    Description: Dislocations represent one of the most fascinating and fundamental concepts in materials science. Most importantly, dislocations are the main carriers of plastic deformation in crystalline materials. Furthermore, they can strongly affect the local electronic and optical properties of semiconductors and ionic crystals. In materials with small dimensions, they experience extensive image forces, which attract them to the surface to release strain energy. However, in layered crystals such as graphite, dislocation movement is mainly restricted to the basal plane. Thus, the dislocations cannot escape, enabling their confinement in crystals as thin as only two monolayers. To explore the nature of dislocations under such extreme boundary conditions, the material of choice is bilayer graphene, the thinnest possible quasi-two-dimensional crystal in which such linear defects can be confined. Homogeneous and robust graphene membranes derived from high-quality epitaxial graphene on silicon carbide provide an ideal platform for their investigation. Here we report the direct observation of basal-plane dislocations in freestanding bilayer graphene using transmission electron microscopy and their detailed investigation by diffraction contrast analysis and atomistic simulations. Our investigation reveals two striking size effects. First, the absence of stacking-fault energy, a unique property of bilayer graphene, leads to a characteristic dislocation pattern that corresponds to an alternating AB B[Symbol: see text]AC change of the stacking order. Second, our experiments in combination with atomistic simulations reveal a pronounced buckling of the bilayer graphene membrane that results directly from accommodation of strain. In fact, the buckling changes the strain state of the bilayer graphene and is of key importance for its electronic properties.     Description: In Biology Oriented Synthesis the scaffolds of biologically relevant compound classes inspire the synthesis of focused compound collections enriched in bioactivity. This criterion is met by the structurally complex scaffolds of natural products (NPs) selected in evolution. The synthesis of NP-inspired compound collections approaching the complexity of NPs calls for the development of efficient synthetic methods. We have developed a one pot 4–7 step synthesis of mono-, bi-, and tricyclic oxepanes that resemble the core scaffolds of numerous NPs with diverse bioactivities. This sequence entails a ring-closing ene-yne metathesis reaction as key step and makes productive use of polymer-immobilized scavenger reagents. Biological profiling of a corresponding focused compound collection in a reporter gene assay monitoring for Wnt-signaling modulation revealed active Wntepanes. This unique class of small-molecule activators of the Wnt pathway modulates the van-Gogh-like receptor proteins (Vangl), which were previously identified in noncanonical Wnt signaling, and acts in synergy with the canonical activator protein (Wnt-3a).

    Notes: Summary Endoscopic decompression of the distended colon has become a useful method of treating non-obstructive colonic ileus. We propose a method of establishing a cecal fistula by means of percutaneous puncture of the colon and pull-through of a Pezzer catheter. Although until now we have only performed this procedure twice, it seems to be a valuable therapeutic approach in cases of paralytic distension of the large bowel.

Indikationen bestehen nicht nur in der Dysphagie bei stenosierenden Prozessen; Schluckstörungen aus neurologischer Ursache wie bei Schddelhirnverletzten waren bei 27 unserer 33 Patienten der AnlaB zur EPG. Die gravierende Komplikation einer sondeninduzierten Refluxoesophagitis läßt sich durch den Wegfall der Magensonde vermeiden. Die Gastrostomie kann wahlweise auch zur Drainage dienen. Weitere Vorzüge sind die sichere Methodik, die Einfachheit des Verfahrens and die Unabhängigkeit von Formeldiäten.
    Notes: Summary Endoscopic gastrostomy, pulling a Pezzer catheter through percutaneously, and using topical anesthesia, has increased the therapeutic possibilities. The indications are not only limited to cases of dysphagia due to stenosis. Neurologic diseases with deglutition disorders were the reasons for performing EPG in 27 of our 33 patients. Esophagitis following the use of gastric tubes can be avoided, and gastrostomy can also easily serve as a drainage method. Other benefits are the possibility of dispensing regular food and the safety and simplicity of the procedure. Thus, it can also be used for those patients for whom laparotomy could be harmful.

