Zusammenfassung Aufgrund der dargelegten Ergebnisse muß die Calciummangeltheorie der Osteoporose in ihrer Allgemeingültigkeit abgelehnt werden. Enterale Calciumresorption und zeitliche Änderung der Hydroxylapatitkonzentration im Knochen nehmen physiologischerweise mit zunehmendem Lebensalter ab. Zwischen beiden Größen besteht eine proportionale Beziehung. In der I. Phase der Osteoporose, der Knochenabbau-Phase, ist diese Proportionalität aufgehoben, der Abfall der Hydroxylapatitkonzentration im Knochen erfolgt wesentlich ausgeprägter als die Verminderung der enteralen Calciumresorption. Somit kann die Verminderung der Calciumresorption nicht Ursache der Osteoporose sein, sie ist vielmehr als Anpassung an einen geringeren Calciumbedarf aufzufassen. Hierbei muß die enterale Resorptionsdrosselung mitunter durch eine temporäre Hypercalciurie unterstützt werden, um die aus dem substanzabbauenden Knochen stammende erhöhte Calciumzufuhr im Calciumpool auszugleichen. Ein neues, pathogenetisch orientiertes Einteilungsschema der Osteoporose wird vorgeschlagen.

Summary In accord with the results presented, the generally prevailing theory of calcium deficiency as the cause of osteoporosis must be discarded. With increasing age both the enteral resorption of calcium and the concentration of hydroxylapatite in bone undergo physiologic decrease, both factors being in a proportional relationship. This proportionality is suppressed in the first phase of osteoporosis where reduction of bone substance takes place: the concentration of hydroxylapatite of the bones decreases more notably than the enteral resorption of calcium. Accordingly, the decrease of enteral calcium resorption cannot be considered as the cause of osteoporosis, but is rather the expression of an adaptation to reduced calcium requirement. In order to regulate the increased calcium supply (originating from the dissolution of bone substance) to the calcium pool, a temporary calcium hyperuresis combines with the decreased enteral calcium absorption. A new classification of osteoporosis, based on the pathogenesis, is proposed.
