Abstract Standardized osteotomies were created to cadaver femoral bones imitating pertrochanteric fractures. We performed osteosyntheses alternately by DHS and gamma-nail, following an especially defined procedure. Cyclical loads were applied to the units, measuring the maximum loading capacity, and documenting the results by radiography. We received high loading capacities in case of stable and relatively unstable osteotomy types when using the DHS. Statistically significant higher rates of the maximum loading capacity were measured for the gamma-nail compared to the DHS, when using it on the complete instable type of osteotomy. These evaluations are discussed concerning their clinical relevance.

Zusammenfassung Pertrochantäre Frakturen wurden an Leichenknochen durch standardisierte Osteotomien imitiert und nach zuvor festgelegter Versuchsanordnung durch DHS- oder Gammanagelosteosynthese versorgt. Die Präparate wurden Belastungszyklen unterzogen, dabei erfolgte jeweils die Messung der maximalen Belastbarkeit sowie anschließend die radiologische Dokumentation der Belastungsversuche. Beim stabilen und relativ instabilen Osteotomietyp ließ sich nach der DHS-Osteosynthese eine hohe Belastbarkeit feststellen. Beim komplett instabilen Osteotomietyp wurden nach der Gammanagelung statistisch signifikant höhere Werte für die maximale Belastbarkeit ermittelt als nach der DHS-Versorgung. SchluBfolgerungen für die klinische Anwendung wurden diskutiert.
