    Description / Table of Contents: Zusammenfassung Eine primäre Meniscusdegeneration des Sportlers kann es nicht geben, da diese nur bei entsprechender Disposition und aus innerer Ursache entsteht. Sie ist nicht auf bestimmte Berufe oder Sportarten beschränkt und kann in jedem Lebensalter beobachtet werden. Der degenerationsförmige Effekt bestimmter Sportarten auf die physiologische Degeneration muß jedoch bejaht werden. Besonders die dynamischen Sportarten mit Hyperextensions-und Torsionsbelastungen des Kniegelenkes in vielfacher Wiederholung führen durch fortgesetzte Dreh- und Scherbelastungen vermehrt zu Meniskopathien.
    Notes: Summary Primary degeneration of menisci of sportsmen is not possible, as it arises from a certain predisposition and internal causes. It is not restricted to a specific occupation, sport or age. However, the physiologically degenerative effect of certain sports must be acknowledged. In particular, dynamic sports which involve repeated stress of hyperextension and torsion of the knee joint will lead to meniscopathy after continual twisting and turning.

    Description / Table of Contents: Zusammenfassung Ein Operationssaal für die operative Orthopädie wurde durch den Einbau einer Reinraumklimaanlage mit turbulenzarmer Verdrängungsströmungaufden modernsten Stand der Technik gebracht. Durch diese Anlage tritt die klimatisierte Luft durch eine Wand aus Hochleistungsschwebestofffilter in den Raum ein. Partikel in der Größe eines Tausendstelmillimeter werden zu 100% abgeschieden. Um das unmittelbar an der Operationswunde und an den Instrumententischen beschäftigte Personal und alle übrigen Personen konsequent zu trennen, wurde das Operationsfeld auf eine Breite von ca. 4,40 m durch fest installierte Glaswände vom übrigen Raum abgetrennt. Dadurch entsteht ein «keimfreier Operationstunnel», welcher stündlich 600mal von praktisch keimfreier Luft durchblasen wird. Als weitere Maßnahme unterziehen sich Operateur, Assistenten und Schwestern strengsten Disziplinvorschriften und bekleiden sich ähnlich den Astronauten (Helm) unter Verwendung einer Atemluft-Absauganlage, um jede Kontaminationsgefahr zu unterbinden. Unter Berücksichtigung dieser Gegebenheiten wurde dem Reinraum ein Bildverstärkersystem integriert, das bei entsprechender Bedienung keine zusätzliche Gefahr bedeutet. Während in den konventionellen Operationsräumen mit Luftkeimzahlen zwischen 50 und mehreren 100 Keimen pro Kubikmeter Raumluft gerechnet werden muß, ist bei dieser Anlage ein Keimgehalt von weniger als ein Keim pro Kubikmeter nachzuweisen.
    Notes: Summary The operating room for surgical orthopedies has been provided with the most up-to-date technology through the installation of a room-sterilizing air conditioning system that operates with a low-turbulence piston flow. With this equipment the conditioned air enters the room through a wall which is a high efficiency filter for suspended substance. Particles down to the size of a thousandth of a millimeter are completely 100% decomposed. In order to strictly seperate the staff working directly on the operation wound and the instrument tables and all the other persons, the operation area is separated on a width of 4.40 m by firmly installed glass walls from the rest of the room. This forms a sterilized operation tunnel through which sterilized air blows 600 times per hour. As further measures, the surgeon, assistents and nurses submit strictly to the disciplinary rules and dress similarity to astronauts (helmet) using a breath exhaust system in order to prevent any danger of contamination. Taking these facts into consideration an image amplifier system is intergrated into the clean room, which does not mean any additional danger if properly handled. While in conventional operation room the number of germs is expected to be between 50 and several 100 germs per cubic meter of room air, the number of germs with this equipment is proved to be less than a germ per cubic meter.

    Notes: Summary Experiences have shown that bandages made of ultraviolet-sensitive synthetic resin offer in various treatments important advantages to conventional plaster casts. The further scientific improvement of the material and the technical equipment is expected to increase the use in the immediate future.

