    Notes: Summary The antihypertensive efficacy of minoxidil and captopril was compared in 23 males with essential or renal parenchymatous hypertension refractory to conventional antihypertensive drug therapy. Following a pretreatment period the patients were randomly assigned to receive either minoxidil, 2.5 mg twice daily (n=12), or captopril, 25 mg twice daily (n=11). In patients with diastolic blood pressure 〉95 mmHg, doses of minoxidil and captopril were increased in 2-week intervals. Patients who maintained diastolic pressure 〉95 mmHg and/or those with intolerable side effects were switched over to the alternative substance. After a mean observation period of 12 weeks a significant decrease in systolic and diastolic blood pressure was observed (179/114 vs 148/92 mmHg in the minoxidil group; 176/111 vs 158/97 mmHg in the captopril group). The primary response rate was 75% in patients treated with minoxidil and 55% in those with captopril (not significant). After the change to the alternative substance two of the four non-responders on captopril and one of the two non-responders on minoxidil became responders. Side effects occurred significantly more often during minoxidil than captopril (p〈0.05). The high efficacy of minoxidil and captopril in the treatment of severe hypertension refractory to conventional drugs was confirmed. Minoxidil lowered blood pressure slightly more than captopril, but it had a higher incidence of side effects than captopril.

    Notes: Summary The clinical appearance, typical localization, diagnostic procedure and the treatment of hemangiosarcoma — the most frequent malignant tumor of the heart are reported by the case of a 27 years old woman. The patient suffered from dyspnea, congestion of the superior caval vein and paroxysmal tachycardia. X-ray showed cardiac enlargement due to pericardial effusion. Echocardiography revealed a large tumor in the right atrium. Computertomography and angiocardiography showed tumor masses at the orifice of the superior caval vein and a bypass of the blood flow via the azygos vein. Thoracotomy yielded an inoperable hemangiosarcoma. By the combined treatment of irradiation and chemotherapy the cardiac tumor completely disappeared, the patient was temporary symptomless. Later metastases occured and the patient died 13 months after diagnosis.

    Notes: Summary In 16 essential hypertensives on a program of energy restriction (800 kcal/day) with and without simultaneous salt restriction, the effects on blood pressure and intracellular Na+ and Ca2+ in red blood cells were studied. A decrease in blood pressure and intracellular free Na+ and Ca2+ was only observed in the cases of simultaneous energy and salt restriction. The beneficial effect of weight reduction in hypertension thus depends on a diminished salt intake and is probably mediated by changes in intracellular free Ca2+.

    Notes: Summary An increase in total urinary neopterin was observed in 12 of 13 patients with acquired immunodeficiency syndrome (AIDS), seven of 13 patients with lymphadenopathy, one of six healthy homosexual males, seven of ten adult patients with staphylococcal pneumonia, 11 of 12 children with viral infections, four of seven children with bacterial infections, and 12 of 13 children with various immune defects. Extremely high values of total urinary neopterin and monapterin were observed in severely ill patients with AIDS and those with familial hemophagocytic lymphohistiocytosis. Neopterin excretion was normal in two AIDS patients with Kaposi's sarcoma, but without opportunistic infections at that time. On reexamination of one of these patients later on, elevated neopterin values were noted. Parallel increases in neopterin and monapterin were found, whereas biopterin was usually normal. The increase in total neopterin was mainly due to 7,8-dihydroneopterin and was accompanied by an increase in 3′-hydroxysepiapterin. Increased neopterin in urine is assumed to reflect the increase in GTP pool and GTP cyclohydrolase I activity as observed in stimulated monocytes. Thus, neopterin, as a measure of the activation of the nonspecific cellular immune system, may be used diagnostically to detect allograft rejection after transplantations and to follow-up HTLV-III positive patients.

Das Konzept der patientenorientierten Zweierkombination erlaubt es dem praktizierenden Arzt, eine bei seinen Patienten möglichst optimale Therapie zu finden. Das Konzept beruht auf der Erfahrung einer individuell stark unterschiedlichen Ansprechbarkeit auf die möglichen Zweierkombinationen. Die früher oft verwendete Dreierkomponententherapie des Stufenschemas kommt nur noch in seltenen Fällen zum Einsatz.

    Description / Table of Contents: Directed Synthesis of Translationally Isomeric [3]-Catenanes1)In a multi-step reaction sequence the tetrahydroxymetacyclophane 12c is synthesized. Acetalisation with 1,25-dichloro-13-pentacosanone followed by nitration and reduction afforded the diamine 13c. By cyclization of this compound in 2-pentanol with sodium carbonate and sodium iodide under high dilution conditions the monomeric products 16, 17, and 18 are obtained in yields of 21.4, 7.7, and 0.9%, respectively. On the basis of mass, 13C NMR and 1H NMR spectra the structure of these compounds is discussed. Starting from the precatenane 16 the [3]-catenanes 25a, b,c and 26a, b, c are obtained in a multi-step reaction sequence. The structure of these compounds is confirmed by mass, 13C NMR, and 1H NMR spectroscopic investigations.
    Notes: In einer vielstufigen Synthese wird das Tetrahydroxymetacyclophan 12c synthetisiert. Durch Acetalisierung mit 1,25-Dichlor-13-Pentacosanon, nachfolgender Nitrierung und Reduktion wird das Diamin 13c erhalten. Dessen Cyclisierung in 2-Pentanol in Gegenwart von Natriumcarbonat und Natriumiodid unter Verdünnungsbedingungen ergibt die monomeren Cyclisierungsprodukte 16, 17 und 18 in Ausbeuten von 21.4. 7.7 und 0.9%. Die Struktur dieser Verbindungen wird anhand der Massen-, 13C- und 1H-NMR-Spektren diskutiert. In einer mehrstufigen Reaktionsfolge werden aus dem Prä-[3]-catenan 16 die [3]-Catenane 25a, b, c und 26a, b, c erhalten. Die Struktur dieser Verbindungen wird durch 13C-NMR, 1H-NMR- und massenspektroskopische Untersuchungen bewiesen.

    Notes: Pumps and compressors for high pressures - strength, seals, wear. In high pressure processes pumps and compressors are used for the pressure range of 100 to 4 000 bar. Very careful design is necessary because of the extreme mechanical demands. The special features of high pressure technology are explained. The methods used for generating high pressures - displacement or fluid flow machine - impose design limitations, partly by the final temperature, Mach number, material fatigue, or vibrations. A survey of typical high pressure pumps and compressors shows the range and limitations of application. There is a definite trend to leak-free systems. The best design of thick-walled components with pulsating pressureload avoids grooves, requires smooth surfaces, and uses tough, clean, and high-strength steels. The various designs of dynamic seals are explained and the state of the art is depicted. Detailed reference is also made to the exceptionally difficult wear problem of oscillating pump valves with abrasive suspensions.
