    Notes: The expression of CD44 isoforms (CD44std, CD44v6, CD44v10) was investigated by an immuno-histochemical technique in 42 basal cell carcinomas (BCC) of the superficial and nodular variety. All BCCs studied displayed very low amounts of CD44std, a receptor for hyaluronic acid. Except for single CD44std-positive cells located preferentially in the central parts of the BCC nests, the bulk of the tumour formations were CD44std-negative. CD44v6 showed a heterogeneous distribution pattern accentuated in the peripheral palisading tumour cells. In superficial BCCs, the labelling intensity for CD44v6) increased with the size of the tumour nests. CD44v10 was not detectable in BCCsOur findings support the notion that CD44v6 is not linked to the metastatic proclivity of tumours originating from keratinocytes. We suggest that the very low expression of the receptor for hyaluronic acid (CD44std) may be one of the factors which block the formation of metastases from BCCs.

    Notes: Septoria nodorum leaf and glume blotch is an important disease of triticale (×_Triticosecale Wittm.) and can cause severe losses of grain yield in some regions. Quantitative genetic parameters for resistance were estimated for 2 years in two locations in triticale genotypes artificially inoculated with S. nodorum. The effect of infection was assessed by a visual symptom rating of flag leaves and spikes and by grain yield traits relative to an uninoculated control. The mean ratings of flag leaves and spikes, calculated from two to four ratings, were 2.6 and 3.9, respectively, with a range of six ratings for spikes and over five for flag leaves. Infection caused an 11.5% mean reduction in kernel weight per spike, which was the result of 13.2% lower 1000-kernel weight. The number of kernels per spike and 50-ml weight were little affected. For all relative grain yield traits, genotypic variation was small with high genotype-environment interaction effects and thus moderate to low heritabilities. In contrast, for visual ratings genotypic variation was high, with low interaction effects leading to high heritabilities. Phenotypic correlation between flag leaf and spike ratings was low, indicating independent disease resistance mechanisms. The best association, although still moderate, was obtained between flag leaf rating and relative 1000-kernel weight. Therefore, visual disease ratings do not satisfactorily assess the effect of Septoria infection on grain yield traits. The reduction in 1000-kernel and possibly 50-ml weight are good indicators, provided that multi-environment tests are conducted.

    Notes: Abstract On the basis of two arteriovenous fistulas, one arteriocaliceal fistula, and the literature concerning these complications, clinical symptoms, diagnostic measures, and therapeutic strategies are discussed. Decreased renal function, severe hypertension, and a bruit over the transplant site —particularly after core biopsy — are said to be indicative of an arteriovenous fistula, while persisting hematuria is seen as evidence of an arteriocaliceal fistula. In both cases, angiographic evaluation is indicated. Therapeutic possibilities include selective angiographic embolization and surgical repair. Large intraparenchymal fistulas may require wedge resection.

    Notes: Abstract Class II major histocompatibility complex (MHC) antigen induction was investigated on enterocytes of heterotopic rat small bowel allografts in the Lewis-Brown Norway strain combination and on isografts in the Lewis-Lewis strain combination. I a antigens were detected with monoclonal antibodies using an immunoperoxidase technique. Generally, MHC class II antigens were not exhibited in the isografted group, with the exception of two long-term isografts that presented the same pattern as normal small bowel. In these cases, I a was expressed in a patchy distribution predominantly in the villi, and only very few enterocytes stained positive in Lieberkühn's crypts. Allografted rats showed a typical pattern of I a expression on the enterocytes during the rejection course. The initial expression was confined to the crypts, indicating a very early stage of rejection when compared to histological findings. More advanced stages of rejection were accompanied by increasing I a biosynthesis in the crypts and I a expression by the epithelium lining the villi. Cyclosporin (CyA) was not able to fully inhibit MHC class II antigen expression; however, the appearance of I a was delayed.

    Notes: Abstract In the kidney a striking parallel exists between the expression of ecto-5′-nucleotidase and of erythropoietin by renal fibroblasts. It was therefore hypothesized that the expression of ecto-5′-nucleotidase in fibroblasts might be controlled by oxygen tension. In order to test this hypothesis, we examined the distribution of the enzyme in a tissue which displays a defined zonation in respect to oxygen tension, namely in the liver; anaemia was used in order to exaggerate this zonation. The distribution of ecto-5′-nucleotidase was investigated by light and electron microscopy using enzyme and immunohistochemical methods in the livers of healthy and of anaemic rats. Anaemia was produced by haemolysis combined with X-ray irradiation. The enzyme was detected in the bile canaliculi, in the connective tissue of the portal triads and of the central veins, and in fat-storing cells probably corresponding to a special form of fibroblasts. In healthy animals the perisinusoidal ecto-5′-nucleotidase activity was slightly higher in the pericentral than in the periportal area of the acinus whereas the inverse was observed for the staining of bile canaliculi. Anaemia provoked an increase of ecto-5′-nucleotidase in fat-storing cells in the pericentral zone of the acinus and in fibroblasts around the central veins, resulting in steepended gradients along the sinusoids. The intralobular gradient of ecto-5′-nucleotidase in perisinusoidal cells and the effect thereon of anaemia suggest that the expression of the ecto-5′-nucleotidase might be directly or indirectly controlled by local oxygen tension.

