    Notes: Background:  Improving quality of life is considered to be a major endpoint and motivation for clinical intervention in patients with perennial allergic rhinitis (PAR). In addition to classical symptoms of congestion, pruritus, and rhinorrhea, patients will often complain of not being able to sleep well at night and of feeling fatigued during the day. Like sleep apnea, PAR has also been shown to cause sleep disturbance and consequently worsen daytime fatigue and somnolence.Hypothesis: It is proposed that by decreasing nasal obstruction due to allergic rhinitis by treating with the topical steroid budesonide, symptoms of daytime fatigue and somnolence can be improved.Methods: Twenty-two subjects were enrolled in a double-blind, placebo-controlled, crossover study using Baalam's design. Patients were treated with either budesonide 128 μg/day or placebo. Subjective data include the Epworth Sleepiness Scale, Functional Outcomes of Sleep Questionnaire, Rhino-conjunctivitis Quality of Life Questionnaire, and a daily diary recording nasal symptoms, sleep problems, and daytime fatigue.Results: The results illustrated that the topical nasal corticosteroid significantly improved daytime fatigue (P = 0.03), somnolence (P = 0.02), and quality of sleep (P = 0.05) compared to placebo in patients suffering from PAR.Summary: Budesonide is able to improve congestion, sleep, and daytime somnolence.

    Description: Great subduction earthquakes exhibit segmentation both within the rupture of individual events and in the long term history of the margin. The 2004 December 26 Aceh-Andaman and 2005 March 28 Nias event in northern Sumatra are two of the largest earthquakes in recent years, with both co- and post-seismic displacements constrained in unprecedented detail. Using aftershock locations from a temporary seismic array in the boundary region between both events and waveform modelling of large aftershocks, we demonstrate that the vast majority of aftershocks in the study region occur on the plate interface within a narrow band ( 20 km) seaward of the outer arc high. Comparing the seismicity distribution to the co- and post-seismic displacements, we infer that the seismic band marks the transition between the seismogenic zone and stable sliding. The location of the band and therefore the transition appears to be correlated with the ∼500 m bathymetry contour. This close correspondence is disrupted at the boundary between the two great earthquakes, where the transition to seismogenic behaviour occurs further landward by ∼25 km. To the west of Simeulue, where seafloor bathymetry throughout the forearc is deeper than 500 m, the seismic band terminates abruptly and the focus of aftershock activity is found near the trench. The seismic efficiency of afterslip varies dramatically along strike: the segment below the Banyak islands, in the gap between the two main asperities of the Nias earthquake, accommodates a much larger proportion of afterslip seismically.

    Description: We use body-waveform modelling to constrain the source parameters of earthquakes occurring globally in oceanic lithosphere beneath the subduction zone outer rise and outer trench slope. These data are then used to map the stress state in the lithosphere of the downgoing plate as it bends into the subduction zone. Our results provide new constraints on the faulting of oceanic lithosphere at the outer rise, which is important for understanding the transmission of plate-driving forces through the subduction system. In all cases, shallow normal-faulting earthquakes are observed at the top of the plate, and are separated in depth from any deeper thrust-faulting earthquakes. No temporal variation associated with large thrust-faulting earthquakes on the subduction interface is seen in the depth extent of each type of faulting at the outer rise. The transition depth from trench-normal extension to compression is found to vary in agreement with models in which deformation is driven by the combination of in-plane stresses and bending stresses, resulting principally from slab pull. Combining the seismologically derived constraints on the thickness of the elastic core of the plate with estimates of the plate curvature, we place upper bounds on the strength of the lithosphere at the outer rise, which is required to be 300 MPa for a constant yield stress model, or governed by an effective coefficient of friction of 0.3.

    Description: Objective Our primary objective was to identify cognitive behavioural therapy (CBT) delivery for people with psychosis (CBTp) using an automated method in a large electronic health record database. We also examined what proportion of service users with a diagnosis of psychosis were recorded as having received CBTp within their episode of care during defined time periods provided by early intervention or promoting recovery community services for people with psychosis, compared with published audits and whether demographic characteristics differentially predicted the receipt of CBTp. Methods Both free text using natural language processing (NLP) techniques and structured methods of identifying CBTp were combined and evaluated for positive predictive value (PPV) and sensitivity. Using inclusion criteria from two published audits, we identified anonymised cross-sectional samples of 2579 and 2308 service users respectively with a case note diagnosis of schizophrenia or psychosis for further analysis. Results The method achieved PPV of 95% and sensitivity of 96%. Using the National Audit of Schizophrenia 2 criteria, 34.6% service users were identified as ever having received at least one session and 26.4% at least two sessions of CBTp; these are higher percentages than previously reported by manual audit of a sample from the same trust that returned 20.0%. In the fully adjusted analysis, CBTp receipt was significantly (p〈0.05) more likely in younger patients, in white and other when compared with black ethnic groups and patients with a diagnosis of other schizophrenia spectrum and schizoaffective disorder when compared with schizophrenia. Conclusions The methods presented here provided a potential method for evaluating delivery of CBTp on a large scale, providing more scope for routine monitoring, cross-site comparisons and the promotion of equitable access.

    Description: Homeostatic scaling allows neurons to alter synaptic transmission to compensate for changes in network activity. Here, we show that suppression of network activity with tetrodotoxin, which increases surface expression of AMPA receptors (AMPARs), dramatically reduces levels of the deSUMOylating (where SUMO is small ubiquitin-like modifier) enzyme SENP1, leading to a consequent increase in protein SUMOylation. Overexpression of the catalytic domain of SENP1 prevents this scaling effect, and we identify Arc as a SUMO substrate involved in the tetrodotoxin-induced increase in AMPAR surface expression. Thus, protein SUMOylation plays an important and previously unsuspected role in synaptic trafficking of AMPARs that underlies homeostatic scaling.
