Zur Morphologie der Epithelkörperchen bei dem primären Hyperparathyreoidismus

On the morphology of the parathyroid in primary hyperparathyroidism

Summary

Forty five cases of primary hyperparathyroidism have been subjected to a careful clinical and morphological examination. The difficulties have been stressed of differentiating between primary parathyroid adenomas and the so-called primary hyper-plasias on pure morphological grounds. To differentiate between the two lesions, the presence of a rim on non-neoplastic parathyroid tissue at the margin of parathyroid adenomas is of paramount significance. Twenty six cases have been diagnosed as parathyroid hyperplasia on the basis of morphological findings only. Thirteen thereof had to be revalued as adenomas after having considered the clinical data.

Several interesting cases with a difficult differential diagnosis have been presented as brief case reports. They include one case of multiple adenomas, three cases of primary clear-cell hyperplasia, one case of chief-cell hyperplasia and one equivocal case of hyperparathyroidism.

The authors stress that a close co-operation between the internist, surgeon and pathologist is necessary for an adequate evaluation of the parathyroid lesions under discussion.

Zusammenfassung

An 45 Fällen von primärem Hyperparathyreoidismus wird auf die Schwierigkeiten der Differentialdiagnose zwischen primären Adenomen und primären Hyperplasien der Epithelkörperchen hingewiesen. Größte Bedeutung kommt dem Nachweis kappenförmiger Anlagerungen von nicht neoplastischem Drüsengewebe zu. In 26 Fällen wurde auf Grund des histologischen Befundes eine primäre Hyperplasie diagnostiziert; 13 dieser Fälle mußten später, auf Grund der klinischen Untersuchungsbefunde, den primären Adenomen zugeteilt werden.

6 differentialdiagnostisch besonders interessante und schwierige Fälle werden in extenso wiedergegeben: 1 Fall von multiplen Adenomen, 3 Fälle primärer Hyperplasie der wasserhellen Zellen, 1 Fall primärer Hauptzellhyperplasie und ein strittiger Fall.

Es wird auf die Notwendigkeit einer engen Zusammenarbeit zwischen Internisten, Chirurgen und Pathologen für die korrekte Bewertung der Epithelkörperchenbefunde hingewiesen.
