    Abstract: Aim. To evaluate the expression of FOX proteins (FOXA1 and FOXA2) in the endometrium during the implantation window in women with a history of reproductive dysfunctions with different thickness of the endometrium. Materials and methods. The prospective cohort comparative study was conducted. The main group included patients with "thin" endometrium (7 mm according to ultrasound on preovulatory days; n=52), the comparison group consisted of women with normal endometrial thickness (7 mm according to ultrasound; n=62; women of both groups with reproductive dysfunctions of unknown reason), the control group included 16 healthy fertile women. Aspiration biopsy of the endometrium was performed on the 68 days after ovulation, as well as venipuncture to obtain a sample of peripheral blood to determine the levels of sex steroids (estradiol E2 and progesterone P). A combined histological and immunohistochemical study of endometrial biopsies was performed. Results. All women had ovulatory values of progesterone P16.1 nmol/l (68 days after ovulation) and normoestrogenemia (E2, pmol/l) in the blood. E2/P was similar in all groups (p0, 05). A pronounced expression of FOXA1 was noted in women with thin endometrium significantly more often (p0.05) with various hormone-receptor characteristics of the endometrium (42% n=22 out of 52) compared with healthy participants (0%; n=0). Reduced FOXA2 expression in the uterine mucosa was significantly more often detected on 68 days after ovulation in women with "thin" endometrium (56% n=29 of 52) than in women with normal endometrial thickness, both in women from the comparison group and in healthy women from the control group (p0.05). In a generalized analysis of the expression of FOX proteins in the endometrium on days 68 after ovulation, it was generally found that every second (50%; n=57 out of 114) women with a history of reproductive disorders (with a reduced and normal M-echo value) expression of proteomic markers differed from healthy women. In the case of "thin" endometrium, more than two thirds of patients (71%; n=37 out of 52) showed differences in endometrial expression of FOX proteins compared with women without a burdened reproductive history. Conclusion. In the majority of women (71%) with a thin endometrium and a history of reproductive dysfunctions, the expression of FOX proteins in the endometrium differed from the control group. Overall, endometrial expression of FOX proteins, which is likely to be different from healthy women, in patients with reproductive dysfunctions of unknown origin is a significant predictor of reproductive failure. At the same time, such an isolated indicator as M-echo value 7 mm according to ultrasound data is not an absolute prognostic marker of endometrial receptivity disorders.

    Abstract: BACKGROUND:Uterine fibroids are increasingly diagnosed in patients of reproductive age. Combined hormonal contraceptives provide reliable protection against unwanted pregnancy and help to reduce or eliminate various clinical manifestations of uterine fibroids. In practice, obstetrician-gynecologists have barriers to prescribing combined hormonal contraceptives to patients with uterine fibroids. AIM:The aim of this work was to study the features of the clinical approach of obstetrician-gynecologists to the management of patients with uterine fibroids interested in combined hormonal contraception. MATERIALS AND METHODS:The method of voluntary anonymous questioning was applied to 366 obstetrician-gynecologists. The work experience of 62.1% (n= 227) of doctors was 10 years or more. The questionnaire included questions about various aspects of the use of combined hormonal contraceptives for contraception in women with uterine fibroids. RESULTS:Only every second (47.5%;n= 174) obstetrician-gynecologist correctly assigns uterine fibroids to the first category of acceptability for combined hormonal contraceptives. The presence of an asymptomatic submucous myomatous node (FIGO 12) or dysmenorrhea in women with uterine fibroids is considered by 29.0% (n= 106) and 5.7% (n= 21) of obstetrician-gynecologists, respectively, as a contraindication to the use of combined hormonal contraceptives. Every ninth participant (11.4%;n= 42) believes that the presence of uterine fibroids limits the duration of use of combined hormonal contraceptives for contraception to 15 years. Almost half (45.6%;n= 167) of the respondents believe that combined hormonal contraceptives have various effects on uterine fibroids (cause regression or growth of fibroids, increase the risk of new fibroids). More than half (53.3%;n= 195) of the respondents are not informed about the reduction in the risk of developing uterine fibroids when using combined hormonal contraceptives for five years or more. CONCLUSIONS:In the analyzed cohort of experienced obstetrician-gynecologists, we identified their barriers to prescribing combined hormonal contraceptives for contraception for women with uterine fibroids. Those are incomplete awareness of the acceptance criteria for combined hormonal contraceptives and insufficient awareness of the lack of effect of combined hormonal contraceptives on the size of existing myomatous nodes and a decrease in the risk of developing uterine fibroids when using combined hormonal contraceptives for five or more years.

