    Notes: Objective To assess the feasibility of autofluorescence spectroscopy in the diagnosis of cervical intraepithelial neoplasia (CIN) using broadband light excitation.Design Feasibility study.Setting Colposcopy clinic of an university hospital.Population Sixty-eight patients at risk for CIN.Methods After excitation with a broadband light between 375 and 440 nm, spectral distribution of native tissue fluorescence (autofluorescence) was acquired from 685 cervical sites for the localisation and differentiation of CIN, and compared with colposcopically directed biopsy and human papillomavirus (HPV) DNA testing.Main outcome measure Detection of CIN.Results The evaluation of spectral measurements revealed significantly lower autofluorescence values for CIN 3 lesions compared with normal tissue (P 〈 0.001), and compared with CIN 1 or CIN 2 (P 〈 0.002). High grade CIN lesions (CIN 2/3) presented with a significant reduced autofluorescence compared with CIN 1 (P 〈 0.002). Patients with a positive HPV DNA testing showed a significantly lower autofluorescence than patients tested negative for HPV DNA (P 〈 0.05). Severe inflammation such as chronic cervicitis may lead to false positive results.Conclusions Autofluorescence spectroscopy represents an interesting approach for the detection of cervical neoplasia. Using an excitation wavelength band between 375 and 440 nm, significant differences between normal and precancerous lesions of the cervix can be seen.

    Notes: Abstract  Background:  Urologists often perform procedures on anterior urethra, which is one of the most sensitive parts in male patients. The aim of the present study was to determine the efficacy of intracorpus spongiosum anesthesia during procedures on anterior urethra in the outpatient clinic.Methods:  A dosage of 3 mL of 1% lidocaine was slowly injected into the glans penis of 51 male patients. Immediately following injection, they underwent different procedures on anterior urethra. The subjective experience of pain in the patients was assessed by questionnaire according to a pain scale.Results:  Forty-seven patients (92.16%, 47/51) had zero pain and four cases (7.84%) had either minor or moderate discomfort, which was limited and tolerable. The anesthetic effect is immediate and has been very satisfactory. All the procedures (mean duration, 33 min), were successfully completed under the intracorpus spongiosum anesthesia. The act of injection into the glans caused instantaneous minor pain in 45 patients (88.23%, 45/51), moderate pain in four patients and no pain in two patients. There were no serious complications with this anesthetic technique, although two cases had instantaneous trance during injection of lidocaine.Conclusions:  Minor procedures on anterior urethra in an outpatient setting with intracorpus spongiosum anesthesia are generally painless, safe, simple, successful and inexpensive compared to those performed in a hospital.

    Notes: In response to heat shock (34°C, 30 min), cell morphology and actin organization in Dictyostelium discoideum are drastically changed. Loss of pseudopodia and disappearance of F-actin-containing structures were observed by using fluorescence microscopy. These changes were paralleled by a rapid decrease of the F-actin content measured by a TRITC-phalloidin binding assay. The effects of heat shock on cell morphology and actin organization are transient: After heat shock (34°C) or during a long-term heat treatment (30°C), cell morphology, F-actin patterns and F-actin content recovered/adapted to a state which is characteristic for untreated cells. Because F-actin may be stabilized by increased amounts of heat shock proteins, their response and interaction with F-actin was analyzed. After a 1 h heat treatment (34°C), the major heat shock protein of D. discoideum (HSP70) showed maximally increased synthesis rates and levels. During recovery from a 34°C shock or during a continuous heat treatment at 30°C, the HSP70 content first increased and then declined slowly toward normal levels. Pre-treatment of cells with a short heat shock of 30 min at 34°C stabilized the F-actin content when the cells were exposed to a second heat shock. Furthermore, a transient colocalization of HSP70 and actin was observed at the beginning of heat treatment (30°C) using immunological detection of HSP70 in the cytoskeletal actin fraction.

    Notes: Abstract In this article a GIS method is presented for riparian environmental buffer generation. It integrates a scientifically tested buffer width delineation model into a GIS framework. Using the generally available data sets, it determines buffer widths in terms of local physical conditions and expected effectiveness. Technical burdens of data management, computation, and result presentation are handled by the GIS. The case study in which the method was used to evaluate the stream buffer regulations in a North Carolina county demonstrates its capability as a decision support tool to facilitate environmental policy formulation and evaluation, and environmental dispute resolution.

    Notes: Abstract This article utilizes the theory of mixtures to formulate a general theory of wave propagation with mass-coupling effect in fluid-saturated porous media. An attempt is made to discuss the physical interpretation and the thermodynamic restriction of the coefficients appearing in the equations obtained. By the comparison it is shown that Biot's classical theory and the present one are essentially consistent. Also, wave velocities in some special cases are calculated, from which it is concluded that mass-coupling and permeability of media greatly affect wave propagation behavior.

    Notes: Abstract The physical-cover-oriented variational principle of nonlinear numerical manifold method (NNMM) for the analysis of plastical problems is put forward according to the displacement model and the characters of numerical manifold method (NMM). The theoretical calculating formulations and the controlling equation of NNMM are derived.     Description: Highlights • Method and application to improve digital soil maps of silt and clay in China • Within the framework of a DSM approach we derived spatial uncertainties. • Spatial uncertainty is based on randomized decision trees. • Model calibration set is refined by purposive sampling in area of high uncertainty. • Method and map refinement is approved using accuracy and uncertainty measures. Digital soil mapping (DSM) products represent estimates of spatially distributed soil properties. These estimations comprise an element of uncertainty that is not evenly distributed over the area covered by DSM. If we quantify the uncertainty spatially explicit, this information can be used to improve the quality of DSM by optimizing the sampling design. This study follows a DSM approach using a Random Forest regression model, legacy soil samples, and terrain covariates to estimate topsoil silt and clay contents in a small catchment of 4.2 km2 in the Three Gorges Reservoir Area, Central China. We aim (i) to introduce a method to derive spatial uncertainty, and (ii) to improve the initial DSM approaches by additional sampling that is guided by the spatial uncertainty. The proposed uncertainty measure is based on multiple realizations of individual and randomized decision tree models. We used the spatial uncertainty of the initial DSM approaches to stratify the study area and thereby to identify potential sampling areas of high uncertainties. Further, we tested how precisely available legacy samples cover the variability of the covariates within each potential sampling area to define the final sampling area and to apply a purposive sampling design. For the final Random Forest model calibration, we combined the legacy sample set with the additional samples. This uncertainty-driven DSM refinement was evaluated by comparing it to a second approach. In this second approach, the additional samples were replaced by a random sample set of the same size, obtained from the entire study area. For the comparative analysis, external, bootstrap-, and cross-validation was applied. The DSM approach using the uncertainty-driven refinement performed best. The averaged spatial uncertainty was reduced by 31% for silt and by 27% for clay compared to the initial DSM approach. Using external validation, the accuracy increased by the same proportions, while showing an overall accuracy of R2 = 0.59 for silt and R2 = 0.56 for clay.
