    Notes: An open, randomized trial was performed to determine the optimal dosage schedule with regard to the efficacy and safety of cyclosporin in severe atopic dermatitis. The study also provided clinical experience with regard to the efficacy and safety of long-term cyclosporin treatment. During a 2-month dose-finding period. 78 patients with severe, long-standing atopic dermatitis received cyclosporin at a dose of either 5 mg/kg per day, decreasing to 3 mg/kg per day (Group A), or 3 mg/kg per day, increasing to 5 mg/kg per day (Group B). Patients were maintained on their optimal dose for a further 10 months. Patients in Group A showed a significantly greater improvement in efficacy parameters over the first 2 weeks than with patients in Group B, but as the dose was decreased in Group A and increased in Group B, these differences were minimized. After 1 year, cyclosporin showed an efficacy of 59.8% in Group A and 51.7% in Group B, assessed by a severity score. Assessed in terms of an area score, these figures were 48.7% and 40%, respectively. Cyclosporin demonstrated a good safety profile during long-term treatment and was generally well tolerated. The lower starting dosage was not associated with higher dropout rates. This study showed no differences in efficacy or adverse events between the two dosage schedules in long-term treatment.

    Notes: Summary This double-blind, parallel group study compared a 2-week course of terbinafine 250 mg/day with a 4-week course of itraconazole 100 mg/day. A total of 190 patients were enrolled, of whom 129 were evaluable for efficacy. At week 8, 69% of patients treated with terbinafine were classified as effectively treated (mycological cure, and clinical assessment total score ≤2) vs. 67% in the itraconazole group. At week 16, however, the rating for effective treatment increased to 71% of the terbinafine group, but decreased to 55% of the itraconazole group. This difference was of borderline statistical significance (P= 0.06). The results of this study demonstrate that both drugs can be used safely, and that 2 weeks' treatment with terbinafine 250 mg daily is as effective as 4 weeks' treatment with itraconazole 100 mg daily, but with fewer long-term relapses.

    Notes: Terbinafine is an allylamine antifungal compound shown to be effective in the oral treatment of onychomycosis. Because of the fungicidal acivity of the drug, a shorter duration of treatment, compared with the currently used oral treatment modalities, can be expected in onychomycosis of the toenail. In the present randomized study, the efficacy of oral terbinaifne treatment (250mg/day) was assessed for periods of 6.12, and 24 weeks. All patients were followed for up to 48 weeks after starting treatment. Of the 120 patients with toenail onychomycosis who entered the study, 98 were evaluable for efficacy. The involvement of the toenails was assessed both clinically and mycologically throughout the study. Evaluation at 24 weeks showed that complete cure of toenail onychomycosis was achieved in 67% of patients treated for 6 weeks, 82% treated for 12 weeks, and 85% treated for 24 weeks. At the end of a further 24 weeks of follow-up, cure rates were 40%, 71% and 79%, respectively. The adverse effects of terbinafine were mostly mild-to-moderate gastrointestinal symptoms. Three patients discontinued treatment because of side-effects. In conclusion, oral treatment with terbinafine is effective and generally well tolerated in patients with onychomycosis. Our results demonstrate that, for toenail onychomycosis, a treatment period of 12 weeks is sufficient.

    Description: Surface elevation measurements from CryoSat-2 data were examined to determine their utility for measuring ice sheet grounding line locations and ice thickness in Antarctica. The boundary between grounded and floating ice is an important glaciological parameter, because it delineates the lateral extent of an ice sheet and it marks the optimal location for computing ice discharge. We present a method for detecting the grounding line as the break in ice sheet surface slope, computed from CryoSat-2 elevation measurements using a plane-fitting solution. Furthermore we measure ice thickness at the grounding line using firn corrected CryoSat-2 data based on the theory of hydrostatic equilibrium. We apply these techniques to map the break in surface slope and ice shelf thickness at the grounding line in four topographically diverse sectors of Antarctica - the Filchner-Ronne ice shelf, the Ekström ice shelf, the Amundsen Sea Sector, and the Larsen-C ice shelf - using CryoSat-2 observations acquired between July 2010 and May 2014. An inter-comparison of the CryoSat-2 break in surface slope with independent measurements of the hinge line position determined from quadruple-difference synthetic aperture radar interferometry (QDInSAR) shows good overall agreement, with a mean separation of 4.5 km. In the Amundsen Sea Sector, where in places over 35 km of hinge line retreat has occurred since 1992. The CryoSat-2 break in surface slope coincides with the most recent hinge line position, recorded in 2011. Ice shelf ice thickness measurements are validated with Radio Echo Sounding (RES) point data and show good overall agreement with BEDMAP 2 ice thickness data. The techniques we have developed are automatic, computationally-efficient, and can be repeated in the future given further data acquisitions offering a complimentary approach to existing techniques.

    Description: Floating ice shelves fringe 74% of Antarctica's coastline, providing a direct link between the ice sheet and the surrounding oceans. A better understanding of Antarctic ice shelves and the physical processes affecting them has been the main objective of ESA’s Polar+ Ice Shelves project. A suite of geophysical products based on Earth Observation datasets from the last decade and modelling has been defined and produced over selected target ice shelves in Antarctica. One of these products, the ice shelf area change, is an important indicator of ice shelf stability in a warming climate, being affected by grounding line retreat as a possible consequence of ice thinning and calving events including ice shelf disintegration or collapse.An ice shelf is bounded at its seaward margin by the calving front while its inland border to the grounded ice of the Antarctic continent is given by the grounding line. Our calving front location is derived from Cryosat-2 swath elevation, while the grounding line is detected as the upper limit of ice shelf tidal flexure from Sentinel-1 and, prior to 2015, ERS-1/2 interferometric data. Time series of complete ice shelf delineations are obtained from the combination of these two products. It is possible to track absolute and relative area change of an ice shelf and additionally to partition the change into the individual contributions induced by the calving front and grounding-line migration. Examples of annual ice shelf perimeters of major ice shelves from 2011 to the present will be shown.

    Description: Totten Glacier is the principal source of ice loss from the East Antarctic Ice Sheet. Although the East Antarctic Ice Sheet as a whole has remained approximately in balance, the response of Totten Glacier to climate forcing remains a key source of uncertainty in predicting its future contribution to sea level rise. Here, we compare and combine estimates of the mass change of Totten Glacier and it's surrounding region from satellite measurements of changes in its volume, ice speed and gravitational potential acquired over the past two decades between 2002 and 2022. Ice losses from the Totten Glacier catchment and two surrounding areas – the Vincennes Bay region and the Moscow University catchment – have doubled since 2002 from 8.5 ± 0.7 Gt/yr to 20 ± 1.5 Gt/yr. We find the largest disagreement in Vincennes Bay, which remains a challenging region in which to monitor mass changes - likely a combination of a paucity in observations of ice thickness, and the regions’ small mass imbalance compared to local SMB fluctuations. Using a regional climate model, we show that only Totten Glacier is losing ice due to it flowing faster than it’s equilibrium state, although the rate of its dynamic ice loss has slowed by 60 %. In total, the region has lost 285 ± 19 Gt of ice and raised the global sea level by 0.8 ± 0.1 mm, with the majority (62 %) of this loss originating from Totten Glacier itself.
