    Notes: Summary Gadolinium-containing microcapsules were evaluated as an agent for gadolinium neutron-capture therapy. Mice were inoculated intraperitoneally with 107 Ehrlich ascites tumor cells and gadolinium microcapsules and exposed to thermal neutrons for 12 min (approximately 1.86×1012 neutrons cm−2). Significantly more mice given gadolinium microcapsules than those given placebo microcapsules or control survived for 60 days and considerably longer (P〈0.0001), indicating that gadolinium neutron-capture reactions effectively suppressed the growth of ascites tumor cells in mice. The results suggest that these microcapsules are an effective gadolinium carrier for neutron-capture therapy.

    Description / Table of Contents: Hintergrund: Analysen des Zellüberlebens sind durchgeführt worden, um die Effekte ionisierender Strahlung zu evaluieren, die durch Neutroneneinfangreaktionen mit Gadolinium-157, Bor-10 und einer Kombination aus beiden entstehen. Material und Methoden: Einzelzellsuspensionen mit und ohne Gadolinium-157 und/oder Bor-10 wurden einer Bestrahlung mit thermalen Neutronen aus dem Reaktor der Universität Kyoto unterzogen und Zellüberlebenskurven generiert. Ergebnisse: Unter äquimolaren Bedingungen war der Effekt auf das Zellüberleben für Gadolinium-157 1,5mal größer als der von Bor-10, sofern ein Vergleich auf dem 10%-Überlebensniveau durchgeführt wurde. Bor-10 verstärkte den Strahleneffekt von Gadolinium-157-Neutroneneinfang um das 1,2fache. Schlußfolgerung: Gadolinium-157 und Bor-10 waren gleich effektiv, sofern sie einer gleichen Zahl von theramalen Neutronen exponiert wurden. Kein Vorteil wurde durch die Kombination aus Gadolinium-157 und Bor-10 beobachtet.
    Notes: Purpose: Cell survival assays were performed to evaulate the effects of radiations released during neutron capture reactions by radolinium-157, boron-10 and by the combination of both. Materials and Methods: Single cell suspensions with or without Gd-157 and/or B-10 were exposed to thermal neutrons produced by the Kyoto University reactor, and standard cell survival curves were obtained. Results: Under the same molarity, cytocidal effects were 1.5 times greater for Gd-157 than for boron when compared at 10% survival levels. The presence of B-10 enhanced the radiation effect of Gd-157 neutron capture by 1.2-fold, suggesting that cells were not sufficiently irradiated as a result of neutron fluency attenuation by the presence of excess neutron capture agents in the medium. Conclusions: When an equal number of atoms were present, Gd-157 was effective as B-10 when exposed to an equal number of thermal neutrons. However, there was no benefit observed in the combination of Gd-157 and B-10 for neutron capture therapy. Further studies are needed to determine optimal Gd-157 and B-10 concentrations as a function of tumor dimension.

    Notes: Summary We present a 24-year-old woman with multifocal and heterochronous recurrence of malignant astrocytomas that were consecutively treated by linac-based stereotactic radiotherapy. The patient had previously received multimodalities of treatment for malignant astrocytomas in the right occipital lobe consisting of surgical resection on four occasions, conventional external-beam irradiation, immunological therapy, and systemic chemotherapy. Thereafter she developed relatively small and well-demarcated recurrent lesions in eight different sites, most of which were located in eloquent and deep-seated regions. She underwent ten fractionated linac-based stereotactic radiotherapy courses with a median total dose of 42 Gy, seven fractions of 6 Gy each in 15 days, experiencing neither any adverse effects nor neurologic deterioration. Three out of 10 treated lesions responded well with subsequent reduction in size, and 80% of them did not increase at least for 6 months, although 66.7% of the lesions eventually exhibited enlargement of the enhanced areas, some of which were presumed as radiation necrosis. These clinical results suggest that the fractionated linac-based stereotactic radiotherapy had significant effects on tumor growth inhibition and attainment of acceptable patient performance status.

    Description / Table of Contents: Hintergrund: Retrospektive Evaluierung der Wirksamkeit einer Bestrahlung mit oder ohne transarterielle Embolisation oder perkutaner Alkoholinjektion bei Patienten mit inoperablem hepatozellulärem Karzinom. Patienten und Methodik: Von Oktober 1984 bis November 1997 wurden 62 Patienten mit einer Gesamtdosis von 50 bis 70 Gy in 25 bis 35 Fraktionen mit oder ohne transarterielle Embolisation oder perkutane Alkoholinjektion behandelt. Der mediane Follow-up betrug 8,6 Monate (1,5 bis 92 Monate). Ergebnisse: Das mediane Gesamtüberleben betrug 9,5 Monate. Signifikante prognostische Einflussfaktoren waren das Ausmaß der prätherapeutischen Leberfunktionseinschränkung, die Bestrahlungsfeldgröße und das Vorhandensein von Tumorthrombosen. Nach sechs Monaten betrug die lokale Kontrolle 67%, nach einem Jahr 54%. Sieben der acht Patienten, die ein Leberversagen erlitten, hatten eine schlechte prätherapeutische Leberfunktion. Schlussfolgerung: Die Bestrahlung mit oder ohne transarterielle Embolisation und/oder perkutane Alkoholinjektion scheint bei der Behandlung des hepatozellulären Karzinoms wirksam zu sein und verlängert das Überleben. Individualisierte Behandlungsstrategien, abhängig von der Tumorpräsentation und dem Ausmaß der Leberfunktionseinschränkung, werden vorgestellt.
    Notes: Purpose: To retrospectively evaluate the effectiveness of radiotherapy with or without transarterial embolization (TAE) and/or percutaneous ethanol injection (PEI) in patients with hepatocellular carcinoma who were ineligible for surgery. Patients and Methods: From October 1984 to November 1997, 62 patients underwent radiotherapy receiving 50 to 70 Gy in 25 to 35 treatments with or without transarterial embolization and/or percutaneous ethanol injection and were followed for a median period of 8.6 months (1.5 to 92 months). Results: Overall median survival rates were 9.5 months. Significant prognostic factors were the extent of pretreatment liver function impairment, radiation field size and the existence of tumor thrombosis. Six-month and 1-year local control rates were 67 and 54%, respectively. Seven of the 8 patients who suffered from hepatic failure had poor pretreatment liver functions. Conclusion: Radiotherapy with or without transarterial embolization and/or percutaneous ethanol injection appears effective in controlling hepatocellular carcinoma and prolonged survival. Individualized treatment strategies are presented depending on the tumor presentation and the degree of liver function impairment.
