    Abstract: Transition of care means the process of educating and empowering adolescents and young adults to take an active role in their own healthcare, develop decision-making skills, and eventually transition from paediatric to adult healthcare providers. Most people don't switch doctors until they are young adults, but it can be beneficial to start preparing children earlier. Objectives To provide a toolset specifically suited for adult and paediatric rheumatologists to help them transition the care of children with rheumatic diseases from adult to paediatric rheumatology care. Methods The preliminary scientific committee identified a total of ten major clinical questions in this study, which was conducted to obtain expert consensus. To reach consensus on the transition of care for young adults and adolescents with rheumatic diseases, the core leadership team sought out researchers and clinicians with expertise in both paediatric and adult rheumatic diseases management. Results The expert panel's participation was restricted to an online survey (n = 18); each expert completed both rounds. After round 2, a total of 10 points had been collected. 88.9% to100% of respondents (ranks 7–9) indicated they agreed with the recommendations. The scientific committee's 10 clinical standards were all prepared in the same style. A structured template describing the integrated pathway for transition of care was developed using the responses to the structured key questions and the results of the literature review. Conclusion The created rheumatology-specific guideline provides young adults with juvenile rheumatic diseases with a focused, multidisciplinary transition of care strategy with equity of access, quality of care, and flexibility and establishes standards for transitional care. Ethics This study was performed in accordance with the Helsinki Declaration. The ‘Clinical, Evidence-based, Guidelines’ (CEG) initiative protocol was approved the local ethical committee: ethical approval code: 34842/8/21, ethical board Tanta University.

    Abstract: This paper investigates factors influencing the shaping of future learning environments. It focuses on the impact of social and cultural requirements on the sustainability of future learning environment. It argues that while today's learning environments are shaped by yesterday's visions, future learning environments are shaped by toady's' visions that might not be acceptable nor valid for future generations. The case of New Kuwait University City in Shedadiyah is used to illustrate how current social and cultural requirements impact the design of a future university campus and inhibit the production of a sustainable environment. Among several socio-cultural factors, the paper focuses on two significant aspects that have dramatically affected the development of the master plan for the New University City; namely separation of students' sexes and car parking requirements. The first requirement was mandated by a parliament decree to build two separate campuses; one for male students and the other for female students. The implementation of this requirement resulted in the duplication of many educational facilities and immensely increased space and budget requirements. The second requirement reflected dependency on automobiles as primary means of transportation in Kuwait. It resulted in a necessity to allocate large areas of land for vehicular traffic and car parking. These two requirements, as well as other socio-cultural requirements, created a great challenge towards achieving the required level of sustainability. The paper concludes that while recognizing that accommodating clients' social and cultural requirements is necessary for the application of a comprehensive sustainability strategy, these requirements might work against achieving required levels of other aspects of sustainability.

    Abstract: Tourism is a major economic source for Egypt, due to its significant natural and cultural attractions. Yet, rapid development and construction of touristic facilities have a negative impact on the fragile natural and cultural heritage. This paper studies the recent touristic developments of the coastal stretch of Ain-Sukhna on the Red Sea coastal region of Galala Mountain, and their impact on the surrounding natural and cultural attractions. Coral reefs and rich marine life have made this stretch among the prime fishing and scuba diving destinations in the world. The area is also famous for its year-round sunny beaches and the spectacular coastal scenic drive where Galala Mountain reaches the Red Sea. Recently, development has started on the mountains following the construction of Galala Mountain Road. Galala City started with Galala University and several residential, touristic, and commercial facilities. This paper studies the pattern of development in the area during the past 40 years and assesses its impact on natural and cultural resources.

    Abstract: On the staggering emergence of the Omicron variant, numerous questions arose about the evolution of virulence and transmissibility in microbes. Main body of the abstract The trade-off hypothesis has long speculated the exchange of virulence for the sake of superior transmissibility in a wide array of pathogens. While this certainly applies to the case of the Omicron variant, along with influenza virus, various reports have been allocated for an array of pathogens such as human immunodeficiency virus (HIV), malaria, hepatitis B virus (HBV) and tuberculosis (TB). The latter abide to another form of trade-off, the invasion–persistence trade-off. In this study, we aim to explore the molecular mechanisms and mutations of different obligate intracellular pathogens that attenuated their more morbid characters, virulence in acute infections and invasion in chronic infections. Short conclusion Recognizing the mutations that attenuate the most morbid characters of pathogens such as virulence or persistence can help in tailoring new therapies for such pathogens. Targeting macrophage tropism of HIV by carbohydrate-binding agents, or targeting the TMPRSS2 receptors to prevent pulmonary infiltrates of COVID-19 is an example of how important is to recognize such genetic mechanisms.

We aimed to develop a specific toolkit tailored to paediatric and adult rheumatologists to assist them in transitioning of care of young people with juvenile onset rheumatic musculoskeletal diseases from the paediatric to adult rheumatology care. Results The expert panel was confined to an online survey ( n  = 18), all the experts completed the two rounds. At the conclusion of round 2, a total of 10 points were gathered. The range of respondents (ranks 7–9) who agreed with the recommendations was 88.9 to 100%. All 10 clinical standards identified by the scientific committee were written in the same way. Based on the answers to the structured key questions and the literature review, a structured template was developed presenting transition of care integrated pathway. Conclusion The developed rheumatology-specific guideline offers adolescents and young adults a focussed, multidisciplinary transition of care approach with equity of access, quality of care and flexibility and set up standards for transitional care for young adults with juvenile rheumatological diseases.

    Abstract: The desert was always part of the Arabian culture and a way of life. After the discovery of oil during the 1940's, traditional Arab societies became urbanized and settled in newly constructed cities and towns. The desert became a place to visit for leisure and entertainment by locals and tourists. In Dubai, the desert is used as a tourist attraction for foreigners. In Kuwait, the desert is a seasonal attraction for families and groups during the winter time. This involves informal leisure activities that endanger the fragile desert environment and cause its pollution and degradation. This paper discusses the impact of informal desert tourism activities on the desert sustainability in Kuwait. It warns against the negative impact of current practices on the fragile desert environment as a result of informal local tourism activities. The paper explores successful examples from the region and the world in order to highlight aspects of successful interventions that blend in with the desert context. It suggests sustainable measures to deal with the planning and designing for desert ecotourism facilities that aim at protecting the fragile desert environment from deterioration and eventual extinction.