    Abstract: The review presents the psychosocial characteristics of young women in todays millennial generation and survey data regarding their hormonal contraceptive requirements. The results of randomized clinical trials and real-life studies of the use of the combined oral contraceptive (COC) Qlaira containing estradiol valerate and dienogest (E2V/DNG) in a dynamic dosing regimen were analyzed, in which the reliability of the contraceptive effect of the drug was identified and confirmed, the restoration of fertility was population level after drug withdrawal, its minimal effect on metabolism and a reduced risk of cardiovascular complications (venous and arterial thrombosis) when using Qlaira COC in comparison with other COCs. The absence of a negative effect of COC E2V/DNG on body weight, sexual function and vaginal microbiocenosis in most women has been proven. The data on satisfactory characteristics of menstrual bleeding (lighter and shorter) in users of Qlaira COC and a decrease in symptoms associated with a hormone-free interval, which often occur when taking other COCs in the 21/7 regimen, are presented. The favorable efficacy and safety profile of COC E2V/DNG in real conditions, the advantages of estrogen bioidentical to the endogenous, in the composition of the drug, the dosage regimen 26/2, the positive effect on the lifestyle, determine the significant satisfaction of young women, including young women with children, and the ability to use the drug for a long time until menopause.

    Abstract: Aim. To evaluate the expression of steroid receptors (estrogen [ER] and progesterone [PR] ) in the endometrium during the implantation window in females with a history of fertility disorders in "thin" and "absolutely thin" endometrium versus healthy females. Materials and methods. A prospective comparative study was conducted. The study group (n=42) included patients with "thin" endometrium (7 mm M-echo 5 mm at cycle days 1113 according to ultrasound); the comparison group (n=10) included females with "absolutely thin" (5 mm according to ultrasound in the pre-ovulatory days) endometrium (females in both groups had a history of infertility and miscarriage of unclear reasons in the anamnesis); the control group included 16 healthy fertile females. A Pipelle biopsy of the uterine mucosa was performed on day 68 after ovulation, and a peripheral blood sample was obtained to measure the concentration of sex steroids (estradiol [E2] and progesterone [P] ). Endometrial samples were examined by histological and immunohistochemical methods (ER, PR expression). Results. All study participants had an ovulatory cycle of P16.1 nmol/L (day 68 after ovulation) and normal estrogen levels (E2, pmol/L). E2/P was similar in all cohorts (p0.05 for all measures). ER and PR expression in the endometrium similar to those in healthy females was detected in 20% of patients in the study and comparison groups (M-echo = 4.83.1 mm): 21% (9/42) and 20% (2/10), respectively. ER and PR expression in the endometrial glands and ER expression in the endometrial stroma were significantly different (p0.05) from healthy females in 79% (41/52) of patients with "thin" endometrium and 80% (8/10) of patients with "absolutely thin" endometrium. No differences in the ER or PR expression in the endometrium in females with hypoplastic endometrium were found (p0.05). Conclusion. The M-echo value does not accurately determine endometrial hormonal-receptor abnormalities: 20% of the study participants with hypoplastic endometrium had ER and PR expression comparable to those in healthy females. No differences were found in the expression of endometrial estrogen and progesterone receptors in females with "thin" and "absolutely thin" endometrium.

    Abstract: This review presents information on demographic processes in Russia and the relevance of highly effective contraception for the prevention of artificial abortion. We present data on the etonogestrel implant (68 mg), an innovative single-stranded subcutaneous implant, a long-acting reversible contraceptive option with high efficacy (Pearl index 0.05) with optimal and typical use of the product. We analyzed the results of the studies that showed a favorable safety profile of the etonogestrel implant with no significant effect on the risks of thrombotic events, metabolic processes, depression incidence, bone mineral density, and characteristics of reproductive function after discontinuation of the drug. The article presents data on possible side effects in the form of changes in the characteristics of menstrual bleeding and a slight increase in body weight, which is comparable with the natural gain in an adult with age. The importance of qualified counseling of patients before and during the use of etonogestrel implants is emphasized.

    Abstract: Overcoming infertility and reducing miscarriage are important challenges in obstetrics and gynecology. In the Russian Federation, the frequency of infertile couples reaches 24%; the percentage of miscarriage does not decrease, accounting for up to 20% of all desired pregnancies. The endometrial factor plays an important role in the causes of gravidar losses. Inadequate phase transformation of the endometrium is a predictor of fetal egg implantation failures. An integral part of endometrial cyclic transformations is the full proliferation of the mucous membrane of the uterine body. However, there is a cohort of women who, due to the insufficiency of proliferative processes in the endometrium, have a syndrome of thin endometrium. The pathogenesis of the formation of a hypoplastic endometrium in women with normal characteristics of hormonal factors is still poorly understood. Particular attention is paid to determining the function of proteomic markers (leukemia inhibiting factor, Forkhead box-protein family, etc.) in the genesis of the hypoplastic endometrium. This review presents an analysis of modern Russian and foreign sources on the receptivity of the endometrium, including the syndrome of thin endometrium in women of reproductive age.

