    Notes: The classical triad of sudden devastating chest pain, electrocardiographic absence of acute myocardial Infarction, and Identification of an upstream flap in the ascending aorta by transesophageal echocardlography (TEE) Indicates aortic type A dissection requiring emergent surgery. Among 34 patients presenting with clinical signs and symptoms of an aortic dissection, three did not show the mandatory flap in the upstream aorta. The only echocardlographic finding was aortic wall thickening Indicating an intramural hematoma. Two of these patients showed early aortic ectasia and one showed a pericardial effusion. Despite the missing flap echocardiographlcally, surgery was performed in all three patients. The surgical approach was the same as that for patients with a type A dissection. Two patients are doing well after the procedure, and one patient died after reoperation. The postoperatlve histologic work-up confirmed that there was no intimal tear or dissection of the intimal layer. We conclude that the echocardiographic finding of an Intramural hematoma combined with typical clinical signs of chest pain, with myocardial infarction ruled out, requires emergent surgical intervention.     Notes: Abstract In this paper the problem of stopping the Multistart algorithm for global optimization is considered. The algorithm consists of repeatedly performing local searches from randomly generated starting points. The crucial point in this algorithmic scheme is the development of a stopping criterion; the approach analyzed in this paper consists in stopping the sequential sampling as soon as a measure of the trade-off between the cost of further local searches is greater than the expected benefit, i.e. the possibility of discovering a better optimum. Stopping rules are thoroughly investigated both from a theoretical point of view and from a computational one via extensive simulation. This latter clearly shows that the simple1-step look ahead rule may achieve surprisingly good results in terms of computational cost vs. final accuracy.

    Notes: Summary Since 1950 many animal radiotracer experiments have been performed to study inner ear kinetics. For the most part in these studies, radionuclides were applied systemically, following which a discontinuous probing of inner ear fluids or of inner ear tissues was done. Two techniques have been developed in the Section for Experimental Otorhinolaryngology of the University of Würzburg. These have been adapted to the direct and continuous measurements of inner ear efflux kinetics for several hour periods. For this purpose, only a tiny amount of radiotracer need be applied directly to the inner ear. Experiments were done on the anesthetized guinea pig as an animal model. In the first technique, a collimator-detector system is focused precisely on the cochlea, which had been quickly resealed after application of the radionuclide bolus via two small holes in the basal turn of the cochlea. The second technique makes use of a perilymph cycling system, whereby a small outer volume includes a microcuvette with a socalled artificial round window. By this latter cycling technique, perilymph clearance kinetics of all kinds of radiotracers — with the exception of tritium labelled ones — can be measured. Calculations from clearance kinetics show that quite small particles with particle weights up to 100, such as the chlorine anion and the potassium cation, as well as urea, glycerol, pyruvate, and lactate, exhibit perilymphatic half-lives varying from 45 to 60 min. These half-live data are plausible in regard to cochlear blood flow measured previously via an independent technique developed by Angelborg et al. For particle weights distinctly beyond 100, half-lives increased gradually according to the operation of a perilymph-blood barrier. For a few tracers such as theophylline, nicotinamideadenine-dinucleotide, urografin, and biligrafin, individual effects are superimposed, giving rise to rather fast kinetics. In contrast, the ototoxic drugs ethacrynic acid and tobramycin exhibit a certain retardation in their clearance kinetics. Very small non-polar gaseous particles such as hydrogen and xenon show extremely short perilymphatic/cochlear half-lives. The half-life of hydrogen is about 4 min which accounts for a maximum clearance consistent with total cochlear blood flow.

    Notes: Summary The effects of some modern high ceiling loop diuretics on the guineapig's inner ear are tested. Short-time experiments are run registrating the cochlear microphonic potentials (CMP) before and after i.v. bolus injections of a diuretic. A 100-point matrix program is performed by automatic sampling of the CMPs after stimulating by five frequencies at 20 sound pressure levels each. Data can be plotted in different scales within 10 min after the end of a whole-day experiment. The data are on-line processed by a IBM 1800 computer.

    Notes: Summary In the total abdominal island skin flap of the rat based on both epigastric vessels in the groin postoperative development after arterial or venous or complete division of the neurovascular bundle was documented. Photographical documentation, planimetry of the surviving skin area were used as methods together with evaluation of the blood supply of the flaps by Xe-133-Clearance. The resulting new architecture of vessels was shown photomacroscopically by Indian ink injection. The function of arterial inflow and venous outflow was pointed out by microangiographical fluorescein methods. So it was possible to combine special operation methods with definitive new vessel structures, typical necrotic areas and postoperative mean value clearance results. Caused by the difference of the Xenon clearance results in the singular case this method was not able to give a secure prognosis concerning the survival of the special flap. The fluorescein microangiography allowed to point out the type of operation choosed and the functional evaluation of the blood supply of the flap.

    Notes: Abstract In a recent paper the authors introduced an infinite class of global optimization algorithms based upon random sampling from the feasible region and local searches started from selected sample points, based upon an acceptance/rejection criterion. All of the algorithms of that class possess strong theoretical properties. Here we analyze a member of that family, which, although being significantly simpler to implement and more efficient than the well known Multi-Level Single-Linkage algorithm, enjoys the same theoretical properties. It is shown here that, with very high probability, our method is able to discover from which points Multi-Level Single-Linkage will decide to start local search.

    Notes: Abstract The present rationale for clinical defibrillation is empirically based, and the technique is the same for different paddle locations and anatomical configurations. Using a two-dimensional finite element model, we studied the effects of such variations on the distribution of myocardial current during defibrillation, and developed a method to quantitatively assess the potential for a defibrillating shock to cause myocardial damage. We used this method to compare five paddle placements, two paddle sizes and three variations of thoracic anatomy. Anatomical variations did not affect current distributions during electrical defibrillation, whereas paddle position had a marked effect. Paddle positions close to the heart with interposed bony structures between the heart and electrode produced focal regions of much higher than average myocardial current magnitudes. Myocytes in the highest current intensity regions dissipated 49 times the energy dissipated by cells exposed to threshold current magnitudes, potentially causing focal myocardial damage. An alternative paddle position was identified which significantly improved the myocardial current distribution. Maximum energy dissipation was only 4·8 times threshold energy, while defibrillation efficiency was reduced by 〈7 per cent, minimising the risk of myocardial damage. These results suggest that paddle position plays a dominant role in determining myocardial current distributions and defibrillation-induced damage.

    Notes: This study investigated as a small diameter vascular replacement the tissue formed adjacent to an implanted cylindrical foreign body, heterologously transplanted. Grafts of 4 mm i.d. were grown in rabbits and transplanted to dogs as segmental carotid and femoral artery replacements. These maintained 50% patency after 3 weeks. Variables in the further development of this concept are discussed.

    Notes: Evidence for the lack of toxicity of various bioglass formulations has been deduced from studies carried out, both in vivo and in vitro, in several different centers. Recent studies of the authors, described here, include testing of solid bioglass implants in the soft tissues of rats and rabbits for time periods of up to eight weeks. Two new techniques are described for the toxicological testing of particulate biomaterials. These tests, which involve rat peritoneal macrophages in culture and a mouse pulmonary biomaterial embolus model, indicate the biocompatibility of bioglass powders. Thus, the surface activity so critical in bone adhesion is without toxic effect in nonosseous tissues in contact with solid bioglass implants. Should wear occur and produce particulate bioglass, the material should be eliminated without consequence.
