    Kurzfassung: We studied whether slower community walking speed and whether greater time spent lying down or sleeping were associated with higher mortality in people with lower extremity peripheral artery disease (PAD). Participants with an ankle–brachial index (ABI) 〈 0.90 were identified from Chicago medical centers. At baseline, participants reported their usual walking speed outside their home and the number of hours they spent lying down or sleeping per day. Cause of death was adjudicated using death certificates and medical record review. Analyses were adjusted for age, sex, race, comorbidities, ABI, and other confounders. Of 1314 PAD participants, 189 (14.4%) died, including 63 cardiovascular disease (CVD) deaths. Mean follow-up was 34.9 months ± 18.1. Relative to average or normal pace (2–3 miles/hour), slower walking speed was associated with greater CVD mortality: no walking at all: hazard ratio (HR) = 4.17, 95% confidence interval (CI) = 1.46–11.89; casual strolling (0–2 miles/hour): HR = 2.24, 95% CI = 1.16–4.32; brisk or striding ( 〉 3 miles/hour): HR = 0.55, 95% CI = 0.07–4.30. These associations were not significant after additional adjustment for the six-minute walk. Relative to sleeping or lying down for 8–9 hours, fewer or greater hours sleeping or lying down were associated with higher CVD mortality: 4–7 hours: HR = 2.08, 95% CI = 1.06–4.05; 10–11 hours: HR = 4.07, 95% CI = 1.86–8.89; ⩾12 hours: HR = 3.75, 95% CI = 1.47–9.62. These associations were maintained after adjustment for the six-minute walk. In conclusion, slower walking speed outside the home and less than 8 hours or more than 9 hours lying down per day are potentially modifiable behaviors associated with increased CVD mortality in patients with PAD.

    Kurzfassung: Sustaining patient motivation for long-term adherence to drug therapies remains a substantial problem for physicians, other health care providers, the patients themselves, and their families. Other therapeutic requests such as dietary changes and weight control may be even more difficult to maintain than taking pills. As part of a controlled experimental design implemented in an outpatient teaching hospital, an educational program was implemented to improve family member support for medical compliance among hypertensive patients. Family members were interviewed, counseled, and provided with a booklet for the purpose of educating and involving them in the home management of high blood pressure. The booklet identified ways the family member could assist the patient with medication compliance, appointment keeping, as well as diet and weight control. These items were identified and recorded as behavioral objectives in the booklet. Patients were followed for three years to assess long-term outcomes. Results showed a strong statistically significant difference between the experimental and control groups, with the experimental group demonstrating higher levels of appointment-keeping behavior, weight control, and BP under control (all p values 〈 .001). Analysis of the main effects of the educational program dem onstrated that the family member support intervention accounted for the greatest decrease in diastolic blood pressure variability, R 2 = .20, p 〈 .001.

    Kurzfassung: Violence against women (VAW) is widespread in East Africa, with almost half of married women experiencing physical abuse. Those seeking to address this issue confront two challenges: some forms of domestic violence are widely condoned and it is the norm for witnesses to not report incidents. Building on a growing literature showing that education-entertainment can change norms and behaviors, we present experimental evidence from a media campaign attended by more than 10,000 Ugandans in 112 rural villages. In randomly assigned villages, video dramatizations discouraged VAW and encouraged reporting. Results from interviews conducted several months after the intervention show no change in attitudes condoning VAW yet a substantial increase in willingness to report to authorities, especially among women, and a decline in the share of women who experienced violence. The theoretical implication is that interventions that affect disclosure norms may reduce socially harmful behavior even if they do not reduce its acceptability.

    Kurzfassung: This study compared the Occupational Aptitude Survey and Interest Schedule (OASIS) scores of 135 community college students with different academic majors. Eighteen one-way ANOVAs were run with the OASIS subtests as the dependent variables and academic major as the independent variable. Students with mechanical-related majors scored higher on the Mechanical interest scale, students with business-related majors scored higher on the Business-Detail interest scale, and students with humanitarian-related majors scored higher on the Humanitarian interest scale than did other groups, providing evidence for the construct validity of the OASIS Interest Schedule.

