    Notes: Abstract X-linked recessive bulbospinal neuronopathy is a motoneuron disorder to be distinguished from amyotrophic lateral sclerosis. Effective treatment is not known. Patients with X-linked recessive bulbospinal neuronopathy may show gynecomastia and testicular atrophy, and a mutation in the androgen receptor gene has been found associated with the disease. Intermediate steps leading from the androgen receptor abnormality to the clinical syndrome have not yet been elucidated. Therefore, binding of androgen ([3H]dihydrotestosterone) to its specific receptor by genital skin fibroblasts cultured from a patient with X-linked recessive bulbospinal neuronopathy and confirmed androgen receptor mutation was studied. Markedly decreased binding capacity was found. We treated the patient for 6 months with nandrolone-decanoate. No effect on his neuromuscular status was observed during 2 years of follow-up.

    Notes: Summary Normal sexual development is the consequence of three sequential interrelated processes: establishment of genetic, gonadal and somatic sex. It is the terminal phase of sexual differentiation-the translation of gonadal into somatic sex, which is governed by the presence or absence of both testosterone and Müllerian-inhibiting hormone and of dihydrotestosterone, which is formed in its respective target tissues. Thus, despite a testis, somatic male sexual differentiation will proceed to a normal male phenotype only if all three hormones are synthesized and act during a critical period of uterine development. Many clinically distinct syndromes are the results of abnormalities in the synthesis or action of the above-mentioned hormones; these syndromes are described in detail. In contrast to male somatic differentiation, female somatic development is independent of these hormones.

    Notes: Summary The ability of various natural and synthetic steroids (some of which are widely used in clinical practice) to compete with dihydrotestosterone receptor binding in human genital skin fibroblasts was studied. Binding was assessed in fibroblast monolayers after incubation for 1 h at 37 °C with 2 nM 3H-dihydrotestosterone in the presence or absence of increasing concentrations of the steroid to be tested. Inhibition constants (Ki) were determined as the concentration of competitor-required for 50% inhibition of3H-dihydrotestosterone binding. In addition, relative binding activity (RBA) of each test compound was calculated. Each competitor was tested in at least two different cell strains. The concentrations of unlabeled methyltrienolone (a synthetic nonmetabolizable androgen) and dihydrotestosterone for 50% inhibition of3H-dihydrotestosterone binding were in the same order of magnitude, namely, 2 nM (2.2 respectively, 2.4 nM), whereas the affinity of testosterone was approximately one-fifth that of dihydrotestosterone. Other potent competitors for dihydrotestosterone binding were three progestins (norgestrel, gestoden, and medroxyprogesterone acetate) which have Ki values similar to testosterone. An order of magnitude lower Ki values (around 10−7 M) were found for the androgen 17α-propylmesterolone, the antiandrogen cyproterone acetate, and the progestin norethisterone acetate. Binding affinities of all other steroids to the androgen receptor were markedly lower and showed the following order of potency: estrogens (estradiol, ethinyl estradiol, diethylstilbestrol) 〉 glucocorticoids as well as aromatase inhibitors and potassium canrenoate. We conclude that (a) among the compounds tested some progestins are very potent in their ability to interact with human skin fibroblast receptors and thus may affect endogenous androgen action; (b) estrogens are relatively weak androgen receptor binders; and (c) this receptor assay in combination with pharmacokinetic and metabolic studies appears to be a useful screening test to evaluate the potency of various steroids for androgen and antiandrogen therapy.

