    Beschreibung: We sought to evaluate the accuracy of myocardial perfusion imaging (MPI) using cadmium-zinc-telluride (CZT) SPECT cameras for the measurement of left ventricular (LV) global and regional function, and the performance of absolute wall motion (WM) and wall thickness (WT) measurements for the detection of myocardial scarring, using cardiac MR as a reference. Methods: Sixty patients with known or suspected coronary artery disease underwent MPI using a CZT SPECT camera, as well as cine and delayed enhanced cardiac MR. Gated MPI data were processed using quantitative gated SPECT software. LV global function was assessed by measuring LV end-diastolic volume (EDV), end-systolic volume (ESV), and ejection fraction (EF). Regional function was assessed by quantifying segmental WM and WT according to the 17-segment model. Myocardial scarring was quantified on each segment using delayed enhanced cardiac MR. Agreement between SPECT and cardiac MR was assessed using Pearson correlation and Bland–Altman analyses. The influence of measurement magnitude on the agreement was analyzed using Spearman correlation coefficients between the mean and SD of measurements. The performance of WM and WT for the detection of segments with more than 25% scar transmurality was assessed using receiver-operating-characteristic analysis. Results: Correlation between methods was excellent for EF ( R = 0.81, P 〈 0.0001) and ESV ( R = 0.88, P 〈 0.0001) and was good for EDV ( R = 0.71, P 〈 0.0001) . Agreement was good for EF (bias, –2.7%; limits of agreement, –15.5 to +10.1) but was lower for EDV (bias, –29.7 mL; limits of agreement, –68.3 to +8.9) and ESV (bias, –9.9 mL; limits of agreement, –30.7 to +10.9). Correlation between methods was fair for WM ( R = 0.49, P 〈 0.0001) and WT ( R = 0.48, P 〈 0.0001). SPECT underestimated WT (bias, –41%; limits of agreement, –108 to +26), with an error depending on thickening magnitude ( = 0.70, P 〈 0.0001). The agreement in WM measurement was higher (bias, –1.4 mm; limits of agreement, –6.9 to +4) and independent of motion magnitude ( = 0.006, P = 0.86). WM and WT were equally able to identify scarred segments (area under the receiver-operating-characteristic curve, 0.74 ± 0.03 and 0.74 ± 0.03, respectively). Conclusion: MPI using CZT SPECT cameras and quantitative gated SPECT analysis accurately quantifies EF but still underestimates LV volumes. WM shows a higher agreement with cardiac MR than does WT, with errors in WT measurement increasing at greater thicknesses. Absolute quantification of segmental WM and WT can equally be used to identify myocardial scarring.

    Beschreibung: Background The standard of care for newly diagnosed glioblastoma is maximal safe surgical resection, followed by chemoradiation therapy. We assessed carmustine wafer implantation efficacy and safety when used in combination with standard care. Methods Included were adult patients with ( n = 354, implantation group) and without ( n = 433, standard group) carmustine wafer implantation during first surgical resection followed by chemoradiation standard protocol. Multivariate and case-matched analyses (controlled propensity-matched cohort, 262 pairs of patients) were conducted. Results The median progression-free survival was 12.0 months (95% CI: 10.7–12.6) in the implantation group and 10.0 months (9.0–10.0) in the standard group and the median overall survival was 20.4 months (19.0–22.7) and 18.0 months (17.0–19.0), respectively. Carmustine wafer implantation was independently associated with longer progression-free survival in patients with subtotal/total surgical resection in the whole series (adjusted hazard ratio [HR], 0.76 [95% CI: 0.63–0.92], P = .005) and after propensity matching (HR, 0.74 [95% CI: 0.60–0.92], P = .008), whereas no significant difference was found for overall survival (HR, 0.95 [0.80–1.13], P = .574; HR, 1.06 [0.87–1.29], P = .561, respectively). Surgical resection at progression whether alone or combined with carmustine wafer implantation was independently associated with longer overall survival in the whole series (HR, 0.58 [0.44–0.76], P 〈 .0001; HR, 0.54 [0.41–0.70], P 〈 .0001, respectively) and after propensity matching (HR, 0.56 [95% CI: 0.40–0.78], P 〈 .0001; HR, 0.46 [95% CI: 0.33–0.64], P 〈 .0001, respectively). The higher postoperative infection rate in the implantation group did not affect survival. Conclusions Carmustine wafer implantation during surgical resection followed by the standard chemoradiation protocol for newly diagnosed glioblastoma in adults resulted in a significant progression-free survival benefit.

    Notizen: : The objective of this study was to apply mathematical expressions commonly used in the analysis of mass-transfer processes to the study of the cod desalting operation to be able to predict the changes suffered by cod during the process. The NaCl concentration in the cod liquid phase at equilibrium was well predicted by using a mass balance. It was observed that the changes in the cod liquid phase concentration and the total weight changes throughout the process could be predicted by using a pseudo-diffusional approach or a biexponential equation, whereas a linear relation was observed between the NaCl cod liquid phase concentration and the cod density.