    Notes: Summary The effect of the mesocaval interposition shunt (n=12) and the distal splenorenal shunt (n=9) on the wedged hepatic venous pressure, the estimated hepatic blood flow, quantitative hepatic function, and the rate of portal systemic encephalopathy was evaluated in 21 patients who had bled from esophageal varices. After mesocaval shunt the wedged hepatic venous pressure was significantly reduced by 42% (from 26±3 mm Hg to 15±5 mm Hg,P〈0.001) compared to 16% only (from 25±3 mm Hg to 21±2 mm Hg,P〈0.005) after distal splenorenal shunt. The estimated hepatic blood flow also decreased significantly after mesocaval shunt by 61% (from 1.45±0.46 l/min to 0.56±0.25 l/min,P〈0.001) compared to 29% (from 1.29±0.32 l/min to 0.91±0.39 l/min,P〈0.05) after distal splenorenal shunt. Despite significantly different influences of both types of shunt operations on wedged hepatic venous pressure and estimated hepatic blood flow (P〈0.001), postoperative changes of hepatic function were comparable in both groups of patients. The galactose elimination capacity, the initial plasma disappearance rate of Bromsulphalein, and the plasma ratio of valine, leucine, and isoleucine to phenylalanine and tyrosine were reduced by 13%, 26%, and 29%, respectively, after mesocaval shunt, compared to 12%, 25%, and 17% after distal splenorenal shunt. Only two patients of the mesocaval shunt group with the largest decrease in estimated hepatic blood flow developed portal systemic encephalopathy postoperatively, and the distal splenorenal shunt patients with their minor hemodynamic sequelae remained free of portal systemic encephalopathy.

Dieser Algorithmus definiert eindeutige Bedingungen für die Steuerung der Insulinpumpe unter Berücksichtigung der aktuellen Blutglucosekonzentration und ihrer Änderungsgeschwindigkeit. Er ist leicht modifizierbar und dadurch an verschiedenste Bedingungen anpaßbar.
    Notes: Summary An algorithm is demonstrated which runs by means of program internal decision schedules for insulin applications during the extracorporeal blood glucose regulation by the artificial endocrine pancreas. This algorithm defines clear conditions for the regulation of the insulin pump considering the actual blood glucose concentration and its speed of change. It is easy to modify and hence feasible under different conditions.

    Description / Table of Contents: Zummmenfassung Berichtet wird der Fall eines 56jährigen Mannes, der während einer 1jährigen Magenulcusanamnese mehrfach radiologisch und gastroskopisch kontrolliert wurde. Dabei war ein, trotz kurzdauernden Remissionen, langsam größer werdendes Uleus ad pylorum festzustellen. Bei der präoperativen Kontrollgastroskopie konnte eine Spontanperforation in das Duodenum am Bulbusendteil mit voller Epithelialisierung des neben dem Pyloras neugebildeten zweiten Kanales festgestellt werden. Die Operation bestätigte den seltenen endo. skopischen Befund eines doppelten Pyloruskanals.
    Notes: Summary A 56-year-old patient was examined in our unit by gastroenterological endoscopy. Several control X-rays and gastroscopies during the previous year had shown a large penetrating ulcer in the prepyloric antrum. Before the operation, gastroscopy revealed a second pylorus canal caused by perforation of the antral ulcer into the duodenal bulb.

    Description / Table of Contents: Zusammenfassung Ein Teil der Ileusbehandlung besteht in der Absaugung des Darminhaltes mittels Intestinalsonden. Die Schwierigkeiten der Sondenplazierung in das Duodenum mittels Lagerungstechnik werden durch endoskopisches Einführen der Sonde umgangen. Bei 102 endoskopisch gelegten Intestinalsonden gelang die Einführung in 90 %; bei 68 durch Lagerung gelegte Sonden nur in 67 %. Die massive, therapieresistente Colondistension kann durch Absaugen über das Coloskop behandelt werden. Bei 38 derartigen Coloskopien gelang dies in 90 %. Beide Methoden sind vom Geübten schnell und ohne Komplikation durchzuführen.
    Notes: Summary Ileus treatment consists partly in draining off the contents of the intestine using intestinal tubes. The endoscopic method avoids the problem of inserting the tube into the duodenum by changing the patient's position. In 102 cases of endoscopic tube insertion a success rate of 90 % was achieved, whereas of the 68 insertions using the “positioning” technique only 67 % were successful. The massive distention of the colon can be treated by drainage through the coloscope. Of 28 such coloscopies, 90 % were successful. Both methods can be performed quickly and without complications by surgeons who are experienced in endoscopy.
