    Description / Table of Contents: Abstract We report a patient undergoing redo cardiac surgery for combined replacement of the aortic and mitral valves. During the course of the operation, a Swan-Ganz catheter – positioned preoperatively – was accidentally fixed to the wall of the pulmonary artery. As this did not interfere with cardiac output measurement or the pulmonary artery pressure wave form, the fixation was not noticed until an attempt was made to remove the catheter. Fluoroscopy revealed both the catheter's immobility and the location of the suture fixation. The patient required a sternotomy to remove the catheter. In order to avoid this complication, the indications for pulmonary artery catheters during cardiac surgery should be carefully considered. If catheters are inserted, their mobility should by all means be ensured before the chest is closed.
    Notes: Zusammenfassung Der vorliegende Fallbericht beschreibt die Nahtfixation eines Swan-Ganz-Katheters in der A. pulmonalis während eines kombinierten Aorten- und Mitralklappenersatzes. Da die Funktionen des Katheters nicht eingeschränkt waren, blieb die Fixation bis zur beabsichtigten Katheterentfernung unbemerkt. Konventionelle Röntgenaufnahmen legten die Diagnose nahe. Die Katheterentfernung erfolgte operativ über eine Resternotomie des Patienten. Zur Vermeidung dieser Komplikation bei kardiochirurgischen Eingriffen sollte, neben einer strengen Indikationsstellung, vor Thoraxverschluß in jedem Fall die freie Beweglichkeit sämtlicher Katheter im Operationsgebiet geprüft werden.

    Notes: Abstract In 9 Goettingen minipigs we studied the effect of E. coli bacteremia on effective pulmonary capillary pressure and the longitudinal distribution of pulmonary vascular resistance. Precapillary pressure gradient (dPa) was calculated as the difference between mean pulmonary artery pressure (MPP) and effective pulmonary capillary pressure (Pc) (dPa=MPP-Pc), postcapillary pressure gradient (dPv) as the difference between Pc and left atrial pressure (dPv=Pc-LAP). The disturbance of pulmonary gas exchange was quantified by the AaDO2 quotient 1-PaO2/PAO2. Live E. coli infusion resulted in hypodynamic circulatory failure. Cardiac index fell from 3.7±0.8l·min−1·m−2 to 2.2±0.7l·min−1·m−2 after bacteremia lasting for 3.5 h. Simultaneously venous pulmonary vascular resistance rose from 25% of total pulmonary vascular resistance before to 32% after 3.5 h bacteremia, thus raising Pc from 11 mmHg to 16 mmHg. The degree of respiratory insufficiency was correlated with changes of MPP, dPa and dPv: 1-PaO2/PAO2=0.2+0.035·dPv (r=0.829). Our results show, that the longitudinal distribution of pulmonary vascular resistance changes during septicemia, thus raising Pc. This may be an important factor in the genesis of septic pulmonary failure.

    Description: To assess potential impacts of climate change for a specific location, one typically employs climate model simulations at the grid box corresponding to the same geographical location. But based on regional climate model simulations, we show that simulated climate might be systematically displaced compared to observations. In particular in the rain shadow of moutain ranges, a local grid box is therefore often not representative of observed climate: the simulated windward weather does not flow far enough across the mountains; local grid boxes experience the wrong airmasses and atmospheric circulation. In some cases, also the local climate change signal is deteriorated. Classical bias correction methods fail to correct these location errors. Often, however, a distant simulated time series is representative of the considered observed precipitation, such that a non-local bias correction is possible. These findings also clarify limitations of bias correcting global model errors, and of bias correction against station data.

    Description: Seasonal reconstructions of the Southern Hemisphere annular mode (SAM) index are derived to extend the record before the reanalysis period, using station sea level pressure (SLP) data as predictors. Two reconstructions using different predictands are obtained: one [Jones and Widmann (JW)] based on the first principal component (PC) of extratropical SLP and the other (Fogt) on the index of Marshall. A regional-based SAM index (Visbeck) is also considered.These predictands agree well post-1979; correlations decline in all seasons except austral summer for the full series starting in 1958. Predictand agreement is strongest in spring and summer; hence agreement between the reconstructions is highest in these seasons. The less zonally symmetric SAM structure in winter and spring influences the strength of the SAM signal over land areas, hence the number of stations included in the reconstructions. Reconstructions from 1865 were, therefore, derived in summer and autumn and from 1905 in winter and spring. This paper examines the skill of each reconstruction by comparison with observations and reanalysis data. Some of the individual peaks in the reconstructions, such as the most recent in austral summer, represent a full hemispheric SAM pattern, while others are caused by regional SLP anomalies over the locations of the predictors. The JW and Fogt reconstructions are of similar quality in summer and autumn, while in winter and spring the Marshall index is better reconstructed by Fogt than the PC index is by JW. In spring and autumn the SAM shows considerable variability prior to recent decades.

    Description: Precipitation downscaling improves the coarse resolution and poor representation of precipitation in global climate models and helps end users to assess the likely hydrological impacts of climate change. This paper integrates perspectives from meteorologists, climatologists, statisticians, and hydrologists to identify generic end user (in particular, impact modeler) needs and to discuss downscaling capabilities and gaps. End users need a reliable representation of precipitation intensities and temporal and spatial variability, as well as physical consistency, independent of region and season. In addition to presenting dynamical downscaling, we review perfect prognosis statistical downscaling, model output statistics, and weather generators, focusing on recent developments to improve the representation of space-time variability. Furthermore, evaluation techniques to assess downscaling skill are presented. Downscaling adds considerable value to projections from global climate models. Remaining gaps are uncertainties arising from sparse data; representation of extreme summer precipitation, subdaily precipitation, and full precipitation fields on fine scales; capturing changes in small-scale processes and their feedback on large scales; and errors inherited from the driving global climate model.

    Description: Metallo-β-lactamases (MBLs) are enzymes that hydrolyze β-lactam antibiotics, resulting in bacterial resistance to these drugs. These proteins have caused concerns due to their facile transference, broad substrate spectra, and the absence of clinically useful inhibitors. To facilitate the classification, nomenclature, and analysis of MBLs, an automated database system was developed, the Metallo-β-Lactamase Engineering Database (MBLED) ( http://www.mbled.uni-stuttgart.de ). It contains information on MBLs retrieved from the NCBI peptide database while strictly following the nomenclature by Jacoby and Bush ( http://www.lahey.org/Studies/ ) and the generally accepted class B β-lactamase (BBL) standard numbering scheme for MBLs. The database comprises 597 MBL protein sequences and enables systematic analyses of these sequences. A systematic analysis employing the database resulted in the generation of mutation profiles of assigned IMP- and VIM-type MBLs, the identification of five MBL protein entries from the NCBI peptide database that were inconsistent with the Jacoby and Bush nomenclature, and the identification of 15 new IMP candidates and 9 new VIM candidates. Furthermore, the database was used to identify residues with high mutation frequencies and variability (mutation hot spots) that were unexpectedly distant from the active site located in the ββ sandwich: positions 208 and 266 in the IMP family and positions 215 and 258 in the VIM family. We expect that the MBLED will be a valuable tool for systematically cataloguing and analyzing the increasing number of MBLs being reported.
