    Abstract: Trial of Labor After Cesarean is an important strategy for reducing the overall rate of cesarean delivery. Offering the option of vaginal delivery to a woman with a history of cesarean section requires the ability to manage a potential uterine rupture quickly and effectively. This requires infrastructure and organization of the maternity unit so that the decision-to-delivery interval is as short as possible when uterine rupture is suspected. We hypothesize that the organizational characteristics of maternity units in Belgium have an impact on their proposal and success rates of trial of labour after cesarean section. Methods We collected data on the organizational characteristics of Belgian maternity units using an online questionnaire. Data on the frequency of cesarean section, trial of labor and vaginal birth after cesarean section were obtained from regional perinatal registries. We analyzed the determinants of the proposal and success of trial of labor after cesarean section and report the associations as mean proportions. Results Of the 101 maternity units contacted, 97 responded to the questionnaire and data from 95 was included in the analysis. Continuous on-site presence of a gynecologist and an anesthetist was associated with a higher proportion of trial of labor after cesarean section, compared to units where staff was on-call from home (51% versus 46%, p  = 0.04). There is a non-significant trend towards more trial of labor after cesarean section in units with an operating room in or near the delivery unit and a shorter transfer time, in larger units ( 〉  1500 deliveries/year) and in units with a neonatal intensive care unit. The proposal of trial of labor after cesarean section and its success was negatively correlated to the number of cesarean section in the maternity unit (Spearman’ rho = 0.50 and 0.42, p value  〈  0.001). Conclusions Organizational differences in maternity units appear to affect the proposal of trial of labor after cesarean section. Addressing these organizational factors may not be sufficient to change practice, given that general tendency to perform a cesarean section in the maternity unit is the main contributor to the percentage of trial of labor after cesarean.

    Abstract: Stillbirth has been recognized as a possible complication of a SARS-CoV-2 infection during pregnancy, probably due to destructive placental lesions (SARS-CoV-2 placentitis). The aim of this work is to analyse stillbirth and late miscarriage cases in unvaccinated pregnant women infected with SARS-CoV-2 during the first two waves (wild-type period) in Belgium. Methods Stillbirths and late miscarriages in our prospective observational nationwide registry of SARS-CoV-2 infected pregnant women ( n  = 982) were classified by three authors using a modified WHO-UMC classification system for standardized case causality assessment. Results Our cohort included 982 hospitalised pregnant women infected with SARS-CoV-2, with 23 fetal demises (10 late miscarriages from 12 to 22 weeks of gestational age and 13 stillbirths). The stillbirth rate was 9.5‰ for singleton pregnancies and 83.3‰ for multiple pregnancies, which seems higher than for the background population (respectively 5.6‰ and 13.8‰). The agreement between assessors about the causal relationship with SARS-Cov-2 infection was fair (global weighted kappa value of 0.66). Among these demises, 17.4% (4/23) were “certainly” attributable to SARS-CoV-2 infection, 13.0% (3/23) “probably” and 30.4% (7/23) “possibly”. Better agreement in the rating was noticed when pathological examination of the placenta and identification of the virus were available, underlining the importance of a thorough investigation in case of intra-uterine fetal demise. Conclusions SARS-CoV-2 causality assessment of late miscarriage and stillbirth cases in our Belgian nationwide case series has shown that half of the fetal losses could be attributable to SARS-CoV-2. We must consider in future epidemic emergencies to rigorously investigate cases of intra-uterine fetal demise and to store placental tissue and other material for future analyses.

    Abstract: Context. The Rosetta spacecraft escorted Comet 67P/Churyumov-Gerasimenko for 2 yr along its journey through the Solar System between 3.8 and 1.24 au. Thanks to the high resolution mass spectrometer on board Rosetta, the detailed ion composition within a coma has been accurately assessed in situ for the very first time. Aims. Previous cometary missions, such as Giotto , did not have the instrumental capabilities to identify the exact nature of the plasma in a coma because the mass resolution of the spectrometers onboard was too low to separate ion species with similar masses. In contrast, the Double Focusing Mass Spectrometer (DFMS), part of the Rosetta Orbiter Spectrometer for Ion and Neutral Analysis on board Rosetta (ROSINA), with its high mass resolution mode, outperformed all of them, revealing the diversity of cometary ions. Methods. We calibrated and analysed the set of spectra acquired by DFMS in ion mode from October 2014 to April 2016. In particular, we focused on the range from 13–39 u q −1 . The high mass resolution of DFMS allows for accurate identifications of ions with quasi-similar masses, separating 13 C + from CH + , for instance. Results. We confirm the presence in situ of predicted cations at comets, such as CH m + ( m = 1−4), H n O + ( n = 1−3), O + , Na + , and several ionised and protonated molecules. Prior to Rosetta, only a fraction of them had been confirmed from Earth-based observations. In addition, we report for the first time the unambiguous presence of a molecular dication in the gas envelope of a Solar System body, namely CO 2 ++ .

