    Notes: The Portex introducer for the laryngeal mask airway was designed as an aid to successful insertion, acting as an idealised ‘artificial hard palate’ to guide the tip of the laryngeal mask into the correct position. A number of authors have investigated laryngeal mask insertion by unskilled personnel in certain situations, one example being nurses during in-hospital cardiopulmonary resuscitation. We investigated whether the introducer had any effect on the incidence of first-time successful LMA placement by unskilled personnel. These were nonanaesthetist doctors, randomly assigned to have one attempt at LMA insertion in an anaesthetised patient, with and without the introducer. In 44 patients with the LMA being inserted according to the manufacturer's instructions, there was a 68% success rate (14 failures). In 45 patients with the LMA being inserted with the aid of a Portex introducer, there was a 96% success rate (two failures). This was a highly significant improvement (p 〈 0.001).

    Notes: The effects of 0.15% quasi-steady-state end-tidal isoflurane on two saccadic eye-movement tests were examined in five volunteers using a newly devised computer-based recording system. The tests were saccadic latency and a countermanding task, the latter being an indicator of the highest levels of conscious performance. A moving light-emitting diode target was displayed on a screen and in the saccadic-latency task the latency of eye movement to the target was measured. In all five subjects the latency increased with anaesthetic by an amount which varied from 8 to 45 ms. This result was significantly different (p 〈 0.05) from subjects without anaesthetic. In the countermanding task, the subject had to voluntarily inhibit movement to the target. Again anaesthetic increased the latency of response, which varied from 6 to 33 ms. This result was significantly different (p 〈 0.05) from subjects without anaesthetic. In these studies it appeared that two tasks, one a simple latency test and the other, the countermanding task, requiring higher cortical processing were equally impaired at subanaesthetic concentrations of isoflurane.

    Notes: In a prospective, randomised, double-blind study of 60 patients who had an elective Caesarean section under spinal anaesthesia we compared 0.1 mg intrathecal morphine with intrathecal saline placebo. All patients received morphine intravenously by patient-controlled analgesia after the operation. Pain, satisfaction and side-effects were assessed at 4 and 24 h after the operation. Pain measured by a 100-mm visual analogue scale was less in the intrathecal morphine group at both times (p 〈0.05) and morphine consumption was lower (p 〈0.01). At 4 h the intrathecal morphine group had more pruritus (p 〈0.001) but there was no difference in satisfaction. At 24 h there was no significant difference in side-effects, but overall satisfaction measured by visual analogue scale was better in the intrathecal morphine group (p 〈0.01). Intrathecal morphine improves pain relief and patient satisfaction after Caesarean section.

    Notes: Abstract This case report describes the history of a patient with an aggressive course of a metastatic extra-adrenal pheochromocytoma (paraganglioma) who received different combination chemotherapy regimens with no or short-lasting clinical benefit. However, during treatment with single-agent paclitaxel, there was a significant clinical improvement, a partial biochemical response and a minor roentgenologic response, which was sustained for 1 year. In this report we present this case and also review the literature on the chemotherapy used for this rare disease over the past 15 years. To enable the activity of paclitaxel against this neoplasm to be determined, more patients need to be treated.

    Notes: Abstract In a retrospective study we evaluated 49 consecutive penetrating keratoplasties for herpes simplex keratitis. Mean follow-up was 44.2 months. Survival analysis with Kaplan—Meier curve showed an overall survival rate (clear graft) of 88% at one year, 76% at two years and 72% at four years postoperatively. Survival analysis showed a recurrence-free survival rate of 72% at one year, 59% at two years and 51% at four years postoperatively. Of the 13 non-primary graft failures, 9 happened in eyes with an HSV recurrence. Recurrence of HSV infection occurred in 18 (39%) eyes at an average of 12.6 months after surgery (range 0.3–46). Five (28%) of the recurrences occurred within two months after the start of steroid treatment for rejection. Nine (50%) of the recurrence cases resulted in a clouded graft at the end of follow-up. 73% of the eyes with a clear graft had a VA of 0.25 or better. We conclude from these data that a recurrence of a herpetic infection following corneal transplantation is the main reason for graft failure in this group.

