    Description / Table of Contents: Résumé Les auteurs présentent une technique originale d'amincissement de la rotule, comme alternative à la patellectomie ou à l'avancement de la tubérosité tibiale antérieure chez les malades présentant des douleurs sévères de la face antérieure du genou. Une étude expérimentale, menée sur 13 genoux provenant de membres amputés, a permis de montrer, à l'aide du papier barosensible Fujifilm Prescale, qu'un amincissement de la rotule de 7 mm entraîne une importante diminution de la surface de contact fémoro-patellaire, de 16.07% à 60° de flexion à 27,9% à 30°. Les pressions enregistrées dans 12 cas (une fois la mesure n'a pas été possible) ont diminué aprés l'ostéotomie d'amincissement, aussi bien sur la facette interne de la rotule que sur l'externe à 30°, 60° et 90° de flexion. Le degré de signification statistique est remarquablement élevé, sauf pour les mesures réalisées sur la facette externe à 60° de flexion. Cette technique ajoute donc une décompression aux bénéfices biologiques de toutes les ostéotomies.
    Notes: Summary The authors describe an original procedure, the patella thinning osteotomy, as an alternative to advancement of the tibial tuberosity or patellectomy in cases of severe anterior knee pain. They present an experimental study on cadaveric knees prior to a clinical trial. This study on 13 knee specimens, using Fujifilm Prescale barosensitive film, shows that thinning of the patella by 7 mm diminishes to a significant degree the patellofemoral contact areas from 16.07% at 60° of flexion to 27.90% at 30° of flexion. Patellofemoral joint reacting forces, measured in 12 knees, also diminish in both medial and lateral surfaces at 30°, 60° and 90° of flexion, the level of statistical significance being over 0.05, except for the lateral surface at 60° of flexion. This technique decompresses the patellofemoral joint, and produces the biological effects found after other osteotomies.

Pneumocephalus was seen on CT and was attributed to compressed air injected into the brain with the missile. This unusual cause of pneumocephalus has apparently not been previously reported.

    Notes: Summary A clinical trial of an immunotherapy which consisted of intra-tumoural injections of autologous lymphocytes with human lymphoblastoid interferon was evaluated in 31 patients with intracranial glioblastoma. Immunotherapy was performed after stereotactic biopsy or surgical resection. The treatment was tolerated well by all patients. Three patients showed positive response to immunotherapy as documented by transient regression or stabilization of the tumour size on computed tomography. Nevertheless, there is no significant difference in the survival time of the patients treated with immunotherapy and those not treated. We conclude that this immunotherapeutic regimen is not beneficial in patients with glioblastoma when used as single treatment after tumoural biopsy or resection.

    Notes: Summary An isocentered system for functional stereotactic procedures with the Cosman-Roberts-Wells frame and a CT localizer that allows extrapolation of target data directly from the CT slice is presented. Based on anatomical landmarks and on the scaled corresponding transverse plates of the Schaltenbrand and Wahren atlas, we delineate the thalamic and cerebellar nuclei. Twenty three image-directed functional procedures were performed in one year on 18 patients (7 with Parkinson's disease, 4 with dystonia, 3 persons with essential tremor, 2 patients with choreo-athetosis and 2 with de-afferentiation pain). The 23 procedures included 19 thalamotomies, two dentatotomies and two stereotactic implantations of deep seated brain electrodes. Successful targeting was verified by intra-operative electrical stimulation and postoperative CT scan. Complete reduction of symptoms was observed in 4 persons with Parkinson's disease and in 2 patients with essential tremor with significant improvement observed in the rest of the patients with the exception of the individual with choreo-athetosis. There were no operation-related complications. The reported technique is safer and less distressing for patients than previous radiological procedures and it makes image-directed stereotactic functional neurosurgery available to many units with the CRW frame.

    Notes: Summary An experimental study in rats has been carried out with the aim of studying the scar formation over the spinal dura in the presence of fibrin glue. Although the main action of fibrin glue is the enhancement of wound healing by increasing the fibrous tissue, our present data suggest that this re-absorbable biomaterial diminishes the epidural scar formation, at least the first two weeks after laminectomy. This finding leads us to consider the use of fibrin glue over spinal duramater when it is likely that reoperation may become necessary in a short period of time after a laminectomy.

    Notes: Summary Thirty adult patients presenting with medulloblastoma between 1974 and 1991 were studied and treated at Puerta de Hierro Clinic. After diagnosis, all patients were treated by surgery followed by radiotherapy and eight of them received adjuvant chemotherapy. We have studied the influence of some factors such as age, sex, location of tumour in the cerebellum, amount of surgical resection and histological variants on survival and recurrence of the disease. Only the histological type has a statistically significant influence on survival and recurrence: we have found that patients presenting classic medulloblastoma have a long survival and a long relapse-free interval.

    Notes: Summary The authors present a case of primary carcinoma of the choroid plexus in a 26-years-old female, localized in the left lateral ventricle. Four years after total removal plus local radiotherapy, the patient presented with an intraparenchymatous metastasis in the right cerebellar hemisphere and intraspinal metastatic seeding. With this kind of malignant growth, a therapeutic approach consisting of radical removal of the tumour plus radiotherapy to the whole neuraxis is proposed.

    Notes: Summary A case of symptomatic subependymoma of the lateral ventricle in a 64-year-old-woman is presented. We have found only 12 cases of symptomatic subependymoma of the lateral ventricle reported in the literature. We think that this location denotes a specific anatomoclinical entity, with a histological picture and a surgical prognosis that are different from those of the symptomatic subependymomas located in other sites of the ventricular system.
