    Notes: Summary Positive after-images (PAIs) evoked by low intensity stroboscopic flash are erased by voluntary large amplitude saccadic eye movements. The duration of the PAI is shortened by a Jendrassik manoeuvre. The results suggest that muscle spindle afferent impulses conducted centrally are involved in the erasure phenomenon. The duration of the PAI is modifiable by drugs. The social implications of PAI are considered briefly.

    Notes: Abstract Renal sonographic evaluation of seven patients with mucocutaneous lymph node syndrome were performed and correlated with clinical and laboratory data either supporting or not supporting renal disease associated with this entity. Four of seven patients demonstrated significant elevations of the BUN, creatinine and/or significant proteinuria. These four patients had renal sonographic findings of increased cortical echogenicity, enlarged kidneys and enhanced corticomedullary differentiation. This complication of mucocutaneous lymph node syndrome has heretofore not been noted.

    Notes: Abstract Pelvic masses in girls have been the subject of much discussion in the ultrasound-computerized tomography literature. Specifically, ultrasound has confirmed that cystic masses tend to be benign while solid masses are considered malignant until proven otherwise. Complex masses still remain an area of diagnostic difficulty and clinical data are essential in reaching a reasonable diagnosis. This paper reviews pediatric pelvic masses in girls and the role ultrasound plays in their evaluation.

    Notes: Abstract Gliomas of the optic nerve, although typically of pilocytic (WHO grade I) histology, can present within the spectrum of astrocytic neoplasia including glioblastoma (WHO grade IV). In certain cases, histologic features alone make the distinction between pilocytic and diffuse astrocytomas difficult. We reviewed 22 cases of optic nerve gliomas, 19 of which were pilocytic astrocytomas (PA), and 3 of which were diffuse, non-pilocytic astrocytomas. The cases were evaluated for their clinical course, radiographic appearance, histologic grade, and proliferation indices as detected by MIB-1 (Ki-67) and p53 antibodies. Of the 19 PA, 14 showed no tumor growth by magnetic resonance imaging, and had Ki-67 and p53 labeling indices (LI) of 〈 1%. The other 5 PA exhibited aggressive behavior manifest by marked diffuse infiltrative tumor growth causing death in 2 patients, 1 of whom was diagnosed with neurofibromatosis type 1 (immunoperoxidase and radiographs not available), and marked local growth with an average time to growth of 39.3 months, a Ki-67 LI of 2–3%, and a p53 LI of 〈 1% in three others. Three of the five aggressive PA histologically demonstrated a finely reticulated pattern, a pattern that appears as an exaggeration or expansion of the normal neuroglia of the optic nerve, and may simulate a diffuse low-grade astrocytoma. Two demonstrated the coarsely reticulated pattern, with the biphasic and microcystic pattern typical of PA. Three diffuse astrocytomas (2 anaplastic astrocytomas and 1 glioblastoma) originated clinically and radiographically from the optic nerve, and revealed a Ki-67 LI of 2–12%, a p53 LI of 2–8%, and an average time to growth of 8 months. We conclude that the majority of PA of the optic nerve are non-aggressive, stabilize radiographically, and have Ki-67 and p53 LI 〈 1%. However, a subpopulation of PA has a propensity for aggressive behavior, and are identified by a Ki-67 LI of 2–3% and a p53 LI of 〈 1%. Diffuse astrocytomas have both Ki-67 and p53 LI 〉 2%. Thus, in cases of aggressive optic nerve tumors in which the histologic review of biopsy material cannot confidently confirm the diagnosis of pilocytic or diffuse fibrillary glioma, a p53 LI of 〉 1% appears to favor the diagnosis of diffuse astrocytoma.

    Notes: Summary We have analysed the distribution of the ΔF508 mutation and the haplotypes of cystic fibrosis (CF) bearing chromosomes among the Israeli CF population. The population was classified according to its ethnic origin and included 3 groups, Ashkenazi Jews, Sephardic/Oriental Jews and Arabs. Haplotype B (KM19 allele 2, XV2c allele 1) was found to be the predominant haplotype in all groups but in each of them the haplotype distribution was different. The ΔF508 mutation was present in all groups and accounts for 32% of the CF mutations. It was mainly associated with the B haplotype but only one third of the CF chromosomes with this haplotype carry the ΔF508 mutation.

