    Notes: Zusammenfassung Eine chronische Niereninsuffizienz muß nicht mit einer Isosthenurie im osmotischen Sinne, d. h. mit einer Isotonie von Harn und Plasma, verbunden sein. Beobachtungen an 11 Patienten zeigen, daß in 6 Fällen der Harn entweder ständig oder vorübergehend gegenüber dem Blutplasma hypotonisch ist. Die osmotische Differenz zwischen Harn und Plasma kann bis zu 100 mOsm/1 erreichen. Dieser Befund kann nicht als Wasserdiurese gedeutet werden. Wahrscheinlicher erscheint uns, daß bei chronischer Niereninsuffizienz in den übriggebliebenen funktionierenden Nephren eine osmotische Diurese auftritt, bei welcher die in den proximaleren Abschnitten entstandenen osmotischen Gradienten in den distaleren Tubulusabschnitten nicht ausgeglichen werden.

    Description / Table of Contents: Zusammenfassung An dem Modell einer experimentellen Niereninfektion bei Ratten haben wir die therapeutische Wirksamkeit einer Gabe mit der von drei täglichen Gaben von Gentamicin verglichen. Im Frühstadium der Infektion ist die Wirkung der beiden Therapieformen nicht statistisch unterscheidbar. Die Wirkung der verschiedenen Verabfolgungsintervalle auf die fortgeschrittene Infektion hing von der Dauer der Therapie ab. Wurde dieselbe Dosis auf einmal oder in drei Injektionen verabreicht, stellte man nach fünf Tagen keine statistischen Unterschiede fest. Nach zehn Tagen wurde die Therapie wirksamer, wenn die Tagesmenge dreimal täglich injiziert worden ist. Der Vergleich einer einmaligen Tagesdosis, die zwei Drittel der dreimal täglich je acht Stunden verabreichten Gesamtdosis betrug, zeigte nach fünf Tagen eine statistisch niedrigere Keimzahl in der Niere als nach drei Injektionen täglich.
    Notes: Summary The therapeutic efficacy of gentamicin given once or three times a day was compared in a model of experimental renal infection in rats. The same amount of gentamicin given either in a single injection or three injections a day produced no statistically significant difference in the treatment of incipient infection. The effect of this mode of administration on advanced infection depended on the length of the therapy. After five days, administration of the same dose given in a single injection or three injections did not result in significant differences. After ten days the therapy proved more effective when gentamicin was injected three times a day. In comparison a single daily dose, amounting to two thirds of the total dose when given three times a day every eight hours, revealed after five days of therapy a statistically significantly lower bacterial count in the kidney than three daily injections of gentamicin.

    Notes: Abstract In 25 patients in different stages of chronic pyelonephritis the renal clearance of uric acid, osmotically active substances and endogenous creatinine and their mutual relationship were investigated. It was found that the ratioC ua/C cr×100 rises in a hyperbolic relation with the decline of endogenous creatinine clearance. The rise of the ratioC ua/C cr×100 rose in a linear relationship to the fractional excretion of all osmotically active substances (C osm/C cr×100). The findings support the idea that osmotic diuresis in the residual nephron is one of the factors conditioning the relative increase of renal uric acid excretion.

    Notes: Abstract The production of hypotonic urine was investigated in 20 subjects suffering from chronic pyelonephritis. The minimum urinary osmolality was significantly influenced by the NaCl content in the diet. Under conditions of low NaCl intake (15 mEq Na/day) the minimum urinary osmolality decreases significantly as compared with the values obtained under normal salt intake, and can reach normal values. The results indicate that the diluting function of the kidneys may be improved by the increase of fractional sodium reabsorption.
