    Notes: Die rot-grüne Koalition hat die Eckpunkte der geplanten Gesundheitsreform 2000 bekanntgegeben. Die Kassen sollen künftig eine wichtigere Rolle bei der Honorarverteilung spielen. Denn die ärztliche Selbstverwaltung habe eine gerechte Verteilung der Geldmittel bislang nicht zustande gebracht, so Bundesgesundheitsministerin Andrea Fischer. Ob die Kassen dazu imstande und willens sind? Besser wäre es, eine Demokratisierung der KVen voranzutreiben. Dr. Jürgen Fischer, BDR-Vorsitzender, über die Vorstellungen, die der BDR in die laufende Diskussion einbringt.

    Notes: Abstract Locally advanced non-small-cell lung cancer (NSCLC) in most cases is not curable at the present time. Owing to the local extent of the tumor, the rate of complete resections is low and, therefore, survival in these patients is poor. For this reason, induction chemotherapy is being investigated in patients expected to have a poor prognosis after standard surgery and radiotherapy. The rationale for induction chemotherapy is to increase the rate of complete resections and achieve early elimination of micrometastases. Clinical investigations have reported an improvement of survival in stage III NSCLC after induction chemotherapy by using different combinations of cytotoxic drugs. Vindesine ranks among the most active single agents in this disease and has been part of a number of combination regimens in induction chemotherapy. The combination of mitomycin, vindesine or vinblastine and cisplatin has produced encouraging results in several studies, indicating a possible improvement of survival in stage III NSCLC, although its superiority to other combinations yet has to be demonstrated.

    Notes: Abstract  We investigated the origin of galanin-positive nerve fibers on motor endplates in rat esophagus using anterograde 1,1′-dioleyl-3,3,3′,3′-tetramethylindocarbocyanine methane sulfonate (DiI) tracing from the nucleus ambiguus combined with galanin immunocytochemistry and calcitonin gene-related peptide immunocytochemistry. To demonstrate spatial relationships of galanin-positive nerve fibers to vagal and enteric nerve fibers on motor endplates, we combined galanin immunocytochemistry with calcitonin gene-related peptide immunostaining for labeling of vagal terminals, and vasoactive intestinal peptide immunoreactivity and NADPH-diaphorase histochemistry for demonstration of enteric nerve fibers. Within fine varicose nerve fibers, galanin was colocalized with vasoactive intestinal peptide and NADPH-diaphorase to a high degree and turned out to be completely separated from calcitonin gene-related peptide-positive or anterogradely DiI-labeled vagal motor terminals. These results indicate that the enteric nervous system is the most important and possibly the only source of galanin-positive nerve terminals on motor endplates in rat esophagus. Galanin may be, in addition to nitric oxide and vasoactive intestinal peptide, a mediator of the enteric coinnervation of striated muscle in this organ.

    Description / Table of Contents: During the last fifty years the chemistry of chloroamine has been developed intensively. Many mechanisms for the reaction between chloroamine and ammonia have been forwarded and discarded again. Today, two mechanisms are discussed which so far both cannot be discarded: The SN2-mechanism and the mechanism including nitrogen monohydride as an intermediate. Nitrogen monohydride, prepared hitherto from hydrogen azide, ammonia, and hydrazine can be isolated at lower temperatures in probably oligomeric state.
    Notes: Nach über einem halben Jahrhundert intensiver Entwicklung der Chloraminchemie und nach Aufstellung und Widerlegung vieler Mechanismen für die Chloramin-Ammoniak Reaktion stehen sich heute zwei Mechanismen gegenüber, die beide durch die bisherigen Experimente nicht zu entkräften sind: Der SN2-Mechanismus und der Mechanismus über Stickstoffmonohydrid als Zwischenprodukt. Stickstoffmonohydrid läßt sich bisher aus Hydrogenazid, Ammoniak und Hydrazin erhalten und bei tiefen Temperaturen in wahrscheinlich oligomerem Zustand ausfrieren.

    Description / Table of Contents: According to the method of Rice, Freamo, and Grelecki solid chloroamine has been decomposed at -190 °C by ultraviolet irradiation. The blue product formed is stable below -150 °C; it is already known from the analogous decomposition of hydrogen azide, and its formation can be understood only by the primary formation of nitrogen monohydride.  -  This result is supported by the identification of hydrogen cyanate during the thermal decomposition of gaseous chloroamine in the presence of carbon mon oxide (5 to 10 Torr, 500 °C). Under the same conditions, hydrazine too produces hydrogen cyanate.  -  Considering the result of Wannagat and Kohnen, who showed nitrogen monohydride and ammonia to form hydrazine, it follows that it is possible to synthesize hydrazine from chloroamine and ammonia via nitrogen monohydride as an intermediate.
    Notes: In Anlehnung an die Methodik von Rice, Freamo und Grelecki wurde festes Chloramin bei -190 °C durch ultraviolette Strahlung zersetzt. Das entstehende blaue Produkt, beständig unterhalb -150 °C, ist bereits durch die entsprechende Zersetzung des Hydrogenazids bekannt; seine Bildung ist nur durch primäres Auftreten von Stickstoffmonohydrid zu erklären.  -  Dieses Ergebnis wird durch den Nachweis von Hydrogencyanat im Anschluß an die thermische Zersetzung gasförmigen Chloramins in Gegenwart von Kohlenmonoxid (5 bis 10 Torr, 500 °C) bekräftigt. Auch Hydrazin liefert unter diesen Bedingungen Hydrogencyanat.  -  Unter Berücksichtigung des Befundes von Wannagat und Kohnen, daß Stickstoffmonohydrid mit Ammoniak zu Hydrazin reagieren kann, folgt, daß es ein experimentell gangbarer Weg ist, Hydrazin aus Chloramin und Ammoniak über Stickstoffmonohydrid als Zwischenprodukt zu synthetisieren.

    Description: The flow field in the area of what was thought to be the source region of the North Brazil Current (NBC) off the northeast coast of Brazil between 5 degrees 30'S and 10 degrees S was investigated in austral spring during November 1992 and compared with observations in October 1990. The data were taken with several different instruments, including vessel-mounted ADCP, lowered-ADCP, Pegasus, CTD and XBTs. The flow was found off the coast at 5 degrees 30'S as well as at 10 degrees S as an undercurrent, the North Brazil Undercurrent (NBUC). The NBUC shows a subsurface core at about 200 m depth with velocities of up to 90.0 cm s(-1), resulting in large northward transports of more than 22 Sv in the upper 1000 m. The transport is about the same at 5 degrees 30'S and 10 degrees S, hence no net inflow from the east is required to feed the NBUC. The climatological Ekman transport is to the south between 5 degrees 30'S and 10 degrees S, and in consequence the northward flow near the surface was reduced and might be one reason for the existence of the undercurrent. The flow near the coast was to the north at 10 degrees S, therefore the Brazil Current had to start as a coastal current south of 10 degrees S. For the zonal sections at 5 degrees 30'S and 10 degrees S the geostrophic computations relative to the density surface sigma(1) = 32.15 kg m(-3) (about 1150 m depth) resulted in transports comparable to those obtained from direct measurements. The results further show that the choice of a correct level of no motion can be supported by the direct observations. A shallower reference based on water mass boundaries alone would reduce the NBUC transport to almost zero. Computations with data from the historical data base for austral fall resulted in a weaker NBUC of less than 20 Sv near 10 degrees S, indicating a possible seasonal signal in the NBUC with a stronger NBUC in austral spring.
