    Kurzfassung: Background and Objectives: Endoscopic laser dacryocystorhinostomy(DCR) is now a well established, effective approach to relieve nasolacrimal duct obstruction.The aim of DCR surgery is to create a permanent opening between the lacrimal sac & nasal mucosa in patient of chronic dacryocistitis. There are many methods to perform DCR but our approach was diode laser assisted endoscopic DCR. The Objective is to study the outcomes of diode laser assisted endoscopic DCR. Material and Methods: This hospital based prospective non-randomized interventional clinical trial included 49 patients with chronic dacryocystitisat National Institute of ENTbetween august 2015 to July 2016. Success and complications were assessed by evaluating the patient at 1 week, 1 month, 3 month,6 month postoperatively and at least 3 month after removal of silicon tube those who were intubated by nasal endoscopy and sac patency test. Success was defined as absence of epiphora and patency of lacrimal drainage system. Results: Mean age with SD was 44.96 ±15.807 (Range 13 – 70 years). Patients between age group 40 – 60 yrs. was the highest (40.8%), Female were predominant 35 (71.4%) than male. Male Female Ratio was 1: 2.5. Septoplasty for DNS in 8 (16.33%) cases, turbinoplastyfor hypertrophied terbinatein 1 case were performed. Complications were injury to middle turbinate, sump syndrome, syneachia and granulation tissue formation. Overall success rate of DCR was seen in 87.76% cases. Conclusion: Laserassisted endoscopic DCR surgery eliminate the need of conventional external DCR. It is easy to perform, cosmetically good, high tech and has excellent outcome.     Kurzfassung: In recent years, polymer-supported magnetic iron oxide nanoparticles (MIO-NPs) have gained a lot of attention in biomedical and healthcare applications due to their unique magnetic properties, low toxicity, cost-effectiveness, biocompatibility, and biodegradability. In this study, waste tissue papers (WTP) and sugarcane bagasse (SCB) were utilized to prepare magnetic iron oxide (MIO)-incorporated WTP/MIO and SCB/MIO nanocomposite particles (NCPs) based on in situ co-precipitation methods, and they were characterized using advanced spectroscopic techniques. In addition, their anti-oxidant and drug-delivery properties were investigated. Field emission scanning electron microscopy (FESEM) and X-ray diffraction (XRD) analyses revealed that the shapes of the MIO-NPs, SCB/MIO-NCPs, and WTP/MIO-NCPs were agglomerated and irregularly spherical with a crystallite size of 12.38 nm, 10.85 nm, and 11.47 nm, respectively. Vibrational sample magnetometry (VSM) analysis showed that both the NPs and the NCPs were paramagnetic. The free radical scavenging assay ascertained that the WTP/MIO-NCPs, SCB/MIO-NCPs, and MIO-NPs exhibited almost negligible antioxidant activity in comparison to ascorbic acid. The swelling capacities of the SCB/MIO-NCPs and WTP/MIO-NCPs were 155.0% and 159.5%, respectively, which were much higher than the swelling efficiencies of cellulose-SCB (58.3%) and cellulose-WTP (61.6%). The order of metronidazole drug loading after 3 days was: cellulose-SCB 〈 cellulose-WTP 〈 MIO-NPs 〈 SCB/MIO-NCPs 〈 WTP/MIO-NCPs, whereas the sequence of the drug-releasing rate after 240 min was: WTP/MIO-NCPs 〈 SCB/MIO-NCPs 〈 MIO-NPs 〈 cellulose-WTP 〈 cellulose-SCB. Overall, the results of this study showed that the incorporation of MIO-NPs in the cellulose matrix increased the swelling capacity, drug-loading capacity, and drug-releasing time. Therefore, cellulose/MIO-NCPs obtained from waste materials such as SCB and WTP can be used as a potential vehicle for medical applications, especially in a metronidazole drug delivery system.

    Kurzfassung: Sonneratia caseolaris (L.) is a common mangrove plant which has significant medicinal value in traditional medicine. Ethanol extract from the fruits of S. caseolaris (SCE) was used in this project to explore its different pharmacological effects considering its traditional usage. In the castor oil-induced diarrheal method, SCE significantly lengthened the latency of the first defecation period up to 95.8 and 119.4 min as well as lowering stool count by 43.3% and 64.4% at the doses of 250 and 500 mg/kg, respectively. In evaluating the neuropharmacological effect using the open-field model, a significant central nervous system (CNS) depressant nature was observed after a reduction in the no. of squares crossed by mice at various time intervals. In evaluating the blood coagulation effect, SCE significantly reduced blood clotting time at 5.86, 5.52, and 5.01 min at 25, 50, and 100 mg/ml doses, respectively. In the assessment of the anthelmintic effect, SCE significantly killed Paramphistomum cervi (P. cervi) where the death times of the nematodes were 40.3, 36.8, and 29.9 min at 12.5, 25, and 50 mg/ml doses, respectively. The extract showed a very poor cytotoxic effect in brine shrimp lethality bioassay. In molecular docking analysis, maslinic acid, oleanolic acid, luteolin, luteolin 7-O-β-glucoside, myricetin, ellagic acid, and R-nyasol showed the best binding affinities with the selected proteins which might be the credible reasons for eliciting pharmacological responses. Among these seven compounds, only luteolin 7-O-β-glucoside had two violations in Lipinski’s rule of five.

