Summary A legal context that classifies the consumption of heroin and cocaine as an illegal act, poses a considerable methodological challenge to research on users of these substances. This is in particular the case for research on those users who are not in treatment and, therfore, cannot be recruited through treatment settings. In a research project on heroin and/or cocaine users outside treatment settings, a sample of 917 individuals was recruited through “Privileged Access Interviewers” in the whole of Switzerland. In the first part of this article, we discuss matters of reliability as well as of validity conceming this method of data collection. In the second part of the article, we discuss the use of low threshold syringe exchanges schemes by the user groups represented in the sample. Only intravenous drug users frequent those services — they are however a minority in the sample (n=238). In several regions of Switzerland syringe exchange schemes do not exist. Where they do exist, they appear to correspond to a need which they are able to cover largely. In the regions without such services, intravenous drug users get their supply of syringes more frequently from pharmacies. However, pharmacies do not compensate the absence of specific syringe exchange schemes. In regions without such schemes, injections with used syringes are more frequent. Thus, regarding Aids-Prevention, there is an urgent need to develop syringe exchange schemes in all parts of the country.

Zusammenfassung In einem juristischen Kontext, der den Konsum von Heroin und Kokain verbietet, stösst die Durchführung von Erhebungen bei Konsumentinnen dieser Substanzen auf erhebliche methodische Schwierigkeiten. Dies ist insbesondere der Fall bei den Konsumentinnen, die sich nicht in Behandlung befinden und deshalb nicht über Betreuungsstrukturen rekrutiert werden können. Im Rahmen eines Forschungsprojekts über Heroin-bzw. Kokainkonsumierende ausserhalb medizinischer Betreuung wurden mittels “privileged access interviewers” in der Schweiz ein Sample von 917 Individuen rekrutiert. In einem ersten Teil des vorliegenden Artikels werden Aspekte der Zuverlässigkeit und der Gültigkeit dieser Erhebungsmethode diskutiert. Der zweite Teil des Artikels beinhaltet Analysen über die Benutzung von niederschwelligen Spritzentausch- und-verteilstellen durch die im Sample erfassten Konsumentinnengruppen. Es sind vor allem intravenös Konsumierende (n=238), die solche Hilfsangebote nutzen. In gewissen Regionen der Schweiz existieren bekanntlich keine Spritzenabgabestellen. Dort, wo es sie gibt scheinen sie hingegen einem Bedürfnis zu entsprechen, das sie abdecken können. In Regionen ohne solche Angebote holen die Konsumierenden ihre Spritzen vermehrt in Apotheken, die jedoch das Fehlen von spezifischen Abgabestellen nicht kompensieren. In diesen Regionen sind Injektionen mit gebrauchten Spritzen häufiger als in Regionen, wo Spritzentausch- und-abgabestellen existieren. Im Hinblick auf die Aids-Prävention sollten deshalb diese Hilfsangebote in der ganzen Schweiz ausgebaut werden.

Résumé Dans le contexte d'une législation qui interdit la consommation d'héroïne et de cocaïne, mener des enquêtes auprès des consommateurs de ces substances pose des problèmes méthodologiques considérables. Ceci est particulièrement vrai pour les consommateurs hors traitement qui ne peuvent être recrutés à travers les structures de prise en charge. Dans une recherche sur les consommateurs d'héroïne etlou de cocaïne hors traitement médical, un échantillon de 917 individus fut constitué par la méthode des “interlocuteurs privilégiés” sur l'ensemble du territoire national. Les problèmes de fiabilité et de validité de cette méthode sont discutés dans une première partie de l'article. Ensuite, nous discutons, à partir de cet échantillon de 917 personnes, des aspects touchant à l'utilisation des services d'échange et de distribution de matériel d'injection stérile. Tout en étant minoritaires dans l'échantillon (n=238), ce sont avant tout les personnes consommant par voie intraveineuse qui utilisent ces services. Il apparaît que la principale raison de non-utilisation par les injecteurs est l'absence de tels services dans certaines régions du pays. Là où ils existent, ces services correspondent donc à un besoin qu'ils arrivent à couvrir. Dans les régions sans services d'échange ou de distribution de matériel d'injection, les pharmacies sont davantage sollicitées pour l'approvisionnement en seringues stériles, sans pour autant compenser l'absence des services à bas seuil. Dans ces régions, les injections avec des seringues déjà utilisées par quelqu'un d'autre sont plus élevée. Du point de vue de la prévention du sida, il y a donc urgence de développer des services d'échange et de distribution de matériel d'injection partout en Suisse.
