Zusammenfassung Unter besonderer Berücksichtigung des mit der KBR quantitativ-verlaufsanalytisch bestimmten Au/SH-Antigengehaltes im Blutserum werden 3 Fälle von Virushepatitis B vorgestellt, die sich durch unterschiedlichen klinischen Verlauf mit unterschiedlichen Au/SH-Antigenkurven auszeichnen. 1. Eine unkompliziert verlaufende akute Serumhepatitis mit innerhalb von 6 Wochen kontinuierlich abfallenden Transaminasen-, Gesamtbilirubin- und Au/SH-Antigenwerten. 2. Eine akute Serumhepatitis im Anschluß an Bluttransfusion bei bestehender Leukämie. Hier war das Immunsystem durch cytostatische Therapie geschwächt; es kam zu einem Anstieg des Au/SH-Antigentiters auf exzessiv hohe und bis zum Tode des Patienten unverändert anhaltende Werte. Im Gegensatz dazu standen nur gering erhöhte Transaminasenund Gesamtbilirubinbefunde. 3. Eine chronisch-aggressive Hepatitis. Bei diesem Patienten wurde ein hoher Antigentiter über 20 Wochen hinweg beobachtet. Auch hier korrelierten die Werte der Transaminasen und des Gesamtbilirubins nicht mit dem Au/SH-Antigentiter, so daß letzterer als einziger serologischer Parameter einen Anhalt für die Schwere des Krankheitsbildes bot.

Summary The concentration of Australia-(Au/SH-)-antigen in serum of patients suffering from acute and chronicactive hepatitis has been determined quantitatively and was compared with the behaviour of transaminases and bilirubin. Out of a follow-up study of 20 patients three characteristic cases are demonstrated. 1. A case of an uncomplicated acute viral hepatitis, characterized by continuously decreasing values of SGOT, SGPT, overall-bilirubin and Australia-antigen titers within six weeks. 2. A case of a complicated acute post-transfusion hepatitis in a leukemic patient under immunosuppressive therapy, characterized by extraordinarily high Au/SH-antigen values but an only insignificant increase of clinical-chemical findings. The high Au/SH-values did not change until the death of the patient. 3. A case of a chronic-active viral hepatitis which was characterized by long lasting high serum Au/SH-antigen concentrations but very low pathological clinical-chemical values. Quantitative Au/SH-antigen determinations by means of the CFT might have some bearing on the prognosis and therapy of viral hepatitis (B).