    Abstract: Background. In Russia, adherence to planned highly effective methods of contraception is still at a low level; therefore, emergency contraception (EC) can be the option for protection against unplanned pregnancy.Study design, materials and methods. The current analysis was undertaken to determine the frequency of EC use among women with and without higher medical degree (MD) and to find out the level of awareness about EC among women and men of the same categories. In order to this, the method of anonymous voluntary questioning was used. The questionnaire included items about the theoretical aspects of EC (filled in by both men and women) and data on the gynecological history and experience of EC application (filled in by women only).Results. Half of respondents (49.8%) had experience of EC use, 80% of them used EC only if unprotected sexual intercourse occurred in the middle of the menstrual cycle. Every forth (24.6%, n = 29) obstetrician-gynecologist mistakenly believes that repeated application of EC can lead to infertility. 45% (n = 64) participants without MD and 42% (n = 61) of doctors of other specialties have the same opinion. The use of EC is considered justified by 86% (n = 227) of doctors and 65% (n = 94) participants without MD. Every tenth responder, irrespective of education level, mistakenly believes in not using EC due to severe negative side effects.Conclusion. Almost a third (30%) of obstetrician-gynecologists do not possess complete information on EC. Awareness about EC is extremely inadequate and comparable to that of doctors of other specialties and people without MD. (For citation: Aganezov SS, Morotskaya AV, Aganezova NV.     Abstract: Aim. To determine awareness about family planning and contraceptive preferences among young people. Materials and methods. Internet questioner about knowledge, preferences, personal experience of contraception usage. 1010 responders (935 from Russia, 75 from other countries) were processed by statistical methods in the IBM SPSS Statistics program. The group was a simple random sample. Results. 92.5% of foreign and 54.2% of Russian respondents with medical specialties do not consider a stroke as a contraindication to combined oral contraceptives COCs (p0.001); 45.3 and 42.2% respondents have similar views about breast cancer in the anamnesis. Uncomplicated varicose veins and uterine fibroids are mistakenly considered contraindications to COCs by 26.4 and 20.8% of foreign participants and 54.5 and 44.4% of Russian participants, respectively. 25.1% healthcare providers from Russia consider hormonal contraception (HC) a possible cause of infertility. Only 4% of Russians of non-medical specialties know about strokes and heart attacks as complications of COCs, 13.8% about arterial hypertension; almost half of foreign participants have such knowledge (p0.05). Every second respondent from Russia and every third from other countries does not know about the timing of use of emergency contraception. Russian and foreign participants, when they were using HC, respectively noted: decreased libido (29.1 and 13%), increased body weight (15.0 and 34.8%), headache (21.8 and 17.4%), mastalgia (20.6 and 13.0%). Every fifth participant did not want to have periods of menstrual bleeding when using HC. 86 women have had pregnancies before, from those 62.2% of Russian and 75% of foreign participants had unplanned pregnancies; 40.2% of respondents from Russia and 25% of foreign participants terminated pregnancy. The most popular contraceptive method among Russian and foreign respondents is male condom (92.7 and 84.6%; p=0.037). Second most popular in Russia is coitus interruptus (57.7%), in third place are COCs (33.1%); among foreign participants COCs are in second place (38.5%). Conclusion. Family planning awareness among young people is insufficient, which is reflected in their contraceptive preferences. Knowledge of family planning and highly effective methods of contraception should be popularized.

    Abstract: Aim. To assess the expression of estrogen (ER) and progesterone receptors (PR) in the endometrium during the implantation "window" in women with impaired reproductive function with low (16.1P30 nmol/l) and high (P30 nmol/l)) functional activity of the corpus luteum of the ovary. Materials and methods. A prospective single-center cohort comparative study. The main group included patients with infertility (n=93) and miscarriage (n=47) of unclear reason in the anamnesis, the control group 16 healthy fertile women. In all cases, vacuum-aspiration endometrial biopsy was performed on the 68 days after ovulation with simultaneous collection of a peripheral blood sample to determine the levels of estradiol and progesterone. Endometrial samples were studied by histological and immunohistochemical (ER, PR expression) methods. Results. Women of the main (1) and control (2) groups had an ovulatory menstrual cycle. The serum estradiol (E2) level was 691.326.5 pmol/l (1) and 707.466.1 pmol/l (2); progesterone (P) 46.11.8 nmol/l (2) and 39.14.9 nmol/l (2). Mid-secretory endometrium (results of histological examination) was determined in all healthy women (n=16) and in 46% (n=64) of the patients of the main group. All women in the control group had adequate hormone-receptor characteristics of the endometrium, in women in the main group in 45% (n=63). Comparative analysis of the expression of ER, PR in the endometrium in patients with different functional activity of the corpus luteum of the ovary was carried out in the groups 1 and 2, in women with different reproductive dysfunctions in the anamnesis (without significant differences; p0.05); subgroup analysis was performed in cohorts of women with normal (63 women in group 1 and 16 in group 2) and inadequate (77 patients in group 1) hormone-receptor status of the endometrium. There were no significant differences in the expression of ER, PR in the endometrium with low/high functional activity of the corpus luteum of the ovary with similar morphological characteristics of the endometrium (p0.05). Correlation analysis did not reveal significant relationships between the level of P in the blood and indicators of endometrial expression ER, PR (p0.05). Conclusion. In the studied cohorts, the prognostic significance of the functional activity of the corpus luteum of the ovary for the adequate expression of estrogen and progesterone receptors has not been established. To clarify the presence/absence of endometrial dysfunction in women with a history of reproductive failures, a morphological examination of the endometrium of the middle secretion phase should be carried out.
