    Notes: Abstract: A 78-year-old man presented simultaneously with subacute cutaneous lupus erythematosus (SCLE) and Sweet syndrome (neutrophilic dermatosis). He was subsequently investigated for and found to have Sjögren syndrome. We believe that the simultaneous occurrence of the above cutaneous manifestations are very suggestive of and should prompt investigation for Sjögren syndrome.

    Notes: Background. We report our experience with two patients with adult onset of chickenpox in the setting of longstanding steroid therapy for nephrotic syndrome. Ours is a 430-bed tertiary care teaching hospital. The Wellesley Hospital, Toronto, Ontario. Both patients presented as self-referrals to the emergency department. Methods. The clinical suspicion of chickenpox was rapidly confirmed in both cases by a Tzanck smear preparation, by viral cultures of the vesicle, serology, and skin biopsy. In both patients therapy with high dose acyclovir, 10 mg/kg q8h, intravenously, was instituted based on clinical presentation. Results. Delay in clinical recognition and treatment in our first case resulted in death due to multiorgan failure (MOF). Improved awareness and rapid treatment of the second patient had a favorable outcome with no sequelae. Conclusions. Chickenpox is not only a childhood illness. Although rare in the adult population, it is associated even in the nonimmunocompromised host with increased morbidity and mortality. Steroid therapy predisposes to early dissemination and a potentially fatal outcome. Adults with immunosuppression should receive prompt systemic treatment with acyclovir.

    Notes: : Woringer-Kolopp disease usually manifests as a solitary cutaneous patch or several patches confined to a single body region. While most cases have a good prognosis, there have been rare reports of cases with an aggressive course and a fatal outcome. The authors present two women who are alive with this condition, which, they believe is a variant of mycosis fungoides.

    Notes: A 16-year-old Vietnamese man presented to the Dermatology Clinic with a 10-year history of bizarre brown patches, which initially started as red asymptomatic ‘‘bumps’' on the trunk, upper and lower extremities, and face. His past medical history was significant for hypothyroidism and idiopathic urticaria. He was on Eltroxin for hypothyroidism. The family history was noncontributory.Physical examination revealed two types of lesion: erythematous, well-circumscribed papules in a linear configuration along with linear hyperpigmented atrophic patches following Blaschko's lines were noted on the lower extremities (〈link href="#f1"〉Fig. 1), right upper extremity, right flank (〈link href="#f2"〉Fig. 2), and right jawline.〈figure xml:id="f1"〉1〈mediaResource alt="image" href="urn:x-wiley:00119059:IJD095-2:IJD_095b_f1"/〉Hyperpigmented atrophic macules involving the lower extremities〈figure xml:id="f2"〉2〈mediaResource alt="image" href="urn:x-wiley:00119059:IJD095-2:IJD_095b_f2"/〉Hyperpigmented atrophic macules involving the trunk and upper extremityInitial biopsies taken from the papular lesions on the right thigh and right elbow revealed the following changes. The first biopsy showed a slightly thinned epidermis with prominent dilated blood vessels in the superficial dermis. There also appeared to be a slight increase in the amount of collagen in the deep dermis. The findings were reported as in keeping with ‘‘epithelial atrophy.’' The second biopsy from the lesion on the right elbow revealed an acanthotic epidermis. The granular layer was absent in several areas and there was marked overlying parakeratosis. In the dermis, there was a heavy perivascular lymphocytic infiltrate. The appearances were consistent with a psoriasiform dermatitis (〈link href="#f3"〉Fig. 3). A biopsy taken from the left thigh approximately 18 months later showed slight irregular acanthosis with dermal edema, dilated blood vessels, and a patchy lymphocytic infiltrate.     Notes: Scrotal glans penis (glans penis plicatum), is manifested by thin deep vertical furrows and tiny papillomas of the glans penis. The condition is similar to the changes seen in scrotal tongue (lingua plicata). Both conditions were seen in the same patient. We are unaware of similar cases described previously. A 26 year old Caucasian married man, complained of a two-year history of tenderness of the head of the penis occurring during sexual intercourse, at about the time of ejaculation. The patient had been aware of cracks on the head of the penis since before the age of six and wondered if these were responsible for his recent discomfort.The patient was aware that his tongue had grooves which did not bother him, but he could not specify when this was first noticed. No family member was known to have a similar penile or tongue condition. On the advice of his general practitioner, he had used on his penis an antifungal cream, ketoconazole (Nizoral), and a topical steroid cream, mometasone furoate (Elocom), without effect. He was otherwise in good health and not on any medication. There was a history of allergy to penicillin.Physical examination revealed that the penis was circumcised. The glans was of normal size and color and had several parallel, thin furrows running from the urethral opening to the corona. Multiple tiny, skin-colored papillomas were present on the glans with the majority on its dorsal aspect. Separating the furrows by pulling the skin folds apart, revealed no cracks or bleeding (Fig. 1). The scrotum was normal and showed no similar changes.The tongue showed typical features of scrotal tongue (lingua plicata) (Fig. 2).