    Abstract: To understand how phosphorus (P)-bearing molecules are formed in star-forming regions, we have analysed the Atacama Large Millimeter/Submillimeter Array (ALMA) observations of PN and PO towards the massive star-forming region AFGL 5142, combined with a new analysis of the data of the comet 67P/Churyumov–Gerasimenko taken with the Rosetta Orbiter Spectrometer for Ion and Neutral Analysis (ROSINA) instrument onboard Rosetta. The ALMA maps show that the emission of PN and PO arises from several spots associated with low-velocity gas with narrow linewidths in the cavity walls of a bipolar outflow. PO is more abundant than PN in most of the spots, with the PO/PN ratio increasing as a function of the distance to the protostar. Our data favour a formation scenario in which shocks sputter phosphorus from the surface of dust grains, and gas-phase photochemistry induced by UV photons from the protostar allows efficient formation of the two species in the cavity walls. Our analysis of the ROSINA data has revealed that PO is the main carrier of P in the comet, with PO/PN & gt; 10. Since comets may have delivered a significant amount of prebiotic material to the early Earth, this finding suggests that PO could contribute significantly to the phosphorus reservoir during the dawn of our planet. There is evidence that PO was already in the cometary ices prior to the birth of the Sun, so the chemical budget of the comet might be inherited from the natal environment of the Solar system, which is thought to be a stellar cluster including also massive stars.

    Abstract: Type of funding sources: None. Background Autonomic dysfunction is a common complication of acute ischemic stroke and has been associated with poor functional outcome and increased mortality. We investigated the potential relation between the myocardial washout rate (WOR) of 123I-meta-iodobenzylguanidine (123I-mIBG), as a measure of cardiac sympathetic activity, and functional outcome in acute ischemic stroke.  Methods 38 patients with ischemic stroke (11 females, 72 years old [61-81)), underwent myocardial 123I-mIBG scintigraphy within the first week after stroke onset. Early (10 minutes post-injection (pi)) and late (4 hours pi) planar scans of the thoracic region were made. Regions of interest (ROI) were drawn over the mediastinum and the heart, and heart-to-mediastinum ratio (HMR) was calculated. Myocardial WOR was calculated as follows: (ROI heart early – ROI heart late)/ (ROI heart early) x 100%. Counts were corrected for background and counts in ROI heart late were corrected for decay. Patients were divided in 2 groups: those with a good functional outcome, defined as modified Rankin Scale (mRS) ≤ 2 at 3 months after stroke (i.e., patient is functionally independent), and those with a poor functional outcome, defined as a mRS & gt; 2.  Results Median WOR was 27,4 % (IQR 10,4-43,6). In univariate analysis, poor functional outcome after stroke was associated with age, stroke severity on admission (measured by the National Institutes of Health Stroke Scale (NIHSS)), beta-blocker use before and during hospitalization, WOR and late HMR. In subsequent multivariate analysis WOR (OR 1.087; 95% CI 1.003-1.177, p = 0.042) was an independent predictor of poor stroke outcome even after adjustment for age and NIHSS. Conclusions In patients with acute ischemic stroke, myocardial washout of 123I-mIBG predicts stroke outcome, even after adjustment for age and stroke severity on admission.

    Abstract: Context. Isotopic abundances in comets are key to understanding and reconstructing the history and origin of material in the Solar System. Data for deuterium-to-hydrogen (D/H) ratios in water are available for several comets. However, no long-term studies of the D/H ratio in water of a comet during its passage around the Sun have been reported thus far. Linear alkanes are important organic molecules that have been found on several Solar System bodies, including comets. To date, the processes of their deuteration are still poorly understood, only the upper limits of isotopic ratios for D/H and 13 C/ 12 C in linear alkanes are currently available. Aims. The aim of this work is to carry out a detailed analysis of the D/H ratio in water as a function of cometary activity and spacecraft location above the nucleus. In addition, a first determination of the D/H and 13 C/ 12 C ratios in the first four linear alkanes, namely, methane (CH 4 ), ethane (C 2 H 6 ), propane (C 3 H 8 ), and butane (C 4 H 10 ) in the coma of 67P/Churyumov-Gerasimenko is provided. Methods. We analysed in situ measurements from the Rosetta/ROSINA Double Focusing Mass Spectrometer (DFMS). Results. The D/H ratio from HDO/H 2 O and the 16 O/ 17 O ratio from H 2 16 O/H 2 17 O did not change during 67P’s passage around the Sun between 2014 and 2016. All D/H ratio measurements were compatible within 1 σ , with the mean value of 5.01 × 10 −4 and its relative variation of 2.0%. This suggests that the D/H ratio in 67P’s coma is independent of heliocentric distance, level of cometary activity, or spacecraft location with respect to the nucleus. Additionally, the 16 O/ 17 O ratio could be determined with a higher accuracy than previously possible, yielding a value of 2347 with a relative variation of 2.3%. For the alkanes, the D/H ratio is between 4.1 and 4.8 times higher than in H 2 O, while the 13 C/ 12 C ratio is compatible, within the uncertainties, with the available data for other Solar System objects. The relatively high D/H ratio in alkanes is in line with results for other cometary organic molecules and it suggests that these organics may be inherited from the presolar molecular cloud from which the Solar System formed.
