    Notes: Summary.  A haemophilia-specific health-related quality-of-life questionnaire (named ‘Hemofilia-QoL’) was developed to assess quality-of-life in adults with haemophilia, and was psychometrically tested. Seventy-three interviews with haemophilia patients and health care professionals were used to generate the items included in the questionnaire, and expert ratings on the items formulated were used to screen them for potential omission. This was followed by psychometric testing in a sample of 35 patients. Preliminary psychometric testing of the revised questionnaire version, which contains 10 domains (physical health, physical role, joint damage, pain, treatment satisfaction, emotional role, mental health, social support), showed acceptable reliability (α = 0.94 for the Hemofilia-QoL total score) and validity, and this will be examined in a subsequent study with a larger patient sample.

    Description / Table of Contents: Abstract On the basis of Rb-Sr muscovite dating an age of about 355 Ma (Late Tournaisian/Early Visean) is assigned to the M1 metamorphism and associated mylonitization of the ultramylonites of the Las Grullas Formation in the Azuaga Group, Badajóz-Córdoba belt. This age may possibly be related to the accretion process (suturing of the Ossa-Morena Zone to the Central Iberian Zone) thought to be responsible for the metamorphism and mylonitization. Muscovite from the Valencia de Las Torres Formation displays a somewhat lower Rb-Sr age of about 337 Ma. The corresponding K-Ar ages indicate that after the termination of the metamorphism it took some 20 Ma before the rocks had cooled below the closure temperature of muscovite to K-Ar. Whole-rock Rb-Sr data indicate that the Las Grullas ultramylonites were probably derived from Ordovician granitoids.
    Abstract: Resumen Dataciones Rb-Sr en moscovitas de alrededor de 355 Ma (Tournaisiense tardío a Viseense) se asignan al metamorfismo M1 y se asocian a la milonitización de ultramilonitas de la Formación Las Grullas en el Grupo de Azuaga, Eje Badajóz-Córdoba. Esta edad podría posiblemente datar el proceso de acreción (sutura de la Zona de Ossa-Morena a la Zona Centro-Ibérica) interpretado como responsable del metamorfismo y milonitización. Moscovita de la Formación Valencia de Las Torres indice una edad Rb-Sr un poco más bajo de cerca de 337 Ma. Las correspondientes edades K-Ar indican que después de la terminación del metamorfismo, se tardó cerca de 20 Ma antes que las rocas se enfriaran por debajo de la temperatura de cierre de la moscovita para K-Ar. Datos Rb-Sr de roca total indican que las ultramilonitas de Las Grullas posiblemente se deriven de granitoides ordovicicos.

Dieses Alter könnte mit dem Akkretionsproze\ (Sutur der Ossa-Morena-Zone zur Zentral-Iberischen-Zone) verbunden sein, der für Metamorphose und Mylonitisierung verantwortlich gemacht wird. Muskovit der Valencia de Las Torres-Formation spiegelt ein etwas geringeres Rb-Sr-Alter von ca. 337 Ma wieder.     Description / Table of Contents: Abstract A Rb-Sr whole-rock investigation of suites of samples from two orthogneiss bodies in the Almendralejo area of the Oporto-Portalegre-Badajoz-Córdoba belt points to an Ordovician age for the granitic magmatism: about 470 Ma for the Almendralejo Gneiss (peralkaline metagranite) and about 425 Ma for the Ribera del Fresno Gneiss (leucocratic subaluminous metagranite), both with high initial87Sr/86Sr ratios. This points to a post-Ordovician (Hercynian) age for the metamorphism and tectonic deformation that affected the area, including the gneisses, and rules out the alleged Cadomian or even older age.
    Abstract: Résumé Une étude par Rb-Sr sur roche totale de séries d'échantillons de deux massifs d'orthogneiss de la région d'Almendralejo, dans la ceinture Oporto-Portalegre-Badajoz-Cordoue, indique un âge ordovicien pour le magmatisme granitique: environ 470 Ma pour le gneiss d'Almendralejo (métagranite peralcalin) et environ 425 Ma pour le gneiss de Ribera del Fresno (métagranite leucocrate subalumineux), tous deux avec des rapports initiaux87Sr/86Sr élevés. Ces résultats indiquent un âge post-ordovicien (hercynien) pour le métamorphisme et la déformation tectonique qui ont affecté la région, y compris les gneiss, et éliminent l'hypothèse proposée par ailleurs d'un âge cadomien ou même plus ancien.