    Kurzfassung: Background: It's fairly uncommon for one autoimmune illness to coexist with another. Systemic lupus erythematosus (SLE) and underactive thyroid are two of the most frequent autoimmune disorders. They could have some kind of connection. Objective: In this study our main goal is to evaluate the association of Systemic Lupus Erythematosus with Autoimmune Hypothyroidism. Method: This cross sectional study was carried out tertiary hospital Bangladesh from June 2021 to June 2022. A total of 100 randomly selected indoor and outdoor SLE patients of both sexes and different age groups were enrolled for the study (Group-1). Total 100, age and sex matched healthy controls without SLE (Group-2) was selected for comparison. Verbal consent was taken from both group and ethical clearance was obtained from local ethical committee. Results: During the study, 21-25 years age group, 31% and 95% were female. In group-1 45% had hypothyroidism, followed by 35% had subclinical hypothyroidism, 35% had enlarged thyroid gland. Whereas in group-2 26% had hypothyroidism, followed by 55% had subclinical hypothyroidism. In Group-1 m hemolytic anemia 25%, ITP 21%, APS 20%, Type I DM 9%, MCTD 8%, RA 5%, Dermatomyositis- Polymyositis 6%, Grave’s Disease 4% and miscellaneous 2%. Whereas in case of Coexisting non-immunologic diseases cushingoid 30%, PUD 6%, CKD 6, dyslipidemia 21%, osteoporosis 5%, hepatitis 2%, IHD 4%, CVD 2%. Conclusion: There was no conclusive proof that SLE caused autoimmune hypothyroidism. The clinical presentation of these disorders is likewise similar. This means that all SLE patients may benefit from having a thyroid screening test performed to check for autoimmune hypothyroidism. It is important to look for any autoimmune disorders that may be present and treat any comorbidity found. All of these things are important to think about while choosing a treatment for SLE and improving its management.

    Kurzfassung: COVID-19, the disease and ‘‘lockdown’’, the preventive measure, can be termed as major watershed of our times as never before in human history human lives and activities have been halted to such a huge level as combat measure against any disease. Under this unprecedented situation, an online survey was conceptualised to see the situation of students’ households during lock down period.Objective: On the basis of that data, feasibility of adherence regarding public health aspects (like washing hands, physical distances, and restricted movement outside house) are discussed through this study. Moreover, the awareness, capability and feasibility of households to abide by the prescribed norms are numerically measured through related social variable and logical insight.Method: Though this cross sectional survey data collected during May 15 to May 31st 2020, three separate indices regarding three major points of guideline e.g. Frequent hand washing, keeping physical distance and staying inside home are devised and finally a composite normalised index is created to judge the overall status of the households.Results: This composite index is robust and sensitive enough. It is found to related with the level of living of the households, household size and some other social parameters.Conclusion: Respondent households being mostly from Kolkata nearby areas (Howrah, Hooghly, 24 PGS (N) & (S)) of Hindu, General and largely (two-thirds) belonging to 'lower-middle class' (family income per month 15000 or less) can be very well assumed as a representative sample of whole west Bengal. With the help of this composite index we may identify the status of any household in the spectrum of plausibility to adhere the guidelines and public health educative measures can be taken for those households in the long run.

    Kurzfassung: Flagellaria indica (Family: Flagellariaceae) is a common climbing plant found primarily in tropical regions of many countries. The plant has various traditional uses, although most of them lack scientific published reports. The crude ethanolic extract of F. indica leaves was fractionated based on polarity using water, ethyl acetate, and n-hexane. Biological screening was conducted on the anthelmintic, blood coagulation, diuretic, and laxative activities of the water, ethyl acetate, and n-hexane fractions of F. indica leaves. In the anthelmintic test, the n-hexane fraction showed a moderate effect with paralysis times of 16.79 and 13.62 minutes and death times of 27.34 and 21.81 minutes, respectively, at doses of 25 and 50 mg/mL.In the blood coagulant test, only the water fraction showed a notable effect. The clotting times were 4.33, 6.02, 7.68, and 8.32 minutes, respectively, at doses of 200, 100, 50, and 25 mg/mL. Diuretic activity was performed to determine the increase in the volume of excreted urine, and electrolyte analysis of urine was performed to determine pH, density, conductance, and Na+, Cl-, K+ levels, as well as natriuretic, kaliuretic, saluretic, and CAI indexes. The ethyl acetate fraction showed better diuretic activity than the n-hexane fraction, while the water fraction did not reveal a notable diuretic effect. The Na+, Cl-, K+, natriuretic, and saluretic indexes were found satisfactory in the ethyl acetate fraction, and the CAI index was better in the n-hexane fraction. In the laxative test, the n-hexane fraction showed the best laxative properties, with an increase in stool weight of 38% and 54% at doses of 250 and 500 mg/kg, respectively. These results suggest that different fractions of F. indica leaves contain distinct phytochemicals that may be responsible for these biological effects. The isolation of bioactive compounds could help justify its traditional uses in modern medicine.

