    Abstract: The WHO estimates that each year 5 million people are left permanently disabled after stroke. Adjuvant treatments to promote the effects of rehabilitation are urgently needed. Cortical excitability and neuroplasticity can be enhanced by non-invasive brain stimulation but evidence from sufficiently powered, randomized controlled multi-center clinical trials is absent. Methods Neuroregeneration enhanced by transcranial direct current stimulation (tDCS) in stroke (NETS) tested efficacy and safety of anodal tDCS to the primary motor cortex of the lesioned hemisphere in the subacute phase (day 5–45) after cerebral ischemia. Stimulation was combined with standardized rehabilitative training and repeatedly applied in 10 sessions over a period of 2 weeks in a planned sample of 120 patients. Primary outcome parameter was upper-extremity function at the end of the 2-weeks intervention period of active treatment or placebo (1:1 randomization), measured by the upper-extremity Fugl-Meyer assessment. Sustainability of the treatment effect was evaluated by additional follow-up visits after 30 and 90 days. Further secondary endpoints included metrics of arm and hand function, stroke impact scale, and the depression module of the patient health questionnaire. Perspective NETS was aimed at providing evidence for an effective and safe adjuvant treatment for patients after stroke. Trial registration : ClinicalTrials.gov Identifier NCT00909714.     Abstract: Host modulation therapy has gained increasing interest in periodontal therapy. This systematic review aimed to evaluate the effects of adjunctive administration of omega-3 fatty acids in periodontal therapy. Methods The search strategy was determined using the “patient, intervention, comparison, outcome” model. A resulting search term was generated using keywords, and the databases were fed. The databases PubMed, Cochrane Library, and LIVIVO were used. Studies were selected for the literature review based on previously specified inclusion and exclusion criteria. Randomized, controlled, blinded studies, longitudinal studies, comparative studies, and clinical studies were included in the review. Additionally, they used omega-3 fatty acids in the treatment of periodontitis. The following parameters were observed: clinical attachment level (CAL), probing depth (PD), gingival index (GI), bleeding on probing (BOP) and plaque index (PI). A meta-analysis was performed for PD and CAL after 3 months. By analyzing the risk of bias, the validity of the results of each study was demonstrated, and its credibility and quality were assessed. Results Of 14 studies found, six were included. The results showed a significant reduction in PD and CAL compared to that in the placebo groups in four out of six involved studies, which was confirmed by the meta-analysis. In one study, a significant reduction in BOP was found. GI was significantly reduced in three included studies. PI also showed a significant reduction in three studies. Conclusions Within the study limitations, omega-3 fatty acids appear to have a positive effect on periodontal wound healing with regard to reduction in CAL and PD. Based on the results, patients receiving periodontal treatment might benefit from nutritional counseling.

    Abstract: The importance of good communication in dentistry is proven in terms of both medical satisfaction and patient‐related recovery and prevention. The present work deals with the comparison of communicative abilities and the communicative self‐assessment of licensed dentists as well as students of dentistry with special emphasis on the influence of the treatment experience. Materials and Methods A total of 34 dentists (experimental group) with an average work experience of 16 years and 36 students (control group) with an average of 1.4 years of treatment experience were included. In addition to a tutor, four types of simulation patients with standardised trained roles (anxious, critical, dissatisfied and difficult to motivate) were used to create reproducible conversations. The self‐assessment and evaluation of the conversation took place by completing questionnaires. Here, an introductory questionnaire was distributed to the participants prior to the conversation and another one after intervention. Whilst the tutors completed their survey during the intervention, the simulation patients answered their questions after the conversation. Results The results showed that the dentists rated their own communication skills significantly higher than the students for anxious ( P   〈  0.001) and unmotivated patients ( P  = 0.026). However, the evaluation of the simulated patients showed that the students achieved higher overall empathy scores (42.03 vs 38.77, P  = 0.016). Conclusion Due to the declining empathy values with increasing treatment experience, communication training is useful for the daily routine of treatment even for experienced dentists.

    Abstract: Motivational interviewing (MI) is an evidence‐based method of promoting oral healthcare behaviour. Conventional training of MI is a time‐consuming and costly aspect in the dental curriculum. Therefore, the aim of this study was to evaluate the effectiveness and acceptance of a MI‐blended learning programme for dental students. Materials and Methods Dental students had to perform an interdisciplinary created "ecourse Motivational Interviewing in medical settings" (eMI‐med). After completion, patient‐student interviews were recorded and evaluated using the Motivational Interviewing Treatment Integrity Code (MITI‐d). Furthermore, the students’ self‐efficacy regarding smoking cessation and oral hygiene motivation was examined and the acceptance of the tool was enquired. Results Forty interviews with 25 different students were analysed with the MITI‐d. Students showed high levels of MI‐adherent behaviour (15.45 ± 6.98), open‐ended questions (9.95 ± 6.90) and reflections (10.43 ± 8.85), which were comparable to previous classroom trainings. In addition, 90% of the students preferred e‐learning over classroom teaching. Furthermore, the students’ therapeutical self‐efficacies were significantly increased by the programme. Conclusion Within the limitations of this study, the created e‐learning programme was able to equip dental students with basic knowledge and MI skills. Furthermore, learning MI through e‐learning may heighten the self‐efficacy of dental students regarding smoking cessation and oral hygiene promotion. Students showed a high acceptance of e‐learning, preferring it over traditional learning.