    Notes: Abstract  A phase II study was performed to assess the efficacy and toxicity of the combination of cisplatin (CDDP) and tamoxifen (TAM) in patients with metastatic malignant melanoma (MM). A total of 31 consecutive previously untreated patients with unresectable measurable MM were given 100 mg/m2 CDDP every 21 days and 60 mg TAM every 12 h daily. All courses were given on an outpatient basis. A total of 119 courses of treatment were given. In all, 5 of the 31 patients (16%) had an objective response (95% confidence interval 5.3 – 34%) and 2 (6%) achieved a clinical complete response. The median duration of response was 7 months. The main side effect was gastrointestinal: 13% of the patients experienced grade 3/4 nausea/vomiting. Hematological or neurological toxicities were mild and rare. In conclusion, the combination CDDP-TAM has limited activity in MM, although its toxicity is tolerable. Our results do not allow us to recommend its use for the treatment of MM.

    Notes: Abstract Background: The aim of this work was to study hemostasis in laparoscopic cholecystectomy in order to determine if there are any changes that indicate a greater risk of thrombosis. Methods: The study was carried out in 20 patients who underwent laparoscopic surgery for noncomplicated cholelithiasis. The average age was 59.4 years (range, 34–77). A total of 75% were female. Mean operation time was 70 min (ranges 35–120). Pneumoperitoneum at 14 mmHg was performed on all patients, who were positioned in the 30° reverse Trendelenburg position. Postoperative mobilization was acheived in 24 hs and patients were discharged 48 hs after the operation. The control group was composed of 12 patients, who were evenly distributed by age, sex, and length of surgery. These patients underwent Bassini herniorraphy for inguinal hernia without any complications or relapse. The following hemostatic parameters were studied: prothrombin activity (PA), activated partial thromboplastin time (APTT), fibinogen (Fg), anti-thrombin III (ATIII), plasma fibrinolytic activity (PFA), euglobulin fibrinolytic activity (EFA), and D-dimer (D-D). Samples were obtained at the following times: (a) under basal conditions the day before surgery, (b) preoperatively, (c) at the end of the operation, (d) 24 hs after the operation, and (e) On the 7th day following the operation. Results: No patient showed any clinical manifestations of thromboembolic disease immediately after surgery or during a medium follow-up period of 16 months (range, 15–18 months). All hemostatic parameters values were within normal range in the basal samples of both groups. In both groups, the mean value of PA showed a significant decrease (p 〈 0.05) in the second, third, and fourth basal samples, returning to normal levels by the fifth determination. The mean value of fibrinogen decreased slightly in the second and third samples, increasing significantly with respect to the fourth and fifth determinations in both groups (p 〈 0.05). The mean value of APTT in both groups was slightly enhanced in the second and third determinations in relation to the first and fifth. The global activity of fibrinolysis (PFA and EFA) increased significantly in the third sample with respect to the other determinations in the group who had laparoscopic surgery (p 〈 0.005). Only EFA increased in the control group (p 〈 0.05). D-D decreased in the preoperative second determination followed by a significant enhancement immediate postoperatively (third), and 24 hs (fourth) (p 〈 0.05); it returned to normal basal values on the seventh day. No significant differences were found between the two groups. Conclusions: These results indicate that laparoscopic cholecystectomy leads to no greater activation of plasma coagulation than low-risk surgery. On the contrary, the increase of fibrinolytic activity in plasma would extend a certain degree of hypocoagulability during surgery, maintaining it for 24 hs and thus possibly reducing thromboembolic risk in patients undergoing this type of surgery.

