Abstract: Patients who develop local recurrence (LR) after primary treatment for breast cancer have a variable risk of systemic recurrence (SR) and all data regarding the risk factors for SR or utility of systemic therapy are retrospective in nature. The data from the previously published papers have been tabulated on prognosis and systemic therapy after LR both after modified radical mastectomy (MRM), and after lumpectomy and radiotherapy (LRT). From these data, a scoring system has been formulated through which it is believed patients can be assigned to either a “high-” or “low-” risk group with regard to SR. This may be helpful in attempting to make decisions about the need for systemic therapy. Many patients who have a LR are at high risk for systemic recurrence, especially after modified radical mastectomy (MRM). A reliable means of assessing this risk is mandatory.?
