    Kurzfassung: Subgroups of autoantibodies directed against voltage-gated potassium channel (K v ) complex components have been associated with immunotherapy-responsive clinical syndromes. The high prevalence and the role of autoantibodies directly binding K v remain, however, controversial. Our objective was to determine K v autoantibody binding requirements and to clarify their contribution to the observed immune response. Methods Binding epitopes were studied in sera (n = 36) and cerebrospinal fluid (CSF) (n = 12) from a patient cohort positive for K v 1.2 but negative for 32 common neurological autoantigens and controls (sera n = 18 and CSF n = 5) by phospho and deep mutational scans. Autoantibody specificity and contribution to the observed immune response were resolved on recombinant cells, cerebellum slices, and nerve fibers. Results 83% of the patients (30/36) within the studied cohort shared one out of the two major binding epitopes with K v 1.2-3 reactivity. Eleven percent (4/36) of the serum samples showed no binding. Fingerprinting resolved close to identical sequence requirements for both shared epitopes. K v autoantibody response is directed against juxtaparanodal regions in peripheral nerves and the axon initial segment in central nervous system neurons and exclusively mediated by the shared epitopes. Discussion Systematic mapping revealed two shared autoimmune responses, with one dominant K v 1.2-3 autoantibody epitope being unexpectedly prevalent. The conservation of the molecular binding requirements among these patients indicates a uniform autoantibody repertoire with monospecific reactivity. The enhanced sensitivity of the epitope-based (10/12) compared with that of the cell-based detection (7/12) highlights its use for detection. The determined immunodominant epitope is also the primary immune response visible in tissue, suggesting a diagnostic significance and a specific value for routine screening.

    Kurzfassung: The interaction between intense 30 fs laser pulses and foam-coated 1.5 μ m-thick Al foils in the relativistic regime (up to 5 × 10 20  W cm −2 ) is studied to optimize the laser energy conversion into laser-accelerated protons. A significant enhancement is observed for foam targets in terms of proton cut-off energy (18.5 MeV) and number of protons above 4.7 MeV (4 × 10 9 protons/shot) with respect to uncoated foils (9.5 MeV, 1 × 10 9 protons/shot), together with a sixfold increase in the bremsstrahlung yield. This enhancement is attributed to increased laser absorption and electron generation in the foam meso- and nanostructure.

    Kurzfassung: Objective —To evaluate interobserver agreement and diagnostic accuracy of brain MRI in dogs. Design —Evaluation study. Animals —44 dogs. Procedures —5 board-certified veterinary radiologists with variable MRI experience interpreted transverse T2-weighted (T2w), T2w fluid-attenuated inversion recovery (FLAIR), and T1-weighted-FLAIR; transverse, sagittal, and dorsal T2w; and T1-weighted-FLAIR postcontrast brain sequences (1.5 T). Several imaging parameters were scored, including the following: lesion (present or absent), lesion characteristics (axial localization, mass effect, edema, hemorrhage, and cavitation), contrast enhancement characteristics, and most likely diagnosis (normal, neoplastic, inflammatory, vascular, metabolic or toxic, or other). Magnetic resonance imaging diagnoses were determined initially without patient information and then repeated, providing history and signalment. For all cases and readers, MRI diagnoses were compared with final diagnoses established with results from histologic examination (when available) or with other pertinent clinical data (CSF analysis, clinical response to treatment, or MRI follow-up). Magnetic resonance scores were compared between examiners with κ statistics. Results —Reading agreement was substantial to almost perfect (0.64 〈 κ 〈 0.86) when identifying a brain lesion on MRI; fair to moderate (0.14 〈 κ 〈 0.60) when interpreting hemorrhage, edema, and pattern of contrast enhancement; fair to substantial (0.22 〈 κ 〈 0.74) for dural tail sign and categorization of margins of enhancement; and moderate to substantial (0.40 〈 κ 〈 0.78) for axial localization, presence of mass effect, cavitation, intensity, and distribution of enhancement. Interobserver agreement was moderate to substantial for categories of diagnosis (0.56 〈 κ 〈 0.69), and agreement with the final diagnosis was substantial regardless of whether patient information was (0.65 〈 κ 〈 0.76) or was not (0.65 〈 κ 〈 0.68) provided. Conclusions and Clinical Relevance —The present study found that whereas some MRI features such as edema and hemorrhage were interpreted less consistently, radiologists were reasonably constant and accurate when providing diagnoses.

    Kurzfassung: A jornada de trabalho extenuante e os problemas sociais experimentados pelos docentes em instituições educacionais podem torná-los vulneráveis à síndrome de Burnout. Este estudo objetivou avaliar a síndrome de Burnout em docentes universitários de instituições privadas. Foi aplicado a 141 professores um questionário referente a variáveis sociodemográficas e atividade docente, e a Escala de Caracterização do Burnout. Os resultados foram descritos aplicando-se o teste Qui-quadrado para se verificar a associação entre o grau das dimensões “Exaustão Emocional”, “Desumanização” e “Decepção”. Para testar o efeito das dimensões sobre as variáveis empregou-se o teste de Kruskal-Wallis, seguido do teste post-hoc de Dunn. A maioria dos docentes foi do sexo feminino, com idade abaixo de 40 anos e horistas. O tempo de atividade docente influenciou no aparecimento das dimensões exaustão, desumanização e decepção. Conclui-se que os índices apresentados pelos docentes nas dimensões sinalizam um processo de Burnout em curso.Palavras-chave: Burnout. Trabalho docente. Stress ocupacional. Qualidade de vida no trabalho. BURNOUT SYNDROME IN UNIVERSITY TEACHERS OF PRIVATE INSTITUTIONS ABSTRACT: The fatiguing workday and the social problems experienced by teachers in Brazilian educational institutions can make them vulnerable to Burnout syndrome. This study aimed to evaluate the Burnout Syndrome in university teachers of private institutions in the northwest and west-central state of Paraná. It was applied to 141 teachers a questionnaire regarding sociodemographic variables and teaching activities. Also used the Burnout Characterization Scale. Data were described using frequency tables, using the chi-square test to verify the association between the degree of the dimensions "Emotional Exhaustion", "Dehumanization" and "Deception". To test the effect of the dimensions of the variables we used the Kruskal-Wallis test, followed by post-hoc Dunn test and a confidence level of 95% (α = 0.05). The majority of teachers were female, aged below 40 years and hourly. The teaching activity of time influence the onset of dimensions Exhaustion, Dehumanization and Deception. It can be concluded that teachers have indexes as the dimensions, signaling an ongoing Burnout process.Keywords: Burnout. Health promotion. Teaching work. Occupational stress. Quality of work life.