    Description / Table of Contents: Zusammenfassung Die Arbeit schildert einleitend das heutige Wissen über die Funktion der Talgdrüsen bei Alopecia areata: Die bisherigen Befunde sind aber widersprüchlich; sie stützen sich zudem nur auf morphologische Untersuchungen. Das Ziel der Arbeit ist, die Talgrüsenfunktion bei der Alopecia areata anhand quantitativer Talgbestimmungen weiter abzuklären. Zunächst werden die gebräuchlichen Methoden zur Talggewinnung aufgeführt. In den eigenen Untersuchungen wird dann nach einem von Pantlitschko u. Raab ausgearbeitetem — hier modifiziertem Verfahren — die Talgverteilung und Talgproduktion über Stirn und Areata-Herden bestimmt. Abschließend wird die Talgdrüsenverteilung und Talgdrüsenaktivität mit einer von Brun et al. entwickelten Färbmethode auf Filterpapierchen abgebildet. Bei zehn Patienten mit Alopecia areata und vier Patienten mit Alopecia areata totalis werden 260 Talgbestimmungen durchgeführt, die Follikelanordnung der Teststellen wird auf 103 Filterpapierchen festgehalten. Die Auswertung der Meßergebnisse zeigt unter anderem: Die Sekretionsleistung der Talgdrüsen nimmt bei Alopecia areata mit fortschreitender Erkrankungsdauer ab.
    Notes: Summary The literature as what is to date known as the function of sebaceous glands in alopecia areata is discussed: however it was found that past findings are contradictory and were moreover based exclusively on morphological research. The aim of this thesis was therefore to clarify the function of sebaceous glands in The aim of this thesis was therefore to clarify the function of sebaceous glands in alopecia areata using a quantitative method to determine the amount of sebum produced. Firstly current methods of obtaining sebum are described. Then the author's own findings on sebum distribution and production in frontal and areata-centers based on a modified method first described by Pantlitschko and Raab are discussed. Finally using a filter paper colouring technique developed by Brun et al. sebaceous gland distribution and activity is illustrated. 260 sebum analyses were made from 10 patients with alopecia areata and 4 with alopecia areata totalis. The follicular pattern in the test area is visible as shown on 103 pieces of filter-paper. Analysis of the results shows that in prolonged alopecia areata the secretory efficiency of the sebaceous gland steadily decreases with duration of the disease.

    Description / Table of Contents: Zusammenfassung Über die Funktion von Talgdrüsen bei der androgenetischen Alopecie liegen bisher nur wenige quantitative Angaben vor. Die Talgdrüsenfunktion wird nun mittels quantitativer Lipidbestimmungen analysiert. Bei acht Männern mit androgenetischem Haarverlust sowie einem Mann mit dichtem Haar werden 131 Sebumteste durchgeführt, um Talgverteilung und-produktion über der Kopfhaut und—zum Vergleich auch über der Stirn—zu bestimmen. Anschließend wird die Talgdrüsenfunktion auf in Osmiumsäuredämpfen geschwärzten Filterpapierchen abgebildet. Die Auswertung der Meßwerte zeigt: mit fortschreitender Dauer der Erkrankung bleibt bei der androgenetischen Alopecie die Sekretionsleistung der Talgdrüsen meist nicht nur erhalten, sondern wird noch erheblich gesteigert.
    Notes: Summary Only a few quantitative findings exist on sebaceous gland function in common baldness. The sebaceous gland function is further explored with help of quantitative lipid determination. 131 sebum tests were carried out on 8 bald-headed and 1 man with a normal amount of hair to determine sebum distribution and production. For comparison similar tests were performed on the forehead of each. Lastly, sebaceous gland patterns using filter paper impregnated with osmic acid are illustrated. Analysis of the results show that with continued duration of the disease not only most is the secretory performance of the sebaceous gland upheld but actually increases noticeably.

    Description / Table of Contents: Summary Aldosterone excretion rate and plasma renin activity were assessed in 4 aeromegalic patients. Aldosterone levels were normal in all instances while renin remained rather low and showed only a very slight increase when measured in the upright position. The possible causes of plasma renin activity suppression are discussed.
    Notes: Zusammenfassung Bei 4 Patienten mit Akromegalie wurden Bestimmungen der Aldosteronexkretionsrate und der Plasmareninaktivität durchgeführt. Bei allen Patienten lag das Aldosteron im Normbereich, während sich das Renin an der unteren Normgrenze bewegte und vor allem nach aktiver Orthostase eine subnormale Stimulation aufwies. Die möglichen Ursachen dieser Suppression der Plasmareninaktivität werden diskutiert.