    Kurzfassung: The project work was designed to investigate the phytochemical and selected pharmacological activities (anti-diarrheal, analgesic, neuropharmacological behavior and anthelmintic) of leaves of Sanchezia nobilis. Hook. F. (Family: Acanthaceae). From its phytochemical analysis we found the presence of reducing sugar, combined reducing sugar, phenolic compounds, tannins, flavonoids, carbohydrates, gums, steroids, alkaloids, glycosides and terpenoids. In vivo antidiarrheal activity was substantiated by prolongation of latent period and decrease in total number of stools. The extract produced 62.49% and 74.01% decrease in stool count at the doses of 250 and 500 mg/kg body weight respectively while the standard drug Loperamide decrease in stool count was found to be 87.05% at a dose of 3 mg/kg body weight. The leaves extract produced 32.7% and 41.78% inhibition of writhing at the doses of 250 mg/kg and 500 mg/kg body weight respectively while the standard drug Diclofenac Na was found to be 74.23% at a dose of 25 mg/kg body weight. The extract showed dose dependent CNS depressant activity by reducing the locomotors activity. Higher dose of this extract (500 mg/kg body weight) comparing with standard Diazepam exposed sedative effect potentially. The extract exhibited concentration dependent anthelmintic activity against Paramphistomum cervi using Albendazole (15 mg/mL) as standard. The paralysis occurred between 9.00 to 83.91 min and death occurred between 11.33 to 86.33 min which were comparable to standard drug Albendazole. So, the present study concluded that the extract is fortified with significant antidiarrheal, CNS depression and anthelmintic activity and moderate analgesic activity.

    Kurzfassung: Background: Systemic lupus erythematosus is characterized by a wide range of symptoms, and hematological abnormalities are a typical complication (SLE). There are some that are immediately life-threatening and need immediate attention, and then there are those that require more careful thought. Objective: The purpose of this research was to better understand the hematological symptoms experienced by people with SLE. Method: This cross sectional study was carried out at tertiary hospital, Bangladesh from January 2021 to October 2022. Where a total of 100 Patients of SLE as diagnosed according to American college of Rheumatology (ACR) criteria were included in the study and patients with hematological problems for due to other diseases were excluded. Results: During the study, 21-25 years age group, 30% and 90% were female. Most common presenting complaints were arthralgia (82%) followed by malar rash(70%) and myalgia (66%). 25% had hypertension, 16% had autoimmune thyroids, 5% had DM, 40% had no history of previous. 80%were taken sterios, followed by 30% were taken Mycophenolate Mofetil, 25% were taken Cyclophosphamide, 9% were taken Methotrexate. In addition to that, Anemia was the most common hematological abnormality detected in 70% patients. Normocytic normochromic anemia (NNA) was the most common peripheral blood film (PBF) findings found in 36% of the patients followed by microcytic hypochromicanemia (MHA) 30%. A positive direct Coomb’s test was found in 40% of the patients and antiphospholipid antibody (APLA) was positive in 20% of the patients. Conclusion: Patients with SLE often have haematological abnormalities. Differentiating haematological abnormalities as a symptom of SLE, a side effect of SLE therapy, or part of another blood dyscrasia is essential.

    Kurzfassung: Background and objective: Eyelid growth is a common clinical condition presented to ophthalmologists. Accurate diagnosis of eyelid tumors is necessary to guide ophthalmologists to design optimal management. We carried out the study to assess the histopathological types of different eyelid growth in tertiarycare hospitals of Dhaka city.Methods: This is a cross sectional study performed at the Department of Pathology of Sir Salimullah Medical College (SSMC), Dhaka, Bangladesh. Samples were collected from hospitals of SSMC and National Institute of Ophthalmology (NIO), Dhaka, Bangladesh. Study period was from January 2012 to December 2013. A total of 93 cases with eyelid growth of both sex were enrolled in the study. After obtaining informed written consent, tumors were excised by the ophthalmologist and the specimens were collected in 10% formalin for histopathological examination.Results: A total of 93 cases of eyelid lesions were examined. The most common age group affected was between 26-50yrs (50.54%). Mean age was 43.22±17.42 (range 19 90 years). Gender distribution of the patients was almost equal (male 51.6%, female 48.4%). Neoplastic lesions were found in 86 cases (92.47%) and non neoplastic growth was present in 7 (7.53%) cases. Benign, pre-malignant and malignant tumors were found in 52 (55.91%), 01(1.08%) and 33(35.48%) cases respectively. Among the malignant lesions, basal cell carcinoma was the most common malignant tumor (36.4%) followed by sebaceous gland and squamous cell carcinoma (27.3%). Nevus was the most common benign lesions (26.9%) followed by sudoriferous cyst (19.2%) and haemangioma (15.4%).Conclusions: All the eyelid lesions removed surgically should be examined histopathologically to establish the correct diagnosis. Accurate diagnosis of specific tumors is important for proper treatment and favorable prognosis.IMC J Med Sci 2017; 11(1): 5-10
