    Notes: Abstract Carteolol, a nonselective beta-adrenergic antagonist with intrinsic sympathomimetic activity, was compared in 1% and 2% topical solutions with 0.5% timolol in 105 patients with primary open-angle glaucoma. In this double-masked, randomized 3-month trial, all three preparations significantly lowered intraocular pressure throughout the study, with no significant differences being observed. There were also no significant differences among the three preparations with regard to ocular or systemic adverse reactions, including heart rate and blood pressure. Offprint requests to: M.B.     Notes: Abstract In 359 glaucoma patients (600 eyes), we measured the peripheral field with kinetic targets and the central 30° field of vision with static testing, using the Field-master-5000 automated perimeter to evaluate the prevalence and significance of step-like visual-field defects along the vertical midline (a hemianopic offset). In all, 129 eyes of 121 patients had a hemianopic offset. In 108 eyes the offsets (involving either the peripheral or the central field or both) were associated with other glaucomatous field changes, whereas 21 eyes showed a peripheral offset as an isolated finding. Of the 8 cases of offsets found in both eyes, 7 were binasal and 1 occurred bitemporally but in separate hemifields. This study suggests that hemianopic offsets occur commonly in glaucoma patients but have limited diagnostic value because most are associated with other glaucomatous field changes. However, these findings help in distinguishing glaucomatous offsets from those caused by neurological lesions.

    Notes: Abstract In 20 patients with open-angle glaucoma in aphakia or pseudophakia whose intraocular pressure had remained uncontrolled on their current medical therapy, the medication was changed from pilocarpine or carbachol to echothiophate iodide. In all, 12 patients (60%) showed a statistically significant improvement in pressure control, 7 (35%) showed no change, and 1 had higher pressure. One-third of the patients with improved intraocular-pressure control eventually required laser or incisional surgery after a mean of 23 months, whereas the remaining subjects were controlled for the duration of the follow-up, which averaged 26 months. Side effects encountered during echothiophate iodide treatment included ocular irritation, decreased vision, and one retinal detachment.

    Notes: Abstract Nineteen glaucoma patients, 17 ocular hypertensives, and 16 normal subjects underwent visual field testing on the Humphrey Field Analyzer using two programs: full threshold (thresholding of all test points with double-crossing technique) and quantification of defects (thresholding only points that deviated more than 6 dB from a presumed normal retinal contour). The purpose of the study was to compare the diagnostic value of information gained by the latter, less time-consuming test with that of the full threshold procedure. The average time requirement per eye was 13 min 11 s for full thresholding and 4 min 22 s for quantification of defects. Of the 104 sets of fields, the diagnosis was at variance in 18. The differences were most often due to shallow defects in otherwise normal fields or shallow defects surrounding deeper scotomas that had been detected by both programs. The shallow defects placed the fields in more advanced diagnostic categories and were all detected with the full threshold technique.

    Notes: Abstract One eye each of ten healthy volunteers was tested with the Rodenstock Optic Nerve Head Analyzer on ten separate occasions. A color-coded pallor map was developed for each examination. A color transparency of each pallor map was then obtained from the television monitor of the Optic Nerve Head Analyzer, using a camera mounted on a tripod at a fixed distance from the screen. A color-coded zone within the center of the disc was chosen for each set of ten studies, and the area of this zone was measured by boundary analysis with another computerized image analyzer, the Oasys Image Processor. There was wide intrasubject variation in all ten volunteers, suggesting that the pallor map generated by the Optic Nerve Head Analyzer, when used in the manner described in this paper, is not adequate for following subtle changes in optic nerve head pallor.
