    Abstract: The aim of this study is to investigate the current status of the diagnosis and treatment of patients with pancreatic neuroendocrine neoplasms (pNENs) undergoing surgery in China. Methods: This is a multicenter cross-sectional study performed in China. Data from patients with pNENs undergoing surgery at 33 high-volume medical centers, where the number of pancreatectomies exceeds 20 cases per year, were collected and analyzed between March 1, 2016 and February 28, 2017. Results: In total, 392 patients with pNENs were enrolled. The male to female ratio was 1.4. The majority of patients were aged between 40 and 70 years. 65.6% of the patients had non-functional tumors. Among those with functional tumors, the percentages of insulinomas, gastrinomas, glucagonomas, and vasoactive intestinal peptide-secreting tumors were 94.8%, 1.5%, 2.2%, and 1.5%, respectively. Multidisciplinary team (MDT) discussion was conducted for 39.0% of the patients. Minimally invasive surgery was performed on 31.1% of the 392 patients. The incidence of grade B/C pancreatic fistula formation was 4.4%. A total of 89.0% of the surgeries achieved R0 resection, and 41.6% of the tumors were well differentiated. Lymph node metastasis was present in 8.9% of the patients. The percentages of patients with grades G1, G2, and G3 disease were 49.2%, 45.7%, and 5.1%, respectively. Conclusion: This multicenter cross-sectional study systematically presents the current status of the diagnosis and treatment of patients with pNENs undergoing surgery in China. MDT consultation for pNENs has not been widely implemented in China. Although the incidence of surgical complications is relatively low, minimally invasive procedures should be further promoted. This study shows us how to improve the outcomes of these patients.

    Abstract: The aim of this study is to investigate the current status of pancreatic cancer patients undoing surgical treatment in China and to find ways to improve the survival of these patients in the future. Methods: This study is a national, multicenter, cross-sectional study in China. Information regarding pancreatic cancer patients undergoing surgical treatment from 34 high-volume tertiary IIIA level hospitals was collected and analyzed from the March 1, 2016 to the February 28, 2017. Results: In total, 2200 pancreatic cancer patients were enrolled from 34 tertiary IIIA level hospitals in 16 provinces across China. The male-to-female ratio was 1.5. More than 80% of the patients were between 50 and 70 years old. The top 4 symptoms were epigastric discomfort, abdominal pain, jaundice, and weight loss. Carbohydrate antigen 19-9 and carcinoembryonic antigen were elevated in 70.9% and 27.1% of patients, respectively. A multidisciplinary team (MDT) discussion was carried out for 35.0% of patients before surgery. The proportion of minimally invasive pancreatic surgeries was approximately 20%. A total of 83.4% of the operations achieved R0 resection, and the incidence of grade 3/4 postoperative complications was 7.7%. Only 13.4% of the patients received postoperative adjuvant chemotherapy. The percentage of pathological stage I tumors was only 24.5%. Conclusion: The majority of pancreatic cancer patients undergoing surgical resection in China are in an advanced stage. The MDT consultations for pancreatic cancer have not been widely carried out. R0 resection has been achieved in most cases, with relatively low incidence of serious complications, but minimally invasive pancreatic surgery should be further promoted. The application of postoperative chemotherapy remains low. This national, multicentre, cross-sectional study comprehensively presents the current status of pancreatic cancer patients undergoing surgical treatment and shows the road to improve survival of these patients in the future.

    Abstract: Lupus nephritis (LN) is one of the most prevalent and serious complications of SLE, with significant effects on patient and renal survival. Although a large number of genetic variants associated with SLE have been identified, biomarkers that correlate with LN are extremely limited. In this study, we performed a comprehensive sequencing analysis of the whole MHC region in 1331 patients with LN and 1296 healthy controls and validated the independent associations in another 950 patients with LN and 1000 controls. We discovered five independent risk variants for LN within the MHC region, including HLA-DRβ1 amino acid 11 ( P omnibus 〈 0.001), HLA-DQβ1 amino acid 45 ( P 〈 0.001; odds ratio, 0.58; 95% confidence interval, 0.52 to 0.65), HLA-A amino acid 156 ( P omnibus 〈 0.001), HLA-DPβ1 amino acid 76 ( P omnibus 〈 0.001), and a missense variant in PRRC2A (rs114580964; P 〈 0.001; odds ratio, 0.38; 95% confidence interval, 0.30 to 0.49) at genome-wide significance. These data implicate aberrant peptide presentation by MHC classes 1 and 2 molecules and sex hormone modulation in the development of LN.