    Notizen: : The candying process usually takes a long time and is accomplished at high temperatures, implying damage in the product sensory properties. Sample vacuum impregnation, followed by successive osmotic steps (2 or more) in sucrose solutions of increasing concentrations, at 15 and 30 °C, has been used in order to improve pineapple candying process. Sample weight (and volume in some cases) has been controlled throughout the process till no notable changes occurred. Then, sample composition and water activity were analyzed. A 3-step process (VI with 25 °Brix followed by successive immersions in 55 and 65 °Brix solutions) provided good process yield, with better sensory properties when working at 15 °C.

    Notizen: The presence of twisted helical flow patterns in the cardiac cavities during ventricular filling and ejection was supposed. This work was intended in order to show that the intra-ventricular trabeculation plays the determining role in such a flow formation and to find some analytical approaches for its analysis. The morphometric study of human left-ventricular and aortic corrosion casts and dynamic measurement of the aorta by MRI-technique were performed. The data were analysed by means of the “Mathematica” program. Two groups of trabecules were identified that refer to the inlet and outlet of the ventricular blood flow. The first group consists of trabecules of the free left-ventricular wall. The second group consists of long trabecules going along the anterior left-ventricular wall and intracavital lines of the papillary muscles. Both are twisted clockwise and converge in the flow direction. Each group of trabecules is oriented towards the mitral or aortic valve orifices, correspondingly. It was concluded that the helical trabecular organization acts as flow directing paddles that change their mutual orientation during the cardiac cycle evolution. The reorientation of the flow takes place due to sequential contraction of the ventricular structures. The formalization of trabecular orientation will allow one to calculate improved models of implantable substitutes and auxiliary devices for cardio-vascular surgery.

    Notizen: Summary The aim of this study was to assess the value of long-term treatment with 60 mg per day of trimetazidine, a cellular antlischemic agent, in comparison with placebo, in patients with ischemic cardiomyopathy controlled by conventional treatments. Twenty patients, with a mean age of 59.5 ±1.6 years, suffering from severe ischemic cardiomyopathy (NYHA IV, 6 patients; NYHA III, 14 patients) confirmed by coronary angiography, were included in the study; four of them suffered from residual angina. All of these patients were receiving long-term treatment with long-acting nitrates associated with digitalis (9 patients), diuretics (15 patients), anticoagulants (13 patients), and antiarrhythmics (11 patients), and were considered to be stabilized at the time of inclusion in the study. The examinations consisted of clinical and laboratory assessment, resting ECG, 24-hour ECG monitoring, X-ray evaluation of cardiac volume (CV), and evaluation of echocardiographic left ventricular shortening (ELVS) and of isotopic ejection fraction (EF). These three parameters were expressed as a percentage variation with respect to the initial value, and their variation between the two groups was compared by means of two-way analysis of variance. Clinically, the therapeutic benefit provided by trimetazidine resulted in: a) an improvement of dyspnea in all patients treated with trimetazicine compared with only one patient with placebo (p 〈 0.001), b) resolution of residual angina, which was unchanged with placebo, c) reduced requirements for complementary treatments (a single case versus eight cases in the placebo group; p 〈 0.01). This clinical improvement was corroborated by a significant difference in the variation between the two groups for CV (p=0.03) and EF (p=0.02), and a nonsignificant variation for ELVS (p=0.09). The results demonstrated the beneficial effect of trimetazidine in the long-term treatment of ischemic cardiomyopathy without any associated adverse effects.

    Notizen: Abstract The case presented here is a 27-year-old patient who was born with common atrium and left superior vena cava (LSVC). Construction of interatrial septum and intraatrial baffle with pericardium was performed 16 years ago. Radionuclide angiocardiography (RAC) showed that a substantial amount of blood flow from the LSVC was directed to the inferior vena cava through the hemiazygos vein (HAV). It also detected a baffle leak and a left to right shunt at the atrial level. Subsequent RAC after reoperation initially showed insignificant flow through the atrial baffle, major flow through the HAV, and no shunt. Repeat RAC one year after surgery showed increased flow through the baffle and diminished flow through the HAV, without a satisfying explanation. This case illustrates the value of RAC in detecting various types of cardiovascular abnormality and subtle hemodynamic changes.

    Notizen: Flow cytometry was used to quantify an inflammatory reaction in vivo as a new approach to evaluating the biocompatibility of biomaterials. The exudate formed inside cylindrical tubes composed of polyvinyl chloride (PVC), silicone elastomer (SIL), or polyurethane (PU) implanted subcutaneously in the dorsal region of rats was collected over a 3-week period. The volume, number of cells, and concentration of fibrinogen were determined in the exudate for the three biomaterials. The exudate was analyzed using a flow cytometry technique after labeling of the leukocytes with a monoclonal anti-CD45 antibody. Fibrinogen rose progressively over the 3-week period for the three polymers. After the different leukocyte lines were identified in rat blood samples, their determination in the exudate revealed differences among the three biomaterials. At day 2, PVC induced a predominantly neutrophilic inflammatory reaction whereas PU and SIL gave a mixture of monocytes and neutrophils. At day 9, the aspect of the cytograms was different, but the identification of the subpopulations was still possible. At day 23, the number of cell events became too low to distinguish the subpopulations. An even more detailed approach might be possible using specific labeling for each leukocyte line to establish a comparison among the three biomaterials. Flow cytometry associated with histomorphometric assessment might provide a precise quantitative in vivo test for determining the biocompatibility of materials. © 1998 John Wiley & Sons, Inc.
