Summary
Serum levels of non-esterified fatty acids, free glycerol and esterified fatty acids were estimated in obese patients and in persons of normal weight after intravenous administration of tolbutamide and glucose. In obesity non-esterified fatty acids and glycerol levels were roughly twice as high as in the non-obese, whereas no difference in levels of esterified fatty acids was found between the two groups. Following administration of tolbutamide or glucose, there was a decrease of concentration of non-esterified fatty acids and glycerol to a minimum during the two hours of observation. In obesity, non-esterified fatty acids and glycerol decreased relatively more than in persons of normal weight. This behaviour is interpreted as due to increased insulin secretion in obesity. The difference in changes in concentration of non-esterified fatty acids and glyoerol in response to a variety of test situations may well be the result of both the glucose-dependent lipogenetic and the glucose-independent antilipolytic effect of insulin. The antilipolytic effect of insulin is discussed with reference to the metabolic aspects of obesity.
Résumé
Cette étude porte sur les taux d'acides gras non estérifiés, de glycérol libre, et d'acides gras estérifiés de personnes obèses et non obèses après l'administration intraveineuse de deux substances B-cytotropes: le tolbutamide et le glucose. Les taux d'acides gras libres et de glycérol dans le sérum prélevé à jeun sont deux fois plus élevés pour les obèses que pour les témoins. Par contre, les taux d'acides gras estériflés sont les mêmes pour les deux groupes. Après l'administration de tolbutamide ou de glucose on observe une diminution d'abord, puis un retour vers le taux initial, des acides gras libres et du glycérol, ceci au cours d'une période d'observation de deux heures. La dépression des taux d'acides gras libres et du glycérol est plus grande chez les obèses que chez les personnes de poids normal. Les auteurs pensent que ce comportement est à mettre en relation avec l'hypersécrétion insulinique caractéristique de l'obésité. Les variations des courbes de concentration des acides gras libres et du glycérol dans différentes conditions expérimentales sont probablement le résultat tant de l'action lipogénétique de l'insuline, action gluco-dépendante, que de son action antilipolytique, gluco-indépendante. La discussion porte sur les relations possibles entre l'action antilipolytique de l'insuline et le métabolisme de l'obésité.
Zusammenfassung
Es wurde das Verhalten der nicht veresterten Fettsäuren, des freien Glycerins und der veresterten Fettsäuren im Serum über- und normal-gewichtiger Personen nach intravenösen Belastungen mit den B-zytotropen Substanzen Tolbutamid und Glucose untersucht. Bei der Fettsucht fand sich im Nüchternserum eine doppelt so hohe Fettsäure- und Glycerinkonzentration gegenüber normalgewichtigen Kontroll-personen. Die veresterten Fettsäuren dagegen zeigten in beiden Gruppen keinen Unterschied. Nach Tolbutamidund Glucosegabe kam es zu einem Abfall der Metabolitkonzentrationen mit nachfolgendem Anstieg der nicht veresterten Fettsäuren und des Glycerins während des 2-stündigen Versuchsverlaufes. Dabei nahmen die nicht veresterten Fettsäuren und das Glycerin bei Fettsüchtigen relativ stärker ab als bei Normalgewichtigen. Dieses Verhalten wird auf die vermehrte Insulinsekretion bei der Adipositas zurückgeführt. Die unterschiedlichen Konzentrationsänderung von nicht veresterten Fettsäuren und Glycerin bei verschiedener Versuchsanordnung dürften Folge des Glucose-abhängigen, lipogenetischen und gleichzeitig des Gluoose-unabhängigen, antilipolytischen Effektes des Insulins sein. Die Bedeutung der antilipolytischen Wirksamkeit von Insulin wird unter dem metabolischen Aspekt der Fettsucht diskutiert.