    Description: The epithelial sodium channel (ENaC) plays an important role in controlling Na + homeostasis, extracellular fluid volume, and blood pressure. Copper metabolism Murr1 domain-containing protein 1 (COMMD1) interacts with ENaC and downregulates ENaC. COMMD1 belongs to the COMMD family consisting of COMMD1–10, and all COMMD family members share a C-terminal COMM domain. Here, we report that COMMD2–10 also interacts with ENaC, and COMMD3 and COMMD9 were selected for further study. Amiloride-sensitive current in mammalian epithelia expressing ENaC was significantly reduced by COMMD3 or COMMD9, and ENaC expression at the cell surface was significantly decreased in the presence of COMMD3 or COMMD9. COMMD3 and COMMD9 retained their ability to reduce current when COMMD1 was knocked down. COMMD3 and COMMD9 were widely expressed in kidney and were colocalized with ENaC in renal collecting duct cells. These data suggest that COMMD3 and COMMD9 may be endogenous regulators of ENaC to regulate Na + transport through altering ENaC cell surface expression.

    Description: Coherent sandy shoals that migrate towards the downdrift coast are observed on many ebb-tidal deltas. In this study, processes that cause the growth and migration of shoals on ebb-tidal deltas are identified. Moreover, the effect of the incident wave energy and the tidal prism of an inlet on the migration speed of these shoals is investigated. For this, a numerical morphodynamic model with an idealized geometric set-up is employed. The model computes the bed-level evolution due to local erosion and deposition of sand driven by tides and waves. Analysis of model results shows that shoals grow when there is a local imbalance between the bathymetry and the wave conditions, which in this study was imposed by manually breaching the ebb-tidal delta or by adding storms to the wave forcing. There are thresholds for shoal formation that depend on the distribution of the sand and the incident wave energy. Wave refraction over the shoals leads to focusing of wave energy and increased wave energy dissipation around the location of the local minimum water depth. This generates residual currents over the shoal and increased skin friction towards the local minimum water depth, which together create a sand transport pattern that induces the growth and migration of the shoal. Sand transport due to asymmetric waves contributes to keeping the shoal a coherent structure. The shoal migration speed increases with increasing incident wave energy and decreasing tidal prism this is because tidal residual currents oppose the wave-driven residual currents that cause shoal migration.

    Description: Background The SYNTAX score (SXscore), an anatomical-based scoring tool reflecting the complexity of coronary anatomy, has established itself as an important long-term prognostic factor in patients undergoing percutaneous coronary intervention (PCI). The incorporation of clinical factors may further augment the utility of the SXscore to longer-term risk stratify the individual patient for clinical outcomes. Methods and results Patient-level merged data from 〉6000 patients in seven contemporary coronary stent trials was used to develop a logistic regression model—the Logistic Clinical SXscore—to predict 1-year risk for all-cause death and major adverse cardiac events (MACE). A core model (composed of the SXscore, age, creatinine clearance, and left ventricular ejection fraction) and an extended model [incorporating the core model and six additional (best performing) clinical variables] were developed and validated in a cross-validation procedure. The core model demonstrated a substantial improvement in predictive ability for 1-year all-cause death compared with the SXscore in isolation [area under the receiver operator curve (AUC): core model: 0.753, SXscore: 0.660]. A minor incremental benefit of the extended model was shown (AUC: 0.791). Consequently the core model alone was retained in the final the Logistic Clinical SXscore model. Validation plots confirmed the model predictions to be well calibrated. For 1-year MACE, the addition of clinical variables did not improve the predictive ability of the SXscore, secondary to the SXscore being the predominant determinant of all-cause revascularization. Conclusion The Logistic Clinical SXscore substantially enhances the prediction of 1-year mortality after PCI compared with the SXscore, and allows for an accurate personalized assessment of patient risk.

    Description: The net transport of sediment between the back-barrier basin and the sea is an important process for determining the stability of tidal inlet systems. Earlier studies showed that in a short basin, tidal flats favor peak ebb-currents stronger than peak flood currents, implying export of coarse sediment, while shallow basins favor stronger flood currents. The new elements considered in this study are 1) arbitrary basin lengths, 2) a narrow inlet that connects the basin to the sea, 3) an asymmetric tidal forcing, and 4) radiation damping. The objective is to gain fundamental insight in how the geometry of a tidal inlet system affects the net sand transport in a tidal inlet. For this purpose, a width- and depth averaged analytical model was constructed. It is found that the length of a back-barrier basin controls the effect that nonlinear hydrodynamic processes have on the tidal asymmetry, and consequently controls whether the currents in the inlet are flood- or ebb-dominant. Furthermore, the cross-sectional area of the inlet controls the ratio between the net sediment transport that results from tidal asymmetry and that caused by the interaction of the principal tide with the residual current. Finally, it is shown that the effect of an asymmetric tidal forcing on the net sand transport depends on the length of the back-barrier basin with respect to the tidal wavelength in that basin.
