    Notes: Summary We report two patients with thrombotic thrombocytopenic purpura who were subjected to plasma exchange. In one case, the plasma levels of platelet factor 4, measured shortly after plasma exchange, increased significantely during plasma exchange. This was followed, however, by a failure to respond to therapy. Repeated plasmapheresis over 3 weeks gave no therapeutic benefit and reversible deep coma occurred. This patient recovered completely after treatment with vincristine. In the second patient, a decline in platelet factor 4 was observed after plasma exchange. This was accompanied by improvement of the patient's condition and a slow rise in platelet count. Plasma exchange was again carried out in this patient because of a recurrence of thrombotic thrombocytopenic purpura 3 years later; again decreased platelet factor 4 plasma levels were observed after plasma exchange and again a therapeutic response followed. Platelet factor 4, therefore, seems to be an effective and early index for the therapeutic benefit of plasma exchange in thrombotic thrombocytopenic purpura.

    Notes: Abstract Type A natriuretic peptide (CDD/ANP99-126) in its circulating form was analyzed with respect to the localization of its bronchodilating effects in asthmatic subjects in vivo. The intravenous infusion of 5.7, 11.4, and 17.1 pmol kg−1 min− CDD/ANP-99-126 caused a significant bronchodilation of both central and peripheral airways. While the localization of the bronchodilating effects was similar to β2-agonists, an improvement in lung function parameters comparable to these substances was not observed. But other members of the natriuretic peptide family may reveal a stronger bronchodilating potency.

    Notes: 40 S40Se20, deposited by thermal evaporation, were obtained in the 400 nm to 2200 nm spectral region. The optical constants of this amorphous material were computed using an optical characterization method based mainly on the ideas of Minkov and Swanepoel of utilising the upper and lower envelopes of the spectrum, which allows us to obtain both the real and imaginary parts of the complex refractive index, and the film thickness. Thickness measurements made by a surface-profiling stylus have been carried out to cross-check the results obtained by the optical method. The dispersion of the refractive index is discussed in terms of the single-oscillator Wemple–DiDomenico model. The optical band gap has been determined from absorption coefficient data by Tauc’s procedure. Finally, the photo-induced and thermally induced changes in the optical properties of a-As40S40Se20 thin films were also studied, using both transmission and reflection spectra.

    Notes: Abstract  The oxazaphosphorine antineoplastic ifosfamide (IF) is metabolized by two different initial pathways: ring oxidation (“activation”), forming 4-OH-IF (“activated IF”), and side-chain oxidation with liberation of chloroacetaldehyde (CAA), forming the inactive metabolites 3-dechloroethylifosfamide or 2-dechloroethylifosfamide (3-DCE-IF, 2-DCE-IF). 4-OH-IF and 4-OH-IF-derived acrolein are thought to be responsible for IF-induced urotoxicity (hemorrhagic cystitis), whereas CAA may be involved in IF-associated nephrotoxicity (renal tubular damage). The thiol compound 2-mercaptoethane sulfonate sodium (mesna) has proved to inactivate sufficiently the urotoxic metabolites of oxazaphosphorine cytostatics and is therefore routinely given to patients receiving IF chemotherapy. The cumulative urinary excretion of IF, 4-OH-IF, 3-DCE-IF, 2-DCE-IF, mesna, and its disulfide dimesna was studied in 11 patients with bronchogenic carcinoma receiving IF on a 5-day divided-dose schedule (1.5 g/m2 daily) with concomitant application of mesna (0.3 g/m2 at 0, 4, and 8 h after IF infusion). On day 1 the mean cumulative 24-h urinary recoveries (percentage of the IF dose) recorded for IF, 4-OH-IF, 3-DCE-IF, and 2-DCE-IF were 13.9%, 0.52%, 4.8%, and 1.5%, respectively. On day 5 the corresponding values were 12.2%, 0.74%, 9.9%, and 3.6%, respectively. This time-dependent increase in urinary excretion of IF metabolites, which is caused by rapid autoinduction of hepatic oxidative metabolism, may result in a higher probability for the development of urotoxic and nephrotoxic side effects during prolonged IF application. The mean 24-h urinary recoveries (percentage of the daily mesna dose) recorded for mesna/dimesna on day 1 (day 5) were 23.8%/45.2% (21.2%/39.8%), respectively. The mean molar excess of urinary reduced (“free”) mesna over 4-OH-IF ranged from 11 to 72 on day 1 and from 6 to 40 on day 5. This indicates that although urinary excretion of 4-OH-IF rises with repeated IF application, mesna in standard doses should sufficiently inactivate the urotoxic IF metabolites.

Benigne intrakavitäre Herztumoren führen zu „malignen“ Komplikationen, so daß die rechtzeitige Diagnose und die entsprechende Therapie von vitaler Bedeutung sind. Unsere Untersuchung faßt eine 10 jährige Erfahrung nach chirurgischer Behandlung von 29 Patienten mit benignen intrakavitären Herztumoren zusammen. Die postoperative histologische Untersuchung ergab in 27 Fällen die Diagnose eines Myxoms, und in 2 Fällen handelte es sich um ein papilläres Fibroelastom. Die führende Symptomatik bestand bei 19 Patienten in einer Dyspnoe. In 8 Fällen kam es zu einem embolischen Ereignis. Die Behandlung bestand bei allen Patienten in der Tumorexstirpation. Die Frühletalität betrug 10 %. Die Spätletalität lag bei 4 %. Die Exstirpation ist die Therapie der Wahl benigner intrakavitärer Herztumoren. Sie ist dringlichst (auf Grund der nicht vorhersehbaren embolischen Komplikationen) durchzuführen, falls der Tumor eine lobulierte knollige Oberfläche aufweist. Dabei ist die deskriptive Rolle der Echokardiographie von überragender Bedeutung.
    Notes: Summary. Benign intracavitary cardiac tumors lead to “malignant” complications, so early diagnosis and adequate treatment are of vital importance. Our investigation summarizes a 10-year experience after surgical treatment of 29 patients suffering from benign intracavitary cardiac tumors. The postoperative histological examination revealed a myxoma in 27 cases; in 2 cases a papillary fibroelastoma was diagnosed. The main symptom in 19 patients was dyspnea. In 8 cases an embolic event occurred. All patients underwent extirpation of the tumor. Early mortality was 10 %, while late mortality was 4 %. The treatment of choice of benign intracavitary cardiac tumors is extirpation, which, due to unpredictable embolic complications, is to be carried out immediately should the tumor have a lobulated tuberous surface. In such cases the descriptive function of echocardiography is of considerable importance.