    Description / Table of Contents: Résumé L'analyse d'échantillons prélevés dans les séries marneuses du Néogène et du Crétacé des bassins de Grenade et de Guadix (sud-est de l'Espagne) montre une corrélation inverse entre la teneur en argile et l'angle de frottement résiduel. Il a été vérifié que la teneur en carbonates de ces sols a une influence significative sur l'angle de frottement résiduel ; il existe un seuil de 25% de carbonates, au-dessus duquel la valeur de φ′r augmente de 20 à 30°. Il a été prouvé que la présence de smectite comme minéral dominant de la fraction argileuse est la cause d'une faible valeur de résistance résiduelle au cisaillement, ainsi que d'une grande fragilité du matériau dans des essais non consolidés non drainés. Il a été prouvé que le degré de compacité de ces sols et, de ce fait, la porosité initiale, a une influence directe sur l'angle de frottement résiduel φ′r.
    Notes: Abstract The analysis of samples taken from the Neogene and Cretaceous marly series of the Granada and Guadix basins (south east Spain) shows an inverse correlation between the clay content and the residual friction angle. It has been verified that the carbonate fraction of the soil has a significant influence on the residual friction angle; there is a threshold of 25% carbonates, above which the value of φ′r increases from 20 to 30°. It has been proved that the presence of smectitic clay as the dominant constituent produces a low value of residual shear strength, associated with high fragility in unconsolidated undrained tests. It has been proved that the degree of compaction of the soil and therefore the initial porosity, has a direct influence on the effective residual friction angle (φ′r).

    Description: 1. Reliable determination of organisms is a prerequisite to explore their spatial and temporal occurrence and to study their evolution, ecology, and dispersal. In Europe, Bavaria (Germany) provides an excellent study system for research on the origin and diversification of freshwater organisms including dinophytes, due to the presence of extensive lake districts and ice age river valleys. Bavarian freshwater environments are ecologically diverse and range from deep nutrient‐poor mountain lakes to shallow nutrient‐rich lakes and ponds. 2. We obtained amplicon sequence data (V4 region of small subunit‐rRNA, c. 410 bp long) from environmental samples collected at 11 sites in Upper Bavaria. We found 186 operational taxonomic units (OTUs) associated with Dinophyceae that were further classified by means of a phylogenetic placement approach. 3. The maximum likelihood tree inferred from a well‐curated reference alignment comprised a systematically representative set of 251 dinophytes, covering the currently known molecular diversity and OTUs linked to type material if possible. Environmental OTUs were scattered across the reference tree, but accumulated mostly in freshwater lineages, with 79% of OTUs placed in either Apocalathium, Ceratium, or Peridinium, the most frequently encountered taxa in Bavaria based on morphology. 4. Twenty‐one Bavarian OTUs showed identical sequences to already known and vouchered accessions, two of which are linked to type material, namely Palatinus apiculatus and Theleodinium calcisporum. Particularly within Peridiniaceae, delimitation of Peridinium species was based on the intraspecific sequence variation. 5. Our approach indicates that high‐throughput sequencing of environmental samples is effective for reliable determination of dinophyte species in Bavarian lakes. We further discuss the importance of well‐curated reference databases that remain to be developed in the future.