    Description / Table of Contents: Summary Wear of the articulating components (especially PE-UHMW) of total hip endoprostheses is the most important technical factor limiting the functional lifetime. To minimize wear debris, ceramic heads, according to ISO 6474 (Al2O3), have been used, from 1969 paired with Al2O3 and since 1975 paired with PE-UHMW. Al2O3 balls articulating with cups made from CFRP have been in clinical use since 1988. Laboratory experiments and in-vivo testing showed minimized wear debris and mild biological response to wear products using CFRP (carbon fiber reinforced plastic) instead of PE-UHMW as the cup material. The articulating surfaces of retrieved ceramic heads (Al2O3– Biolox®) and cementless CFRP cups (carbon fiber reinforced plastic, Caproman®) were compared using sphericity measurement techniques, scanning electron microscopy (SEM) and roughness measurements (including advanced roughness parameters Rvk or Rpk according to ISO 4287). Altogether, the first results of the clinical study showed that the combination Al2O3-ball/CFRP-cup came up to the expected lower wear rates compared with the conventional combinations. The wear rates are comparable with the combination Al2O3/Al2O3 without the material-related problems of ceramic components in all ceramic combinations.
    Notes: Zusammenfassung Verschleiß der artikulierenden Komponenten (vor allem PE-UHMW) ist in der Hüftgelenkendoprothetik der wichtigste limitierende Faktor für eine Langzeitstabilität. Um den Abrieb zu minimieren, werden seit 1969 keramische Komponenten (Al 2O3 nach ISO 6474) und seit 1975 die Kombination Al2O3/PE-UHMW verwendet. Die Paarung Aluminiumoxidkeramik und kohlenstoffaserverstärktes Epoxidharz (Caproman®) wird seit 1988 klinisch eingesetzt. Bei dieser Kombination zeigten In-vitro-Experimente und In-vivo-Untersuchungen im Gegensatz zu PE-UHMW geringe Abriebraten und eine günstige biologische Reaktion auf Abriebprodukte. Die Reibflächen von explantierten keramischen Hüftköpfen (Al2O3– Biolox®) und zementfrei implantierten Caproman®-Pfannen wurden anhand von rasterelektronenmikroskopischen Untersuchungen, Sphärizitätsmessungen und computergestützten Rauheitsmessungen (incl. der erweiterten Rauheitsparameter Rvk oder Rpk nach ISO 4287 [18]) untersucht. Die mittelfristigen Ergebnisse im Rahmen einer klinischen Studie zeigen, daß mit der Paarung Al2O3/Caproman® auch im Körpermilieu die in den Voruntersuchungen ermittelten günstigen Reibeigenschaften erreicht werden und eine Verbesserung gegenüber den herkömmlichen Werkstoffpaarungen erzielt werden kann. Die dabei ermittelten geringen Verschleißraten liegen im Bereich der Tribopaarung Al2O3/Al2O3, ohne deren materialspezifische Probleme bei Keramik-Keramik-Kombinationen aufzuweisen.

    Notes: Summary Our investigations in human specimens and animal experiments show that prostheses with a spongy metal surface can become fixed by bony ingrowth to varying degrees. After consistent clinical and radiological follow-up over 12–65 months (mean 34 months) in a prospective study of 100 consecutive patients in whom 106 cementless total hip replacements had been carried out, we were able to show that after 1 year 82% of patients were pain-free with the prosthesis fixed by bony ingrowth, 8% were pain-free with the prosthesis fixed by dense fibrous tissue and 10% were not pain-free, but did not want revision surgery. Thigh pain decreased steadily from 53% 3 months postoperatively to 6.6% 15 months after operation. We believe that, when the implantation technique is exact, a high percentage of cementless spongy metal prostheses are fixed by bony ingrowth, and that a cementless prosthesis should be implanted only in patients under 60 years of age without osteoporosis.

    Notes: Summary Osteoblastoma, defined as a benign bone tumor by Jaffe and Lichtenstein, together with osteoid osteoma and multifocal osteoblastoma, belongs to the osteoblastic tumors. Due to the variable features of this tumor, differential diagnosis comprises fibrous dysplasia, giant cell tumor, aneurysmal bone cyst and osteoid osteoma; histological analysis is the only way to obtain a definite answer about the tumor type. Occasionally differential diagnosis from aggressive osteoblastoma and osteosarcoma may be difficult. The history of symptoms is usually long and diagnosis may be difficult both clinically and histologically.

    Notes: Abstract In an in vitro study of the sheep’s meniscus, the possibilities to reduce the thermal tissue necrosis caused by high-frequency electrosurgery of the meniscus were studied. Under standardized conditions, machine-made sections through the meniscus were cut using electrodes of various thicknesses and different settings of the electric generator. The thermal tissue necrosis near the cut through the meniscus was determined using light microscopy and image analysis on the specimen stained according to Masson-Goldner. With a constant voltage of 250 V tissue necrosis was significantly less for electrodes of a diameter of 0.5 mm than for 1.5-mm electrodes (187.3 μm as compared with 368.0 μm). For electrosections carried out using a commercially available electrode with a low voltage of 250 V as well as with a power-controlled generator, tissue necrosis was also significantly less than with a constant high voltage of 395 V (181.4 μm and 210.2 μm as compared with 325.0 μm). Thus, an effective reduction of thermal tissue necrosis in arthroscopic partial meniscectomy is possible when thin electrodes and power-controlled generators are used.
