    Kurzfassung: This article analyzes the evolution of inequality in mean male height in central Spain considering the generations born from 1837 to 1915, measured in the drafts from 1858 to 1936 (n = 53,503). Mean adult height reflects a crude indicator of net nutritional status, a proxy for currently known measures of stunting and wasting. The results reveal a cycle of stagnation and decline in average height at the age of 21 for those born from the 1850s to the 1870s and a subsequent positive secular trend to exceed baseline levels. The coefficient of variation shows how inequality in height followed an opposite pattern, with an increase in the mid-nineteenth century and a subsequent decline, with an overall decline. The great migratory wave towards Latin America (1880–1930) barely affected the area studied here. The available evidence on the occupations and educational level of the recruits reveals a ranking in average height related to family background and personal income, educational level and literacy, propinquity to food and ownership and/or management of land. Therefore, socioeconomic status largely predicted adult height in Spanish men during the period. Reducing absolute poverty and increasing access to education remain cornerstones to reducing malnutrition, even in the current world.

    Kurzfassung: Objective: The aim of this paper is to describe the protocol of pilot cluster randomized controlled trial (RCT) that will evaluate the effects of a lifestyle-based intervention. The Archena Infancia Saludable project will have several objectives. The primary objective of this project is to determine the 6-month effects of a lifestyle-based intervention on adherence to 24-h movement behaviors and Mediterranean diet (MedDiet) in schoolchildren. The secondary objective of this project is to test the intervention effects of this lifestyle-based intervention on a relevant set of health-related outcomes (i.e., anthropometric measurements, blood pressure, perceived physical fitness, sleep habits, and academic performance). The tertiary objective is to investigate this intervention’s “halo” effect on parents’/guardians’ 24-h movement behaviors and adherence to the MedDiet. Methods: The Archena Infancia Saludable trial will be a cluster RCT submitted to the Clinical Trials Registry. The protocol will be developed according to SPIRIT guidelines for RCTs and CONSORT statement extension for cluster RCTs. A total of 153 eligible parents/guardians with schoolchildren aged 6–13 years will be randomized into an intervention group or a control group. This project focuses on two fundamental pillars: 24-h movement behaviors and MedDiet. It will mainly focus on the relationship between parents/guardians and their children. Behavior change strategies for dietary and 24-h movement behaviors in schoolchildren will be based on healthy lifestyle education for parents/guardians through infographics, video recipes, brief video clips, and videos. Conclusions: Most of the current knowledge on 24-h movement behaviors and adherence to the MedDiet is based on cross-sectional or longitudinal cohort studies, warranting a need to design and conduct RCTs to obtain more robust evidence on the effect of a healthy lifestyle program to increase 24-h movement behaviors and to improve adherence to the MedDiet in schoolchildren.

    Kurzfassung: Background: Type 2 diabetes mellitus (T2DM) is a chronic disorder, with patients exhibiting hyperglycemia in fasting and postprandial states. T2DM has several complications, including loss of sensation in more distal body parts. Good peripheral sensitivity is essential as this affects different parameters related to activities of daily living, such as leg strength and balance. The objectives of this project were to assess the effects of an 8-week whole-body vibration (WBV) training program on (1) vibration perception threshold (VPT), (2) balance, (3) strength, (4) lipidic profile, (5) health-related quality of life, (6) diabetic neuropathy, and (7) body composition in T2DM patients. Methods/Design: A double-blind, randomized controlled study, with WBV and placebo groups, was carried out. Both groups performed 8 weeks of intervention, with 3 sessions per week, completing a total of 24 sessions. There were two groups: the experimental group, i.e., the WBV group, who received WBV therapy; and the placebo group, who completed a simulated training program that was developed on a Galileo Fitness platform, connected to software displayed on a screen. The participant could see the parameters of the simulated vibration training (duration, amplitude, and frequency), but it was the software that controlled the speakers placed inside the vibration platform. Ninety patients with T2DM (56 males and 34 females) were recruited for the intervention. Participants were assigned equally to the WBV (n = 45) and placebo (n = 45) groups. Primary outcome measures were (1) HbA1c and (2) vibration threshold. Secondary measures were (1) health-related quality of life, (2) balance, (3) strength, (4) body composition, (5) blood pressure, (6) diabetic neuropathy, and (7) lipidic profile. Statistical analysis was carried out by treatment intention and protocol. Discussion: This project aimed to investigate the effects of WBV training on HbA1c, vibration threshold, and incremental cost-effectiveness ratio in T2DM patients. In future, guidelines will be provided for the incorporation of the main obtained conclusions into the social-sanitary system and businesses.

    Kurzfassung: The separation dynamics of a multiphase fluid mixture is studied with the aim to validate the gas reduction from the mixture by means of a KMS helical static mixer device through a laboratory prototype based on the jet-pump artificial lift system for oil extraction. The study is focused on the cyclone phenomenon, produced by the KMS, to experimentally verify the reduction of foaming in a reservoir behaving as a horizontal gravity separator of an oil–gas–water mixture under specific dynamics conditions of velocity and pressure of an incoming streamline, as well as some physical mounting configurations of the device. The results were numerical and experimentally validated, projecting the real performance in three-phase separators in an oil extraction field to improve efficiency in pumping machines regarding the negative effects of the gas. Concerning the gas reduction, a KMS of two elements located at 100 mm from the entrance of the horizontal reservoir and 20 mm from a gas casing tube successfully improves the gas isolation from the mixture, from 29.2% to practically 0% over 200 min in the reservoir, to let oil and water become separated by sedimentation. This gas reduction is indirectly assessed by the emulsion that is generated, which affects the time of oil separation in the counterbalance.

    Kurzfassung: The lack of highly effective treatments for fibromyalgia (FM) represents a great challenge for public health. The objective of this parallel, pilot randomized controlled trial (RCT) was two-fold: (1) to analyze the clinical effects of mindfulness plus amygdala and insula retraining (MAIR) compared to a structurally equivalent active control group of relaxation therapy (RT) in the treatment of FM; and (2) to evaluate its impact on immune-inflammatory markers and brain-derived neurotrophic factor (BDNF) in serum. A total of 41 FM patients were randomized into two study arms: MAIR (intervention group) and RT (active control group), both as add-ons of treatment as usual. MAIR demonstrated significantly greater reductions in functional impairment, anxiety, and depression, as well as higher improvements in mindfulness, and self-compassion at post-treatment and follow-up, with moderate to large effect sizes. Significant decreases in pain catastrophizing and psychological inflexibility and improvements in clinical severity and health-related quality of life were found at follow-up, but not at post-treatment, showing large effect sizes. The number needed to treat was three based on the criteria of ≥50% Fibromyalgia Impact Questionnaire (FIQ) reduction post-treatment. Compared to RT, the MAIR showed significant decreases in BDNF. No effect of MAIR was observed in immune-inflammatory biomarkers (i.e., TNF-α, IL-6, IL-10, and hs-CRP). In conclusion, these results suggest that MAIR, as an adjuvant of treatment-as-usual (TAU), appears to be effective for the management of FM symptoms and for reducing BDNF levels in serum.