    Description / Table of Contents: Summary Cervical lymphadenopathies are widespread diseases in otorhinolaryngology. There are many differential diagnoses which have to be considered when swelling persists. To gain further information it is advisable to perform sonographically controlled fine needle aspiration biopsy (FNP). If there is no reliable result, it is necessary to remove a lymph node. We report on the rare case of isolated histiocytosis X in lymph nodes. A 63-year-old male had noticed a swelling of the right side of the neck about 6 weeks previously. The preliminary examinations (FNP/blood examinations) revealed no pathological findings. Only the histological examination delivered the diagnosis of histiocytosis X. Systemic manifestation was excluded by staging examinations. No further therapy was necessary after lymph node extirpation. The report demonstrates how to diagnose and treat this rare disease.
    Notes: Zusammenfassung Lymphknotenschwellungen gehören zu den häufigen Erkrankungen im HNO-Bereich. Es gibt eine Reihe von Differentialdiagnosen, die bei länger bestehenden Schwellungen zu bedenken sind. Zur Gewinnung einer orientierenden Zytologie empfiehlt sich die sonographisch kontrollierte Feinnadelpunktion (FNP). Wenn diese kein sicheres Ergebnis bringt, ist eine Lymphknotenexstirpation indiziert. Wir schildern den seltenen Fall einer isoliert auftretenden Histiozytosis X des Lymphknotens. Bei einem 63jährigen männlichen Patienten war eine Schwellung rechts zervikal aufgetreten. Die Voruntersuchungen (Labor/FNP) zeigten keine pathologischen Befunde und erst durch die histologische Präparataufarbeitung konnte die Diagnose einer Histiozytosis X gestellt werden. Eine disseminierte Manifestation konnte durch eine umfassende Diagnostik ausgeschlossen werden, so daß nach erfolgter Halslymphknotenexstirpation keine systemische Therapie mehr notwendig war. Anhand der Kasuistik werden die notwendige Diagnostik und Therapieverfahren aufgezeigt.

    Description / Table of Contents: Abstract Pigmented villonodular synovitis (PVNS) is a disease that involves the lining of joints, bursae and tendon sheaths. The incidence is low and estimated to be 1.8 patients per million population. The cause of PVNS is unclear and discussed to be either inflammatory or neoplastic. PVNS has been described in 2 forms different for prognosis and treatment (nodular and diffuse). The articular form almost appeares in the knee joint as we describe below in a 14 year-old patient. PVNS was first defined in 1941 by Jaffe e.a. [7]. Because of the uncommon occurence of the disease it is difficult to amass patient series to allow confirmed statements on therapy and outcome. Larger patient series raise out of long periods of time. Differences in outcome and recurrence rates exist for the nodular and diffuse form. Clinical findings are moderate pain and swelling of joints due to effusion and synovial proliferation. Magnetic resonance imaging shows typical findings. Surgical procedures are recommended as open or arthroscopic synovectomy for the diffuse form of PVNS, local excision for the nodular form and arthrodesis or prosthetic replacement for joint destruction.
    Notes: Zusammenfassung Die pigmentierte villonoduläre Synovialitis (PVNS) ist eine Erkrankung der Gelenksynovialis, Bursen und Sehnenscheiden mit geringer Inzidenz (1,8/1 Mio. Einwohner). Ätiologisch wird sowohl ein chronisch-entzündliches Geschehen als auch ein neoplastischer Ursprung diskutiert. Prognostisch und therapeutisch relevant ist die Unterteilung in die diffuse und die lokalisiert-noduläre Manifestation, deren bevorzugte und meist einzige Lokalisation das Kniegelenk ist. Auch im vorliegenden Fallbericht wird über die monoartikuläre noduläre Manifestation im Kniegelenk eines 14jährigen Patienten berichtet. Die Erstbeschreibung der PVNS stammt von Jaffe et al. aus dem Jahre 1941 [7]. Aufgrund der geringen Inzidenz sind durch große Fallzahlen gesicherte Aussagen zu Therapie und Prognose in der Literatur selten und betreffen retrospektiv analysierte Patientenkollektive aus großen Zeiträumen. Gesichert ist die prognostische Relevanz der Manifestationsform (diffus oder nodulär ). Klinische Leitsymptome sind meist moderate Schmerzen und Schwellungszustände des Gelenks, zum einen bedingt durch einen intraartikulären Erguß, zum anderen im fortgeschrittenem Erkrankungsstadium bedingt durch die pathologische Gewebezunahme. Die spezifische Diagnostikmethode der Wahl ist die Magnetresonanztomographie ( MRT ). Das stadien- und manifestationsorientierte Vorgehen umfaßt an operativen Verfahren die offene oder arthroskopische Synovektomie im Falle der diffusen Erscheinungsform, die lokale Tumorexstirpation bei nodulärem Befall, weiter bei fortgeschrittener Erkrankung die Gelenkresektion mit Arthrodese oder Prothesenersatz.
