in an environment suited to the severity of the disorder 3) so that the management of conflicts and difficulties which hinder readaptation can continue in a therapeutic group or in individual treatment. Present experience indicates that, as the patient grows into a social role and as the ego-functions are stabilised, the personality disturbances become less, whether they are part of a “defect” state or of a prepsychotic ego-dysfunction.
