Résumé Nous avons envoyé des questionnaires à 519 médecins établis à Munich, dont 323 omnipraticiens, 143 spécialistes en médecine interne et 55 spécialistes en affections nerveuses. 185 médecins ont répondu à ce questionnaire (105 omnipraticiens = 32%, 50 spécialistes en médecine interne = 35%, 25 spécialistes en affections nerveuses = 45%). Le dépouillement des réponses a confirmé l'hypothèse selon laquelle il y aurait des différences dans l'appréciation, par les médecins des diverses spécialités, des méthodes thérapeutiques et des difficultés rencontrées dans le traitement, et cela surtout entre les spécialistes en affections nerveuses et les autres médecins. Tandis que la thérapie médicamenteuse est utilisée à peu près dans la même proportion par les 3 groupes de médecins, les spécialistes en affections nerveuses faisaient plus souvent admettre des patients dans des institutions spécialisées et utilisaient aussi plus souvent des méthodes psychothérapeutiques. Cependant, sur le plan du nombre absolu, ces dernières étaient rarement utilisées. Les spécialistes en affections nerveuses collaboraient aussi d'une manière plus intensive avec les organismes sociaux que les médecins des autres groupes. Il n'y a pas de différence essentielle en ce qui concerne leur opinion quant au pronostic parmi les 3 groupes de médecins mentionnés; de même en ce qui concerne la fréquence des alcooliques dans leur clientèle. On peut tout au plus dire que les médecins qui soignent entre 6 et 10 alcooliques par trimestre déploient l'activité thérapeutique la plus grande. Les médecins de tous les groupes sont d'accord pour dire que la difficulté la plus grande, rencontrée dans la thérapie des alcooliques, réside dans le manque de coopération de leurs patients. Ils mentionnent aussi souvent la collaboration réduite des membres de la famille et les possibilités restreintes de thérapie en institution.

Zusammenfassung An 519 niedergelassene Ärzte Münchens (323 Ärzte für Allgemeinmedizin, 141 Fachärzte für innere Medizin und 55 Nervenärzte) wurden Fragebögen versandt. 185 Ärzte (35%) beantworteten diese Rundfrage (105 Ärzte für Allgemeinmedizin=32%, 50 Fachärzte für innere Medizin = 35% und 25 Nervenärzte=45%). Die Auswertung dieser Beantwortung bestätigte die Hypothese, daß unter den Ärzten der verschiedenen Fachgruppen hinsichtlich der therapeutischen Methoden und der angegebenen Schwierigkeiten in der Behandlung Unterschiede bestehen, und zwar in erster Linie zwischen den Nervenärzten einerseits und den Ärzten für Allgemeinmedizin und für innere Medizin andererseits. Während die medikamentöse Therapie bei allen 3 Ärztegruppen etwa in gleichem Umfang angewandt wird, nehmen die Nervenärzte häufiger Überweisungen in stationäre Behandlungseinrichtungen vor und wenden auch etwas öfter psychotherapeutische Verfahren an, die aber, absolut gesehen, selten eingesetzt werden. Insbesondere arbeiten Nervenärzte mit den Fürsorgeinstitutionen wesentlich intensiver zusammen als die Ärzte sonstiger Fachdisziplinen. In der Prognose stellung unterscheiden sich die 3 Ärztegruppen nicht wesentlich, ebensowenig fanden sich wesentliche Unterschiede, wenn man die Ärzte nach der Frequenz der Alkoholiker in der Praxis gruppiert. Es läßt sich hier höchstens sagen, daß die Ärzte, die 6–10 Alkoholiker pro Quartal behandeln, die meiste therapeutische Aktivität entfalten. Als Hauptschwierigkeit für die Therapie der Alkoholiker wird von allen Ärztegruppen übereinstimmend die mangelnde Kooperationsbereitschaft der Patienten angegeben. Des weiteren wird besonders häufig auf die geringe Mitarbeit der Angehörigen und den Mangel an stationären Behandlungsmöglichkeiten hingewiesen.

Summary A survey of the treatment of alcoholics by medical practitioners in Munich was conducted. Five hundred nineteen licensed physicians, 323 in general practice, 143 internists, and 55 neuropsychiatrists were sent questionnaires. One hundred eighty-five physicians, or 35%, responded: 110 general practitioners, 50 internists, and 25 neuropsychiatrists. The questionnaire asked the physicians to estimate the frequency with which they were consulted by alcoholic patients and to express their opinion about various treatments and prognosis. Analysis of the responses confirms the hypothesis that physicians differ in their therapeutic methods and in their estimates as to the difficulty in treatment and outcome according to specialties. The greatest difference exists between neuropsychiatrists on the one hand, and general practitioners and internists on the other. Whereas all three groups use pharmacological treatments approximately to the same extent, neuropsychiatrists resort to inpatient treatment significantly more often and also more often use psychotherapeutic methods. The total number of patients treated in psychotherapy, however, is small. Neuropsychiatrists also work more frequently with community agencies than do the other doctors. In estimating prognosis there is no significant difference among the three groups, nor was there any significant difference in the frequency with which they saw alcoholic patients. In all groups it appeared that those physicians who saw 6 to 10 alcoholic patients per three months, or more, also seemed to have the most therapeutic stance towards them. All physicians agreed that the greatest difficulty in treting alcoholics is their low level of motivation for cooperation in treatment. Next to this, the most common factor cited as a difficulty in treatment was the low level of cooperation and motivation for treatment on the part of the patients' immediate families.