    Abstract: The aim of this randomized, controlled, double-blinded clinical trial was to examine the additional healing effect of transgingival visible light and water-filtered infrared-A (VIS + wIRA) in the treatment of periodontitis patients compared with the standard therapy by subgingival instrumentation (SI). Therefore, forty patients with untreated periodontitis received a non-surgical periodontal treatment. Using a split-mouth study design, one quadrant of the upper jaw was randomly either exposed to VIS + wIRA four times for 20 min within two weeks in addition to SI or received only SI. Three and 6 months after intervention, clinical parameters (probing depths (PDs), clinical attachment level, bleeding on probing (BOP), furcation, tooth mobility, plaque control record, and papilla bleeding index) were re-evaluated. In the presence of PD of 4 mm and positive BOP or PD 〉 4 mm, SI was performed again. Moreover, the patients were asked about their discomfort using a visual analog scale from 1 to 10 for each side of the maxilla. Statistical analysis demonstrated no differences between quadrants at re-evaluation for clinical parameters (p 〉 0.05) after 3 and 6 months. Concerning pain perception, patients described less pain on the irradiated side (p = 0.016). In the treatment of patients with periodontitis, VIS + wIRA did not show an additional effect on the clinical outcome after 3 and 6 months. Patients described less pain on the irradiated quadrant after treatment.

    Abstract: Large explosive tropical volcanic eruptions inject high amounts of gases into the stratosphere, where they disperse globally through the large-scale meridional circulation. There is now increasing observational evidence that volcanic halogens can reach the upper troposphere and lower stratosphere. Here, we present the first study that combines measurement-based data of sulfur, chlorine and bromine releases from tropical volcanic eruptions with complex coupled chemistry climate model simulations taking radiative-dynamical-chemical feedbacks into account. Halogen model input parameters represent a size-time-region-wide average for the Central American eruptions over the last 200 ka ensuring a comprehensive perspective. The simulations reveal global, long-lasting impact on the ozone layer affecting atmospheric composition and circulation for a decade. Column ozone drops below 220 DU (ozone hole conditions) in the tropics, Arctic and Antarctica, increasing biologically active UV by 80 to 400%. Our model results could potentially be validated using high-resolution proxies from ice cores and pollen records.

    Abstract: A strong midwinter warming occurred in the Southern Hemisphere (SH) stratosphere in September 2002. Based on experiences from the Northern Hemisphere (NH), this event can be defined as a major warming with a breakdown of the polar vortex in midwinter, which has never been detected so far in the SH since observations began at the earliest in the 1940s. Minor midwinter warmings occasionally occurred in the SH, but a strong interannual variability, as is present in winter and spring in the NH, has been explicitly associated with the spring reversals. A detailed analysis of this winter reveals the dominant role of eastward-traveling waves and their interaction with quasi-stationary planetary waves forced in the troposphere. Such wave forcing, finally leading to the sudden breakdown of the vortex, is a familiar feature of the northern winter stratosphere. Therefore, the unusual development of this Antarctic winter is described in the context of more than 50 Arctic winters, concentrating on winters with similar wave perturbations. The relevance of preconditioning of major warmings by traveling and quasi-stationary planetary waves is discussed for both hemispheres.

    Abstract: Several meteorological datasets, including U.K. Met Office (MetO), European Centre for Medium-Range Weather Forecasts (ECMWF), National Centers for Environmental Prediction (NCEP), and NASA’s Goddard Earth Observation System (GEOS-4) analyses, are being used in studies of the 2002 Southern Hemisphere (SH) stratospheric winter and Antarctic major warming. Diagnostics are compared to assess how these studies may be affected by the meteorological data used. While the overall structure and evolution of temperatures, winds, and wave diagnostics in the different analyses provide a consistent picture of the large-scale dynamics of the SH 2002 winter, several significant differences may affect detailed studies. The NCEP–NCAR reanalysis (REAN) and NCEP–Department of Energy (DOE) reanalysis-2 (REAN-2) datasets are not recommended for detailed studies, especially those related to polar processing, because of lower-stratospheric temperature biases that result in underestimates of polar processing potential, and because their winds and wave diagnostics show increasing differences from other analyses between ∼30 and 10 hPa (their top level). Southern Hemisphere polar stratospheric temperatures in the ECMWF 40-Yr Re-analysis (ERA-40) show unrealistic vertical structure, so this long-term reanalysis is also unsuited for quantitative studies. The NCEP/Climate Prediction Center (CPC) objective analyses give an inferior representation of the upper-stratospheric vortex. Polar vortex transport barriers are similar in all analyses, but there is large variation in the amount, patterns, and timing of mixing, even among the operational assimilated datasets (ECMWF, MetO, and GEOS-4). The higher-resolution GEOS-4 and ECMWF assimilations provide significantly better representation of filamentation and small-scale structure than the other analyses, even when fields gridded at reduced resolution are studied. The choice of which analysis to use is most critical for detailed transport studies (including polar process modeling) and studies involving synoptic evolution in the upper stratosphere. The operational assimilated datasets are better suited for most applications than the NCEP/CPC objective analyses and the reanalysis datasets (REAN/REAN-2 and ERA-40).
