    Description: In this paper, a number of new approximations are introduced to estimate the perturbative diffusivity ( χ ), convectivity ( V ), and damping ( τ ) in cylindrical geometry. For this purpose, the harmonic components of heat waves induced by localized deposition of modulated power are used. The approximations are based on semi-infinite slab approximations of the heat equation. The main result is the approximation of χ under the influence of V and τ based on the phase of two harmonics making the estimate less sensitive to calibration errors. To understand why the slab approximations can estimate χ well in cylindrical geometry, the relationships between heat transport models in slab and cylindrical geometry are studied. In addition, the relationship between amplitude and phase with respect to their derivatives, used to estimate χ , is discussed. The results are presented in terms of the relative error for the different derived approximations for different values of frequency, transport coefficients, and dimensionless radius. The approximations show a significant region in which χ , V , and τ can be estimated well, but also regions in which the error is large. Also, it is shown that some compensation is necessary to estimate V and τ in a cylindrical geometry. On the other hand, errors resulting from the simplified assumptions are also discussed showing that estimating realistic values for V and τ based on infinite domains will be difficult in practice. This paper is the first part (Part I) of a series of three papers. In Part II and Part III, cylindrical approximations based directly on semi-infinite cylindrical domain (outward propagating heat pulses) and inward propagating heat pulses in a cylindrical domain, respectively, will be treated.

    Description: In this paper, a number of new explicit approximations are introduced to estimate the perturbative diffusivity ( χ ), convectivity ( V ), and damping (τ) in cylindrical geometry. For this purpose, the harmonic components of heat waves induced by localized deposition of modulated power are used. The approximations are based on the heat equation in cylindrical geometry using the symmetry (Neumann) boundary condition at the plasma center. This means that the approximations derived here should be used only to estimate transport coefficients between the plasma center and the off-axis perturbative source. If the effect of cylindrical geometry is small, it is also possible to use semi-infinite domain approximations presented in Part I and Part II of this series. A number of new approximations are derived in this part, Part III, based upon continued fractions of the modified Bessel function of the first kind and the confluent hypergeometric function of the first kind. These approximations together with the approximations based on semi-infinite domains are compared for heat waves traveling towards the center. The relative error for the different derived approximations is presented for different values of the frequency, transport coefficients, and dimensionless radius. Moreover, it is shown how combinations of different explicit formulas can be used to estimate the transport coefficients over a large parameter range for cases without convection and damping, cases with damping only, and cases with convection and damping. The relative error between the approximation and its underlying model is below 2% for the case, where only diffusivity and damping are considered. If also convectivity is considered, the diffusivity can be estimated well in a large region, but there is also a large region in which no suitable approximation is found. This paper is the third part (Part III) of a series of three papers. In Part I, the semi-infinite slab approximations have been treated. In Part II, cylindrical approximations are treated for heat waves traveling towards the plasma edge assuming a semi-infinite domain.

    Notes: To investigate the relationships between allergen inhalation challenge and other diagnostic procedures, inhalation challenge with house dust (HD) allergen, intradermal skin tests with HD allergen, inhalation challenge with methacholine and circulating HD allergen-specific IgE levels were examined in 104 patients with bronchial asthma. Using the single exposure method, allergen inhalation challenge was performed. Forty-three patients had positive bronchial responses to allergen and 61 patients had negative bronchial responses. With serially diluted HD allergen (10-3 to 10-6, w/v), skin-test sensitivity was expressed as the highest dilution required to produce a weal of more than 9.9 mm. With the continuous exposure method, bronchial responsiveness to methacholine was evaluated as the number of units of inhaled methacholine (PD35-Grs) from the start to the point at which Grs had decreased by 35% from its baseline value. The level of circulating HD allergen-specific IgE was measured with the Phadebas. RAST system and the results were assessed as a RAST score. Using discriminate analysis, in which the independent variables were skin-test sensitivity. PD35-Grs and the RAST score, only in 30% of all patients was bronchial responsiveness to inhaled HD allergen predictable. Therefore, we suggest that inhalation challenge with allergen is an essential test for determining the role of a specific allergen in airways at present.

