    Notes: Point-of-care testing of coagulation parameters provides a more rapid assessment of test results compared with laboratory testing. A new coagulation monitor (GEM® PCL, Instrumentation Laboratory, Kirchheim, Germany) was evaluated. Point-of-care data for activated partial thromboplastin time and prothrombin time (expressed as the international normalised ratio) and turn-around-time were compared. Coagulation parameters were compared in the blood of 57 patients with and without heparin therapy. The point-of-care and laboratory test results showed a bias (SD) of − 0.26 (4.55) s for activated partial thromboplastin time and − 0.011 (0.150) s for prothrombin time. The average turn-around-time was 3 min for point-of-care testing vs. 52 min for laboratory testing. We conclude that the reliability of point-of-care testing is sufficient for clinical use.

    Notes: Abstract– Studies were performed with both a mechanical and a photosensory arrangement of the experimental tanks. Silver (migratory) eels were more active than yellow (stationary) eels. Comparison of pooled data on yellow eels tested under natural geomagnetic field conditions with those on eel tested under unnatural ones prcded no evidence of difference in activity. Silver and yellow eels showed stronger activity during the 6 days around new moon compared with the phase around full moon as well as a peak before new moon and a smaller peak before full moon. Under each kind of unnatural field condition, the activity relationships reversed or were cancelled. The directional choice of eels under natural conditions (controls) pointed to magnetic NW and mostly also in the opposite direction. When the magnetic north was changed by 90°, the tested yellow eels followed this change to about the same degree. The additional change of both the inclination (from 68° or 45° or 30°) and total intensity (by a reduction to 31% of the natural field) resulted in a change of the preferences of about 90° relative to magnetic north. Reversal of magnetic north by 180°, which is identical to a change of the inclination from 68° to 112°, also resulted in a change of the preferences by about 90°. Three experiments using compensation of both the horizontal and the vertical geomagnetic field led to circular directional preferences at random in two cases and in one case to about NW of the residual field magnetic north. The navigational abilities of the eel, on the basis of its magnetic sensitivity, are discussed.

    Notes: Summary In a randomized, double-blind study oral doses of 50 mg carvedilol (Dilatrend) were compared with 40 mg propranolol in 16 male patients with coronary heart disease, CHD [12 without significant stenoses following percutaneous transluminal coronary angioplasty (PTCA), 4 with multivessel disease]. Bicycle ergometry in the supine position was performed before and 80 min after drug application; measurements were done at rest, during and after exercise. Clinically, the total exercise time and the onset of angina and exhaustion were noted, while the investigated hemodynamic parameters were heart rate, systemic and pulmonary pressures and resistances, cardiac index, and lower limb blood flow. Clinically, carvedilol improved the exercise tolerance more than propranolol as regards angina and exhaustion. Hemodynamically, carvedilol did not lead, as the classic betablocker propranolol does, to an increase in systemic or pulmonary resistance, to a decrease in cardiac output, or to an increase of the pulmonary capillary wedge pressure during exercise, but instead caused opposite changes. In contrast to propranolol, the post exercise lower limb blood flow had increased significantly. The differences in action between the two betablockers can be explained by the vasodilating properties of carvedilol. Due to these acute effects, carvedilol may be preferred to propranolol in the treatment of CHD patients with hypertension, peripheral occlusive artery disease, and/or coronary vasospasm.

    Notes: Mit steigenden Lebensalter nimmt die Inzidenz von malignen Tumoren zu. Neben der Chirurgie stellt die Radio- therapie eine funktions- und organ- erhaltende Therapiemaßnahme mit kurativem Potential bei lokalisierten Tumorerkrankungen dar. Sowohl die Ansprechraten als auch die längerfristigen Tumorkontrollraten unterscheiden sich bei den meisten malignen Tumoren im Alter nicht von denjenigen bei jüngeren Patienten. Voraussetzung ist die Anwendung moderner anspruchsvoller Technologie bei Therapieplanung und Durchführung sowie die Bestrahlung mit konventioneller Fraktionierung und unveränderter Gesamtdosis. Die Hypofraktionierung (Bestrahlung mit wenigen aber hohen Einzeldosen) darf in der Kurativtherapie nicht zu Funktionseinschränkungen an gesunden Organen führen. In der Palliativtherapie erleichtert sie die ambulante Durchführung. Akute Strahlenfolgen können bei älteren Patienten ausgeprägter sein als bei jüngeren und erfordern die engmaschige Überwachung und u. U. auch den frühen Einsatz supportiver Maßnahmen. Mit erhöhten Spätfolgen ist nur nach Bestrahlung eines durch vorbestehende Komorbidität eingeschränkten Organes zu rechenen. Im Gegensatz zu jüngeren Patienten müssen im Alter kritische Normalgewebe und Organe prätherapeutisch sorgfältig auf ihre Funktionsreserve hin untersucht werden und die Ergebnisse nach Möglichkeit bei der Bestrahlungsplanung berücksichtigt werden.