    Abstract: The enteric-coated mycophenolate sodium (EC-MPS), whose active constituent is mycophenolic acid (MPA), has been widely clinically used for organ transplant recipients. However, its absorption is delayed due to its special designed dosage form, which results in difficulty to monitor the exposure of the MPA in patients receiving the EC-MPS. This study was aimed at developing a relatively practical and precise model with limited sampling strategy to estimate the 12-hour area under the concentration–time curve (AUC 0–12 h ) of MPA for Chinese renal transplant recipients receiving EC-MPS. Methods: A total of 36 Chinese renal transplant recipients receiving the EC-MPS and tacrolimus were recruited in this study. The time point was 2 weeks after the transplantation for all the patients. The MPA concentrations were measured with enzyme-multiplied immunoassay technique for 11 blood specimens collected predose and at 0.5, 1, 1.5, 2, 2.5, 3, 4, 6, 8, and 12 hours after the morning dose of EC-MPS. The measured AUC was calculated with these 11 points of MPA concentrations with the linear trapezoidal rule. Limited sampling strategy was used to develop models for estimated AUC in the model group (n = 18). The bias and precision of different models were evaluated in the validation group (n = 18). Results: C 4 showed the strongest correlation with the measured AUC. The best 3 time point equation was 6.629 + 8.029 × C 0 + 0.592 × C 3 + 1.786 × C 4 ( R 2 = 0.910; P 〈 0.001), whereas the best 4 time point equation was 3.132 + 5.337 × C 0 + 0.735 × C 3 + 1.783 × C 4 + 3.065 × C 8 ( R 2 = 0.959; P 〈 0.001). When evaluated in the validation group, the 4 time point model had a much better performance than the 3 time point model: for the 4 time point model: R 2 = 0.873, bias = 0.505 [95% confidence interval (CI), −10.159 to 11.170], precision = 13.370 (95% CI, 5.186–21.555), and 77.8% of estimated AUCs was within 85%–115% of the measured AUCs; for the 3 time point model: R 2 = 0.573, bias = 6.196 (95% CI, −10.627 to 23.018), precision = 21.286 (95% CI, 8.079–34.492), and 50.0% of estimated AUCs was within 85%–115% of the measured AUCs. Conclusions: It demanded at least 4 time points to develop a relatively reliable model to estimate the exposure of MPA in renal transplant recipients receiving the EC-MPS. The long time span needed restricted its application, especially for the outpatients, but it could be a useful tool to guide the personalized prescription for the inpatients.

    Abstract: Ca 2+ ions passing through a single or a cluster of Ca 2+ -permeable channels create microscopic, short-lived Ca 2+ gradients that constitute the building blocks of cellular Ca 2+ signaling. Over the last decade, imaging microdomain Ca 2+ in muscle cells has unveiled the exquisite spatial and temporal architecture of intracellular Ca 2+ dynamics and has reshaped our understanding of Ca 2+ signaling mechanisms. Major advances include the visualization of “Ca 2+ sparks” as the elementary events of Ca 2+ release from the sarcoplasmic reticulum (SR), “Ca 2+ sparklets” produced by openings of single Ca 2+ -permeable channels, miniature Ca 2+ transients in single mitochondria (“marks”), and SR luminal Ca 2+ depletion transients (“scraps”). As a model system, a cardiac myocyte contains a 3-dimensional grid of 10 4 spark ignition sites, stochastic activation of which summates into global Ca 2+ transients. Tracking intermolecular coupling between single L-type Ca 2+ channels and Ca 2+ sparks has provided direct evidence validating the local control theory of Ca 2+ -induced Ca 2+ release in the heart. In vascular smooth muscle myocytes, Ca 2+ can paradoxically signal both vessel constriction (by global Ca 2+ transients) and relaxation (by subsurface Ca 2+ sparks). These findings shed new light on the origin of Ca 2+ signaling efficiency, specificity, and versatility. In addition, microdomain Ca 2+ imaging offers a novel modality that complements electrophysiological approaches in characterizing Ca 2+ channels in intact cells.