    Description: During cell division, a mitotic spindle is built by the cell and acts to align and stretch duplicated sister chromosomes before their ultimate segregation into daughter cells. Stretching of the pericentromeric chromatin during metaphase is thought to generate a tension-based signal that promotes proper chromosome segregation. However, it is not known whether the mitotic spindle actively maintains a set point tension magnitude for properly attached sister chromosomes to facilitate robust mechanochemical checkpoint signaling. By imaging and tracking the thermal movements of pericentromeric fluorescent markers in Saccharomyces cerevisiae , we measured pericentromere stiffness and then used the stiffness measurements to quantitatively evaluate the tension generated by pericentromere stretch during metaphase in wild-type cells and in mutants with disrupted chromosome structure. We found that pericentromere tension in yeast is substantial (4–6 pN) and is tightly self-regulated by the mitotic spindle: through adjustments in spindle structure, the cell maintains wild-type tension magnitudes even when pericentromere stiffness is disrupted.

    Description: Purpose: Cultured tumor fragments from melanoma metastases have been used for years as a source of tumor-infiltrating lymphocytes (TIL) for adoptive cell therapy (ACT). The expansion of tumor-reactive CD8 + T cells with interleukin-2 (IL2) in these early cultures is critical in generating clinically active TIL infusion products, with a population of activated 4-1BB CD8 + T cells recently found to constitute the majority of tumor-specific T cells. Experimental Design: We used an agonistic anti–4-1BB antibody added during the initial tumor fragment cultures to provide in situ 4-1BB costimulation. Results: We found that addition of an agonistic anti–4-1BB antibody could activate 4-1BB signaling within early cultured tumor fragments and accelerated the rate of memory CD8 + TIL outgrowth that were highly enriched for melanoma antigen specificity. This was associated with NFB activation and the induction of T-cell survival and memory genes, as well as enhanced IL2 responsiveness, in the CD8 + T cells in the fragments and emerging from the fragments. Early provision of 4-1BB costimulation also affected the dendritic cells (DC) by activating NFB in DC and promoting their maturation inside the tumor fragments. Blocking HLA class I prevented the enhanced outgrowth of CD8 + T cells with anti–4-1BB, suggesting that an ongoing HLA class I–mediated antigen presentation in early tumor fragment cultures plays a role in mediating tumor-specific CD8 + TIL outgrowth. Conclusions: Our results highlight a previously unrecognized concept in TIL ACT that the tumor microenvironment can be dynamically regulated in the initial tumor fragment cultures to regulate the types of T cells expanded and their functional characteristics.     Description: Purpose: Hormone receptor–positive (HR + ) breast cancer is clinically and biologically heterogeneous, and subgroups with different prognostic and treatment sensitivities need to be identified. Experimental Design: Research-based PAM50 subtyping and expression of additional genes was performed on 63 patients with HR + /HER2 – disease randomly assigned to neoadjuvant multiagent chemotherapy versus endocrine therapy in a phase II trial. The biology associated with treatment response was used to derive a PAM50-based chemoendocrine score (CES). CES's predictive ability was evaluated in 4 independent neoadjuvant data sets ( n = 675) and 4 adjuvant data sets ( n = 1,505). The association of CES, intrinsic biology, and PAM50 risk of relapse (ROR) was explored across 6,007 tumors. Results: Most genes associated with endocrine sensitivity were also found associated with chemotherapy resistance. In the chemotherapy test/validation data sets, CES was independently associated with pathologic complete response (pCR), even after adjusting for intrinsic subtype. pCR rates of the CES endocrine–sensitive (CES-E), uncertain (CES-U), and chemotherapy-sensitive (CES-C) groups in both data sets combined were 25%, 11%, and 2%, respectively. In the endocrine test/validation data sets, CES was independently associated with response. Compared with ROR, 〉90% of ROR-low and ROR-high tumors were identified as CES-E and CES-C, respectively; however, each CES group represented 〉25% of ROR-intermediate disease. In terms of survival outcome, CES-C was associated with poor relapse-free survival in patients with ROR-intermediate disease treated with either adjuvant endocrine therapy only or no adjuvant systemic therapy, but not in patients treated with (neo)adjuvant chemotherapy. Conclusions: CES is a genomic signature capable of estimating chemoendocrine sensitivity in HR + breast cancer beyond intrinsic subtype and risk of relapse.