    Notes: Abstract Background: Rectus sheath hematoma (RSH) is a rare entity that can mimic an acute abdomen. Therefore, we designed a study to analyze the etiology, frequency, diagnosis using ultrasound, and treatment of RSH. Methods: A total of 1,257 patients admitted for abdominal ultrasound for acute abdominal pain or unclear acute abdominal disorders were evaluated. Results: In 23 (1.8%) patients, an RSH was diagnosed; three of them were not diagnosed preoperatively by ultrasound. Of 13 men and 10 women (mean age, 57 ± 23 years), 13 developed RSH after local trauma, three after severe coughing, two after defecation, and five spontaneously. Fifteen had nonsurgical therapy, and eight underwent surgery. The use of anticoagulants was accompanied by a larger diameter of the RSH (p 〈 .012), and surgical therapy was more frequently required in these patients. In the surgically treated group, more intraabdominal free fluid could be detected by ultrasound (p 〈 .0005), patients required less analgesics (p 〈 .001), and the mean hospital stay was shorter (p 〈 .001). Conclusions: RSH is a rare condition that is usually associated with abdominal trauma and/or anticoagulation therapy. Ultrasound is a good screening technique. Nonsurgical therapy is appropriate but leads to a greater need for analgesics. Surgery should be restricted to cases with a large hematoma or free intraabdominal rupture.

    Description / Table of Contents: Zusammenfassung Wir untersuchten die Frage, ob es gelingt, mit einer hypothermen Leberperfusion therapeutisch wirksame Cytostaticakonzentrationen im Gewebe zu erzeugen. In Lebern von SD Ratten wurden durch direkte Zellinoculation Walker Tumore erzeugt. Die Lebern wurden in situ mit verschiedenen Konzentrationen von Doxorubicin hypotherm perfundiert. Die Konzentrationen im tumortragenden Gewebe korrelierten direkt mit der Perfusionsdauer und der Perfusatkonzentration. Die hypothermen Cytostaticaperfusionen haben einen deutlichen therapeutischen Effekt. Ein entscheidender Vorteil des vorgestellten hypothermen Verfahrens ist der geringere apparative Aufwand bei vergleichbarem therapeutischen Effekt.
    Notes: Summary We were interested whether hypothermic drug perfusion would yield an effective concentration of a cytotoxic drug in tumor-bearing rat livers. Walker tumors were inoculated by a single injection of diluted cells into one liver lobe. The livers were perfused in situ with various concentrations of doxorubicin. The concentrations found in the liver correlated well with the perfusion time and the drug concentration in the perfusate. The hypothermic isolated liver perfusions showed a marked therapeutic effect. A major advantage of this approach is the simplicity of the hypothermic perfusion device compared with normo- or hyperthermic devices with a similar therapeutic effect.

    Description / Table of Contents: Zusammenfassung Zur Verhinderung von Gleitphänomenen am evertierten Dünndarm wurden bei 26 Patienten konventionelle Brooke-Ileostomien beiderseits paramesenterial mit Klammernahtreihen stabilisiert. Dabei wurden keine Frühkomplikationen beobachtet. Im Vergleich zu einem historischen Kontrollkollektiv konnten Spätkomplikationen (Retraktion, Prolaps) in einem mittleren Beobachtungszeitraum von 2 Jahren von rund 16 auf 0% reduziert wurden. Das formstabile Stoma erleichterte zusdtzlich die Versorgung.
    Notes: Summary In order to prevent nipple-sliding of the everted small bowel, conventional Brooke ileostomies in 26 patients were stabilized on both sides of the mesentery by longitudinal rows of staples. There were no early complications. With respect to historical controls with unstapled ileostomies, late complications (retraction, prolapse) could be reduced from about 16 to 0% in a median observation time of 2 years. The ileostomies were stable in size and form and therefore appliances were facilitated.

    Notes: Abstract This study was designed to investigate whetherin combined stomach, small bowel, and pancreas transplantation allograft rejection occurs in the individual organs concomitantly and with the same intensity. Heterotopic enbloc transplantation of the stomach, small bowel, and pancreas was performed in a Lewis-to-Brown Norway rat combination. Group 1 animals received no immunosuppressive therapy while animals in group, 2 were treated with cyclosproin (10 mg/kg body weight, orally) daily. Grafts were histologically evaluated on the 5th (subgroups 1a and 2a) and 10th (subgroups 1b and 2b) postoperative days. The degree of rejection was defined as moderate, intermediate, or severe according to predefined criteria. The results indicate that the small bowel is more susceptible to rejection than either the stomach or the pancreas. Mucosal biopsies of the stomach are unlikely to provide a reliable guide to rejection in the small bowel.