    Abstract: Few studies on electroconvulsive therapy (ECT) investigate efficacy and safety on depressive adolescents with strong suicidal ideation. Our study examined adolescents (aged 13–18 years) with major depressive disorder to explore ECT effectiveness in improving suicidal ideation and depressive symptoms, as well as its impact on cognitive function. Methods This nonrandomized controlled trial enrolled 183 adolescent patients suffering from major depressive disorder. The ECT group (n = 81) was treated with antidepressants and 8 rounds of ECT for 2 weeks. The control group comprised 79 patients treated with antidepressants only. Depressive symptoms, suicidal ideation, and cognitive functions were assessed at baseline (pre-ECT) and at 2 and 6 weeks post-ECT. Results The ECT group showed significant improvements over control in suicidal ideation from the end of treatment to 6 weeks after ( P 〈 0.001). Depressive symptoms also improved ( P 〈 0.001). Patients treated with ECT demonstrated poorer performance in delayed memory, attention, and language, but these impairments were transient. Thus, ECT was generally safe in adolescent patients with major depressive disorder. Conclusions Our findings verified ECT as effective and safe for improving suicidal ideation and depressive symptoms of adolescent patients with major depressive disorder. In addition, partially impaired cognitive function recovered gradually after ECT.

    Abstract: Background and Objective: Inflammation was one of the most important pathophysiological processes during myocardial ischemia. However, it is still poorly understood about the possibilities of these pro- or anti-inflammation factors be used as prognostic parameters for subsequent attenuation of heart functions. Methods and Results: Chinese experimental mini swines (25±4 kg) were instrumented with a size-matched Ameroid constrictor on the left anterior descending branch (LAD) under general anesthesia in sterile conditions (N=15). And a sham group (N=6) was included. 4 weeks after thoracotomy, more than 90% occlusion of the LAD was proved by coronary angiography. Early stage of ischemia was found by echocardiography which showed focal ischemia in the anterior wall of left ventricular (from apex to papillary muscle level) and unchanged ejection fraction. Meanwhile, Enzyme-linked immunosorbent assay of serum showed that IL-8 (86.83 ±10.96 vs. 130.3 ±10.94-pg/ml, P =0.0322) and IL-10 (0.9619 ±0.1421 vs. 1.660 ±0.1607-pg/ml, P =0.0111) were dramatically decreased compared to sham group, while no significant changes were found in IL-1 (0.3075 ±0.03558 vs. 0.3045 ±0.057371 -ng/ml, P = 0.9643), IL-6 (307.7 ±27.81 vs. 315.8 ±23.07-pg/ml, P =0.8637), high sensitivity C reactive protein (hsCRP) (12.94 ±1.324 vs. 9.943 ±1.442-ng/ml, P =0.2087) and TNF-a(1.769 ±0.1467 vs. 1.260 ±0.3209-ng/ml, P =0.1121). Furthermore, 8 weeks after thoracotomy, ejection fraction(EF) of myocardial ischemia animals were dramatically attenuated (44.69 ±4.494 vs. 60.60 ±3.420-%, P =0.0428). Subsequent EF (8 weeks postoperation) showed a good linear regression with the IL-10 and hsCRP concentration in the early stage of myocardial ischemia (4 weeks postoperation) (EF= 0.525+ 0.132* IL-10- 0.015* hsCRP, P =0.001) when stepwise regression was performed with all inflammation related factors above as dependent variables. Conclusion: These results demonstrate that lower IL-10 and higher hsCRP in serum may be predictors for the attenuation of heart function caused by myocardial ischemia. Further Clinical trials may help to prove the efficiency of these novel insights of prognosis for ischemia heart diseases.