A single-blind cross-over method was used to investigate the effects of ketotifen on the total respiratory resistance response curve to inhalation of a high concentration of allergen (1:5 w/v house dust or 1:10 w/v Japanese cedar pollen) and the weal response to intradermal injection of serially diluted allergen (1:103–1:106 w/v). Five subjects received 2 mg of ketotifen every 12 hr for the first week and an inactive placebo in the same manner for the second week. The remaining five subjects received the placebo for the first week and ketotifen for the second week. Ketotifen did not alter base-line pulmonary function, but significantly delayed the time from the start of allergen exposure to the beginning of elevation of total respiratory resistance (P〈0·02), and the time from the start of allergen exposure to the point at which total respiratory conductance decreased by 35% from the base-line value (P〈0·01). Ketotifen also decreased skin test sensitivity significantly (P〈0·01)

    Notes: Background: Granulocyte-macrophage colony stimulating factor (GM-CSF) and inlerleukin (IL)-5 or IL-8 have been .suggested to play an important role in the pathogenesis of eosinophilic airway inflammation in bronchial asthma or neutrophilic airway inflammation in chronic bronchitis, respectively, However, GM-CSF and IL-8 have biological activities to either eosinophils or neutrophils.Objective: To investigate the contribution of these cytokines to airway inflammation, we compared the cellular differential and immunolocalization of GM-CSF, IL-5 and IL-8 in sputum cells from patients with bronchial asthma and chronic bronchitis.Methods: Cytospins of sputum cells from 12 patients with bronchial asthma and 12 with chronic bronchitis were subjected to cellular differential counting and immunocytochemistry with antihuman GM-CSF, IL-5 and IL-8 antibody.Results: The predominant cells in bronchial asthma were eosinophils and lymphocytes, while those in chronic bronchitis were neutrophils. All cytokines examined were detected in either bronchial asthma or chronic bronchitis, although the percentage of GM-CSF and IL-5 positive cells in bronchial asthma (53.4 ± 6.0 [mean±sfm]% and 9.7 ± 2.8%, respectively) was significantly higher than that in chronic bronchitis (11.4±2.5%; P 〈 0.001 and 1.7plusmn;0.3%; P 〈 0.007. respectively). In contrast, the percentage of IL-8 positive cells in chronic bronchitis (23.8 ± 7.0%) was significantly higher than that in bronchial asthma (7.7 ± 1.9%; P 〈 0.04). The cells positive for IL-5 were lymphocytes in bronchial asthma and chronic bronchitis. The cells positive for GM-CSF in bronchial asthma were predominantly eosinophils. while those in chronic bronchitis were monocytes/macrophages and neutrophils. In contrast, neutrophils are mainly positive for IL-8 in chronic bronchitis, while monocytes/macrophages and bronchial epithelial cells are positive for IL-8 in bronchial asthma.Conclusion: The immunochcmical comparison of GM-CSF and IL-8 localization in sputum cells between bronchial asthma chronic bronchitis suggests the differential regulation and roles of these cytokines in eosinophilic vs neutrophilic airway inflammation, resulting in the development of different types of airway inflammation.

    Notes: We report here a case of apocrine adenosis of the breast in a 66-year-old woman. To clarify the nature of this lesion, we examined it by clonal analysis.The lesion, 43 mm at its greatest dimension, was ill-circumscribed, lacking a fibrous capsule, and was composed of compact glands that showed typical histological features of apocrine adenosis. Clonality analysis, using the polymerase chain reaction (PCR)-based method for females heterozygous for a BstXI polymorphism of the X-linked phosphoglycerokinase (PGK) gene, revealed the monoclonal nature of the lesion.This result strongly suggests that some populations of apocrine adenosis are already neoplastic, and this could contribute to the premalignant potential of this lesion.