    Notes: Abstract Cerebral sparganosis, a parasitic disease, rarely produces a chronic active inflammatory response in the brain. Clinically and radiographically the process may mimic a neoplasm. We report a 30-year-old man who underwent surgical exploration for a mass in the insular cortex. Histology revealed a densely fibrotic mass heavily infiltrated with plasma cells and lymphocytes, in which were embedded parasitic forms consistent with sparganosis. We describe the MRI appearances and pathologic features. Intracranial mass lesions secondary to sparganosis must be considered in patients with a history of travel to endemic areas, especially Asia.

    Description / Table of Contents: Zusammenfassung Nach Adaptation an ein Licht-Dunkelprogramm (8.00 bis 20.00 Licht, 20.00 bis 8.00 Dunkelheit) wurde der Acetylcholingehalt (ACh) im Mittelhirn und N. caudatus von Ratten sowie im Gesamthirn von Mäusen bestimmt. Die höchsten ACh-Konzentrationen wurden bei Ratten um 24.00 gefunden; die höchsten ACh-Werte bei der Maus waren nicht signifikant von den Konzentrationen um 24.00 und 12.00 verschieden. Signifikante ACh-Minima traten im Mittelhirn der Ratte um 12.00, im N. caudatus sowie im Mäusegehirn um 18.00 Uhr auf. Während der Dunkelperiode wurde die größte Toxicität gegenüber i.v. bzw. i.p. injiziertem ACh bei Mäusen gefunden. Ähnliche zeitliche Unterschiede ergaben sich für Neostigmin, Pilocarpin und Oxytremorin. Die Toxicität von Carbachol zeigte Maxima bei 12.00 und 24.00. Für Physostigmin, einen CholinesteraseHemmstoff mit einem tertiären Stickstoff, lagen sowohl die Toxicitätsmaxima als auch die Toxicitätsminima in der Dunkelperiode. Die rhythmische Schwankung der Toxicität von i.p. verabfolgtem Atropin verhielt sich umgekehrt wie die der Cholinomimetica (außer Carbachol). Auch nach i.v. Applikation von Scopolamin und N-Methylatropin war ein entsprechender rhythmischer Einfluß auf die Toxicität nachweisbar. Die Vorbehandlung mit N-Methylatropin schwächte die Toxicität zwar ab, der Toxicitätsrhythmus gegenüber ACh und Physostigmin war aber nicht verändert. Die Toxicität von Cholinomimetica zeigt ein Maximum während der höchsten ACh-Konzentrationen im Gehirn und der höchsten Stoffwechselaktivität. Bei den anticholinergen Substanzen zeigte sich ein umgekehrtes Verhältnis. Die Beziehungen zum zentralen ACh-Gehalt sind unklar.
    Notes: Abstract Acetylcholine (ACh) levels were determined in rat midbrain and caudate nucleus and mouse whole brain in mature animals adapted 3 weeks to a programmed lighting schedule (08:00 to 20:00 h light; 20:00 to 08:00 dark). Peak ACh levels in the rat occurred at 24:00 h; in the mouse the peak at 06:00 h did not differ significantly from values obtained at 24:00 and 12:00 h. Significant trough values occurred at 12:00 h in rat midbrain and at 18:00h in rat caudate nucleus and mouse brain. In mice toxicity of I.V. and I.P. administered ACh is maximal during the dark. Neostigmine, pilocarpine and oxotremorine have toxicity patterns similar to ACh. Carbachol toxicity peaked at 12:00 and 24:00 h. With physostigmine, a tertiary anticholinesterase, peak and trough occurred during the dark. The atropine (I.P.) toxicity rhythm is a mirror image of the cholinomimetic rhythm (except for carbachol). I.V. scopolamine and atropine methyl nitrate patterns resemble atropine's but are less clear-cut. Atropine methyl nitrate pretreatment reduced ACh and physostigmine toxicity but did not alter the overall pattern. Cholinomimetic toxicity and brain ACh patterns are similar, peaking when metabolic activity is maximal. An inverse relationship holds for the anticholinergic drugs. The relationship to central ACh is uncertain.