    Notes: Während die primäre Prävention das Auftreten von Krankheiten durch Ausschaltung schädlicher Faktoren noch vor deren Wirksamwerden verhindern soll, und die sekundäre Prävention durch Aufdeckung von Krankheiten in möglichst frühem Stadium – auch subklinischem, serologischem Stadium – deren rechtzeitige Behandlung zum Ziel hat, soll die tertiäre Prävention bei eingetretener, klinisch manifester Krankheit deren Progression mit möglichen begleitenden Komplikationen verhindern. Das Ziel der Tertiärprävention oder Tertiärphrophylaxe in der Onkologie ist neben der Verhinderung bzw. Verminderung der lokalen/loko-regionären Rezidivfrequenz auch die Verminderung der Fernmetastasenfrequenz und damit die (potentielle) Erhöhung der Rate an langfristigem, tumorfreiem Überleben. Bei einigen Tumorentitäten werden beide Ziele erreicht.

    Notes: Rhabdomyosarkome machen ca. 10–20% aller Weichteilsarkome des Erwachsenen aus. Hauptlokalisationen sind der HNO-Bereich und die Orbita (37%), der Urogenitaltrakt (21%), die Extremitäten (20%) und das Retroperitoneum (8%) [6]. Alveoläre und embryonale Rhabdomyosarkome treten vornehmlich vor dem 25. Lebensjahr auf, während pleomorphe Rhabdomyosar kome einen medianen Altersgipfel nach dem 45. Lebensjahr haben. Die wichtigsten prognostischen Faktoren sind das TNM-Stadium, die Lokalisation des Primärtumors, das Ausmaß der operativen Radikalität und der histopathologische Differenzierungsgrad [8].

    Notes: Summary In 25 patients, who received repeated doses of cis-DDP, we determined iodo-131-hippurate-clearance, creatinine clearance and serum creatininc to assess the nephrotoxic effect induced by cis-DDP. A highly significant decrease in the iodo-131-hippurate-clearance was found between the first and the following cycles. There was no significant change in serum creatinine and creatinine clearance however. The radio-hippurate-clearance showed a clinically not important reduction during several cytotoxic courses in 15/25 patients (60%). A clinically relevant impairment of the radio-hippurate-clearance occurred in 8/25 patients (32%), mainly before the third and fifth cytotoxic cycle. In two patients this clearance did not alter or was even rising. In 12 cases a diminuation of the iodo-131-hippurate-clearance was observed, which in only 5 cases correlated to a decreased creatinine clearance and in only two cases to an elevated serum creatinine. Therefore, we consider neither creatinine nor creatinine clearence nor the combination of both parameters to be appropriate to assess tubular nephropathy induced by cis-DDP. Radio-hippurate-clearance should play a superior role deciding on treatment stop or dose reduction of cis-DDP. The regression of cis-DDP-induced nephropathy was not evaluated.

    Notes: The results of a study to determine the biological stability of clear, colorless, biocide-treated PVC film are presented. With the exception of a resistant control, all the PVC film samples tested contained a plasticizer susceptible to biological deterioration. Biocide-treated and control PVC film samples were subjected to biological attack in soil burial. Portions of each sample were exposed to leaching and to weathering in a weatherometer followed by soil burial to test the stability of each biocide to loss by water and weathering exposure. Of the 32 biocide preparations tested only 2, N-(trichloromethylthio) phthalimide and copper 8-quinolinolate, a known effective reference compound, were found to provide good protection for treated film in direct soil burial and in soil burial following leaching and weatherometer exposure. Natural weathering outdoors confirmed the results found with weatherometer exposed samples. The N-(trichloromethylthio) phthalimide was less effective than the reference compound in suppressing surface growth. In addition, N-(trichloromethylthio) phthalimide may not be compatible with all PVC formulations.
