    Notes: The design and development of equipment for remote transducer output signal delivery is described which is based on a fiber-optic transmission-reception system employing subcarrier angular modulation for true direct current coupling, high linearity, and low noise behavior. Also, a fiber-based digital uplink for controlling the remote transmitter is presented which provides gain/attenuation programming, calibration signal insertion, battery test, and turn on/stand-by, both manual and automatically using and embedded computer controller. Performance measurements are given.

    Notes: The steady-state compressive creep of Al2O3 with 10 vol% SiC whiskers having grain sizes of 1.2, 2.3, and 4.0 μm has been measured at 1400°C in argon. The creep rate is related to the free volume within the whisker network, not the nominal grain size. The results are consistent with diffusional-controlled creep with different contributions from grain-boundary sliding. Under low stresses, only Liftshitz sliding is possible and the diffusional process controls deformation, while at stresses over a threshold, Rachinger sliding is the mechanism controlling deformation. The evolution between Liftshitz and Rachinger sliding is marked by a significant increase in the value of the stress exponent.

    Notes: Electrical cardioversion is the most effective and safe method to restore sinus rhythm in patients with persistent AF. However, at least 25% of electrical cardioversions are unsuccessful. The aim of the present study was to evaluate, in a prospective, randomized and double-blind study, the efficacy of a pretreatment with intravenous flecainide in patients who underwent electrical cardioversion. Fifty-four consecutive patients with persistent AF, mean arrhythmia duration 8 (mean 3–18) weeks, were randomized in two groups. In the first group (n = 26), patients received flecainide (2 mg/kg as a 30-minute IV infusion) before electrical cardioversion. In the second group (n = 28), 100 mL IV infusion of 5% glucose was administered 30 minutes before electrical cardioversion. The study evaluated the (1) acute efficacy of electrical cardioversion, (2) mean and maximal energy required, (3) mean number of shocks needed, and (4) incidence of complications. The two groups were similar in terms of age, sex, mean AF duration, left ventricular systolic function, atrial dimension, and cardiovascular risk factors. Seventy-seven percent of patients recovered sinus rhythm with electrical cardioversion. No statistical difference was noted between the two groups: flecainide 19/26 (73%) versus placebo 23/28 (82%). No significant differences were found concerning mean or maximal energy and number of shocks required. No major complications were observed. After a 30-day follow-up, 54% of patients maintained sinus rhythm with no difference between the two groups. Pretreatment with intravenous flecainide before electrical cardioversion is not useful in reducing technical failure of cardioversion, however, flecainide does not diminish the effectiveness of electrical cardioversion.     Notes: HURLÉ, A., et al.: Optimal Location for Temporary Epicardial Pacing Leads Following Open Heart Surgery. Temporary epicardial pacing wires are routinely placed in patients undergoing cardiac surgery. Eighteen suitable patients undergoing elective surgery were prospectively studied. Their sensing and stimulating characteristics were studied at various locations. Subepicardial pacing leads were applied to the lateral wall of the LV, apex of the LV, anterior wall of the RV, diaphragmatic wall of the RV, and diaphragmatic wall of the LV. Impedance, R wave amplitude, slew rate, and stimulation thresholds were measured on postoperative days 1 and 5. Impedance remained unchanged in time with no significant differences between locations. R waves and slew rates were significantly lower in the anterior RV wall. Stimulation thresholds displayed no differences on day 1, but they increased significantly in all locations on day 5. These thresholds were significantly lower in the lateral and diaphragmatic LV walls on day 5, and the rate of voltage increase was also lower in these two locations. Five patients presented phrenic nerve stimulation when stimulating the lateral LV wall. The authors advocate the diaphragmatic wall of the LV as the best location for placing temporary leads. The anterior wall of the RV is not recommended for pacing purposes.

    Notes: Between February 2003 and January 2004 a microwave-modified Maze III procedure was performed as an associated procedure in nine patients in chronic atrial fibrillation undergoing surgery for heart valve disease. Clinical follow-up was carried out in all survivors, and an echocardiographic assessment done in all those in sinus rhythm, during the first week of February 2004. There were six women and three men with a mean age of 60 ± 9.4 years. Their rhythm at the end of surgery was sinus in 2 patients, nodal in 4, and complete AV block in 3. One patient died in hospital and there patients had no other complications related to the procedure. By the time of hospital discharge, four patients were in sinus rhythm and four were in atrial fibrillation. After a mean follow-up of 5.2 ± 3.3 months there were no late deaths, 5 patients were in sinus rhythm, 1 required a permanent pacemaker in DDDR mode for persistent sinus bradycardia, and 2 remained in atrial fibrillation. Echocardiographic assessment, performed at a mean of 4.9 ± 2.5 months after surgery in all patients in sinus rhythm or with a pacemaker, demonstrated biatrial contraction in five patients. The Cox-Maze III procedure can be performed safely and with good results using microwave energy instead of the conventional “cut and sew” technique.

    Notes: The composition of tender leaves from five species of the family Cruciferae (Alliaria petiolata, Cardaria draba, Cakile maritima, Capsella bursa-pastoris and Sisymbrium irio)from different locations of the southeast of Spain (Almeria) was investigated. The proximate composition, mineral elements (Na, K, Ca, Mg, P, Fe, Cu, Zn and Mn), fatty acids, vitamin C, carotenoids and oxalic acid contents were determined. The available carbohydrate contents ranged from 1.4% - 8.2%. Vitamin C contents were high, specially in Alliaria petiolata (261 mg/100 g). Carotenoid content was relatively high in Alliaria petiolata (13.3 mg). Mineral elements were present at higher concentrations than is common for other green leafy vegetables. Fiber amounts were also remarkable (1.96–5.38 g/100 g). The ω3 series of fatty acids were predominant in the three species (Capsella bursa-pastoris 54.02%). Oxalic acid values and ratios of oxalic acid/Ca were moderate, being higher in C. maritima. We believe that thesespecies of crucifers could be used for dietary purposes, due to the amount and diversity of nutrients they contain.
