    Description: Background: Hospital-based Emergency Departments are struggling to provide timely care to a steadily increasingnumber of unscheduled ED visits. Dwindling compensation and rising ED closures dictate thatmeeting this challenge demands greater operational efficiency. Methods: Using techniques from operations research theory, as well as a novel event-driven algorithm for processingpriority queues, we developed a flexible simulation platform for hospital-based EDs. Wetuned the parameters of the system to mimic U.S. nationally average and average academic hospitalbasedED performance metrics and are able to assess a variety of patient flow outcomes includingpatient door-to-event times, propensity to leave without being seen, ED occupancy level, and dynamicstaffing and resource use. Results: The causes of ED crowding are variable and require site-specific solutions. For example, in a nationally average ED environment, provider availability is a surprising, but persistent bottleneck in patient flow. As a result, resources expended in reducing boarding times may not have the expected impact on patient throughput. On the other hand, reallocating resources into alternate care pathways can dramatically expedite care for lower acuity patients without delaying care for higher acuity patients. In an average academic ED environment, bed availability is the primary bottleneck in patient flow. Consequently, adjustments to provider scheduling have a limited effect on the timeliness of care delivery, while shorter boarding times significantly reduce crowding.     Description: Purpose To obtain robust estimates of 31 P metabolite content in mouse skeletal muscles using our recently developed MR absolute quantification method and a custom-built 1 H/ 31 P dual tuned radiofrequency (RF) coil optimized for mouse leg. Materials and Methods We designed and fabricated a probe consisting of two dual tuned 1 H/ 31 P solenoid coils: one leg was inserted to each solenoid. The mouse leg volume coil was incorporated with injector coils for MR absolute quantification. The absolute quantification method uses a synthetic reference signal injection approach and solves several challenges in MR absolute quantification including changes of coil loading and receiver gains. Results The 1 H/ 31 P dual tuned probe was composed of two separate solenoid coils, one for each leg, to increase coil filling factors and signal-to-noise ratio. Each solenoid was equipped with a second coil to allow injection of reference signals. 31 P metabolite concentrations determined for normal mice were well within the expected range reported in the literature.     Description: ABSTRACT Although triphenyltin (TPT) compounds are ubiquitous pollutants in urbanised coastal environments in Asian regions, their toxicities to marine organisms are still poorly known. This study was designed to investigate the toxicity of triphenyltin chloride (TPTCl) on the rotifer Brachionus koreanus across different levels of biological organisation. Firstly, we concurrently performed a 24 h static-acute toxicity test and a 6-day semi-static multigenerational life-cycle test using the rotifer. Our results demonstrated that the 24-h median lethal concentration of TPTCl for the rotifer was 29.6 μg/L and the 6-day median effect concentration, based on the population growth inhibition, was 3.31 μg/L. Secondly, we examined the expression of 12 heat shock protein ( hsp ) genes, four glutathione S-transferase ( GST ) genes, one retinoid X receptor ( RXR ) gene and 13 cytochrome P450 ( CYP ) genes in the rotifers after exposure to 20 µg/L TPTCl for 24 h. Among these studied genes, hsp90α2 , GST-O and CYP3045C1 were the most significantly up-regulated genes with a relative expression level up to 32.9, 4.4 and 62.6 folds, respectively. The expression of these three genes in the rotifers showed an increasing trend in the first few hours of TPTCl exposure, peaked at 3 h ( hsp90α2 and GST-O ) and 12 h ( CYP3045C1 ) respectively, and then gradually returned to a lower level at 24 h. Such up-regulations of hsp and GST genes probably offer cellular protection against the TPT-mediated oxidative stress while the accelerated induction of CYP genes possibly facilitates the detoxification of this toxicant in the rotifer. © 2014 Wiley Periodicals, Inc.     Description: Background: Childhood obesity is a major public health issue in many countries, including China. The importance of parenting relative to the healthy development of children requires the development of instruments for assessing parental influence on child dietary pattern. This study aimed to confirm the internal reliability and validity of a self-report measure on parental feeding styles, including emotional feeding, instrumental feeding, prompting or encouragement to eat, and control over eating. Methods: A 27-item parental feeding style questionnaire (PFSQ) was translated into Chinese and then translated back into English to verify consistency. The questionnaire was then used to conduct a cross-sectional survey on the parents of Hong Kong preschoolers. The internal reliability and validity of the questionnaire were examined by Cronbach's alpha and exploratory factor analysis, respectively. Results: 4,553 completed questionnaires were received. Cronbach's alpha of subscales ranged from 0.63 to 0.81, and the overall reliability was good (alpha =0.75). The factor structure of this questionnaire was similar to that of the original and Turkish versions. One-factor structure was identified for emotional feeding, instrumental feeding (four items), and prompting or encouragement to eat, whereas a two-factor structure was revealed for control over eating. Conclusion: The Chinese version of the PFSQ has good reliability and validity in assessing parental feeding styles in Hong Kong. Researchers can use this instrument to improve their understanding on how parental feeding styles may affect the dietary patterns and ultimately the weight statuses of children among Chinese-speaking populations across different countries.

    Description: Skull bone metastasis (SBM) presents as solitary bone mass, which may spread either outward to the scalp or inward to the dura and the subdural tissues. SBM may occur in 4% -22% of all cancer patients and is thought to be haematogenous in origin at late disease stage 1,2 . However, SBM as initial clinical presentation is rare. Although breast is the most common source for SBM, diagnosis can be difficult due to the lack of specific symptoms, particularly in patients without cancer history.