    Kurzfassung: The entity commonly referred to as chondrolipoma is a rare and enigmatic breast lesion with unclear histogenesis and a complete lack of molecular characterization. It is uncertain whether it represents a hamartoma, choristoma, or a distinct neoplasm, including possibly a variant of mammary-type myofibroblastoma. We report two additional chondrolipomatous lesions of the breast. The lesions had varying histologic and immunohistochemical features similar to myofibroblastoma, including the loss of retinoblastoma (Rb) protein expression in one lesion. Molecular analysis by chromosomal microarray analysis performed on a second lesion did not demonstrate a loss of 13q14 or 16q typical of myofibroblastoma. Our findings further support the concept that at least a subset of breast lesions that historically have been classified as chondrolipoma are related to myofibroblastoma. However, the lack of myofibroblastoma-specific molecular alterations in one lesion suggests chondrolipomas may also have varying origins.

    Kurzfassung: Platelet adhesion is mediated by von Willebrand factor (vWF), and disintegrin-like and metalloprotease domain with thrombospondin type-1 motif, number 13 (ADAMTS13) is a protease that cleaves vWF. A change in the balance between vWF and ADAMTS13 in favor of thrombosis might occur shortly before ischemic cardiovascular (CV) events. Objective: To determine whether vWF, ADAMTS13, and the ratio of vWF and ADAMTS13 change during the months preceding an acute CV event. Design: Prospective longitudinal observational study. Setting: Outpatient. Patients: A total of 595 participants with peripheral artery disease (PAD). Measurements: Blood samples were obtained every 2 months for up to 3 years and hemostatic factors examined at intervals preceding events. Results: Sixty-one participants (cases) experienced events and were matched to 122 PAD controls. During the 2-month interval prior to an event, cases (n = 48) had higher levels of the vWF and ADAMTS13 than controls (n = 95; P = .05), but significance was lost after adjusting for the baseline differences in myocardial infarction, unstable angina, and stroke. During the 10 months prior to an event, median values for vWF and the ratio of vWF and ADAMTS13 were higher in cases than in controls, but the differences were not statistically significant. However, in a subset of 20 patients with complete bimonthly data, there was a trend toward an increase in the ratio in the 10 months prior to a CV event ( P = .04). Conclusion: In patients with PAD experiencing an ischemic CV event, a significant increase in the ratio of vWF to ADAMTS13 prior to the event could not be confirmed, although there was a weak trend in this direction.

    Kurzfassung: The purpose of this study was to investigate the construct validity of the Occupational Aptitude Survey and Interest Schedule (OA-SIS) by comparing interest and aptitude scores of a sample of 126 community college students majoring in different academic areas. Planned orthogonal comparisons revealed that science majors scored highest on the Scientific interest scale, mechanical majors scored highest on the Mechanical interest scale, business majors scored highest on the Business Detail interest scale, and humanitarian majors scored highest on the Humanitarian interest scale. These results replicate and extend the findings of Parker and Green (1987).

    Kurzfassung: The 2010 Haiti earthquake represents one of the most devastating earthquakes in history. Damage to structures was widespread across the city of Port-au-Prince, but its intensity varied considerably from neighborhood to neighborhood. This paper integrates damage statistics with geologic data, shear wave velocity measurements, and topographic information to investigate the influence of these conditions on the damage patterns in the city. The results indicate that the most heavily damaged areas in downtown Port-au-Prince are underlain by Holocene alluvium with shear wave velocities that average about 350 m/s over the top 30 m. The remainder of Port-au-Prince is underlain mostly by older geologic units with higher shear wave velocities. Damage was also concentrated on hillsides around Port-au-Prince. These pockets of damage appear to have been caused by a combination of factors, including topographic amplification, soil amplification, and failure of weakly cemented, steep hillsides.
