    Description / Table of Contents: Summary A HIV-infected patient in stage CDC 3A with porphyria cutanea tarda (type I) was treated with chloroquin for some months and this led to full remission. The patient course is compared with earlier reports on HIV-infected patients with porphyria cutanea tarda. Possible provocation factors for porphyria cutanea tarda in HIV infection are considered. The increased prevalence of porphyria cutanea tarda in HIV infection, the interaction of HIV and UV irradiation as a prediposing factor for porphyria cutanea tarda and the photosensitivity present in HIV infection are discussed.
    Notes: Zusammenfassung Bei einem HIV-infizierten Patienten im Stadium CDC 3A mit Porphyria cutanea tarda (Typ I) führte eine mehrmonatige Therapie mit Chloroquin zur vollständigen Rückbildung der Hautveränderungen. Dieser Patient wird mit bisherigen Berichten von HIV-infizierten Patienten mit einer Porphyria cutanea tarda verglichen unter besonderer Berücksichtigung der Provokationsfaktoren für eine Porphyria cutanea tarda bei HIV-Infektion. Es werden die erhöhte Prävalenz der Porphyria cutanea tarda bei HIV-Infektion, die Interaktion von HIV und UV-Strahlung als Prädisposition für eine Porphyria cutanea tarda und die Lichtempfindlichkeit bei HIV-Infektion diskutiert.

    Description / Table of Contents: Summary Fractionated irradiation with X-rays or megavoltage is an important therapeutic option in the palliative treatment of Kaposi’s sarcoma. We report on three HIV patients demonstrating bullous cutaneous eruptions in the irradiation field. Three homosexual men (age 45–60 y) stage of disease CDC 3C, were treated with fractionated soft X-rays with the scheme conventionally used by us (total dose 30 Gy, single doses 3 Gy). Immediately to three days after completion of treatment large blisters appeared within the irradiation field, resolving after 2–4 weeks, leaving postinflammatory hyperpigmentation. The cause of this rare reaction to soft X-ray therapy, potentially suggesting an individually altered radiation sensitivity, is so far unknown. Controlled prospective studies applying different fractionation schemes will be necessary to better elucidate this issue.
    Notes: Zusammenfassung Die fraktionierte Röntgenbestrahlung ist eine wichtige Behandlungsform des Kaposi-Sarkoms. Wir berichten über 3 HIV-positive Patienten, bei denen nach fraktionierter Röntgenweichstrahltherapie von Kaposi-Sarkomen bullöse Hautreaktionen im Bestrahlungsfeld auftraten. Drei homosexuelle männliche Patienten (45–60 Jahre, Krankheitsstadium CDC 3C) wurden mit Röntgenweichstrahlen (43 kV, 25 mA, FHA 15 u. 30 cm, Einzeldosis 3 Gy 5mal/Woche, Gesamtdosis 30 Gy) behandelt. In den ersten 3 Tagen nach Beendigung der Strahlentherapie kam es im Bestrahlungsareal zu einer bullösen Reaktion, die sich innerhalb von 2–4 Wochen unter Hinterlassung von Restpigmentierungen zurückbildete. Die Ursache dieser seltenen Reaktion nach Röntgenweichstrahltherapien, die auf eine individuell erhöhte Strahlenempfindlichkeit hinweist, ist nicht bekannt.

    Description / Table of Contents: Summary A 53 year old man presented with a giant variant of granuloma faciale, closely resembling rhinophyma. Therapeutic approaches with cryosurgery and dapsone were unsuccessful. Surgical reconstruction of the nasal skin resulted in an excellent and long lasting effect. We give a short overview of this relatively rare disease, describe an unusual manifestation and discuss the therapeutic possibilities. Surgical procedures seem to offer the best results, despite the inflammatory pathogenesis of the disease.
    Notes: Zusammenfassung Wir berichten über einen 53jährigen Patienten mit einem seit 10 Jahren bestehenden, langsam progredienten knoten- und plaqueförmigen Granuloma eosinophilicum faciei unter dem klinischen Bild eines Rhinophyms. Behandlungsversuche mit Kryotherapie und Dapson blieben erfolglos. Eine chirurgische Abtragung brachte einen lang anhaltenden therapeutischen Erfolg. Wir geben eine kurze Übersicht über diese relativ seltene Erkrankung und stellen eine ungewöhnlich ausgeprägte Form des Granuloma eosinophilicum faciei vor. Insbesondere diskutieren wir die bisher propagierten Therapieansätze. Chirurgische Verfahren scheinen, trotz der entzündlichen Genese der Erkrankung, den besten Erfolg zu versprechen.

    Notes: Conclusions Long-term murine CD4+ T cell clones can be classified into two distinct subsets which produce either IL-2 and IFN-γ (Th1) or IL-4 and IL-5 (Th2). This functional separation of CD4+ T cells has subsequently been shown to be of great biological relevance, since these T helper cell subsets can be associated with protective or pathogenic immune responses. While a large number of in vitro and in vivo experiments underlined the biological relevance of Th1 and Th2 phenotypes, the ontogeny of Th1 or Th2 cells remained unknown until recently. Experiments published during the last 3 years have shown that cytokines such as IL-2, IL-4 and IFN-γ can influence the LK response pattern of CD4+ T cells. However, further investigations demonstrated that these cytokines alone had no effect on the modulation of the LK response pattern. Prolonged TCR binding by mitogenic mAb delivered a signal required for the induction of the Th2 phenotype. Whereas this signal cannot be replaced by soluble factors, ligation of the TCR together with LK leads to the development of Th2. The fact that TCR-mediated induction of Th2 in vitro takes the same time as the antigen-mediated commitment of Th1- or Th2-like cells in vivo underlines the role of the TCR in T helper cell subset differentiation. Since the two subsets can be derived from a common postthymic CD4+ precursor, the Th1 or Th2 phenotype of a T cell is acquired during T cell differentiation and is not secondary to the expansion of distinct subpopulations which are predetermined for the production of a specific LK pattern. The structure of the natural ligands of the TCR, i. e., antigens and MHC class II-encoded molecules, have been shown to influence the development of T helper cells with distinct LK profiles in vivo. This suggests that the TCR-ligand interaction is decisive for the commitment of a peripheral CD4+ T cell towards a Th1- or a Th2-like response pattern.
