Multisite biventricular pacing therapy offers significant clinical improvement in some stimulated patients with electrocardiographic criteria of cardiac dyssynchrony. However, observational data increasingly suggest that patients suffering from congestive heart failure in presence of modest QRS widening may also derive benefit from cardiac resynchronization therapy (CRT), and that some patients can be significantly improved clinically after system implantation despite no apparent change in QRS width. This pilot study explored the value of an echocardiographic model to identify cardiac electromechanical dyssynchrony parameters (EDP) in candidates for CRT, and their potential correction after implantation. The study included 66 consecutive CRT recipients of CRT in NYHA functional class III or IV who had one or more atrioventricular, interventricular or intraventricular dyssynchrony criteria. An immediate improvement was observed in 85% of the population with a partial or total correction of their EDP. However, the modifications in EDP differed considerably between recipients of de novo CRT systems and patients with previously implanted standard pacing systems upgraded with the implantation of a left ventricular lead. EDP measurements appear to identify potential candidates for CRT, and to confirm the success of system implantation. (PACE 2003; 26[Pt. II]:137–143)

    Notes: Overlapping biphasic (OLBI) stimulation is a new pacing waveform consisting of two simultaneous monophasic pulses of opposite polarities applied to a bipolar electrode. The goal of this prospective study was to compare, using conventional pacing leads, the acute energy pacing thresholds, measured at 0.5-ms pulse duration, associated with bipolar versus OLBI (mode 7 and mode 8) pulse delivery. Results: Thirty one leads were tested in 20 patients. Of these leads, 7 (23%) were implanted chronically, 12 (39%) were in atrial positions, and 19 (61%) in ventricular positions. Energy pacing thresholds were significantly lower (-25.6 ± 25.6 %, P = 0.005) in OLBI mode 8 (1.30 ± 3.96 μJ) compared to bipolar (1.55 ± 4.37 μJ) pacing, regardless of the pacing site or length of service of the leads. In contrast, OLBI mode 7, which has an anodal component, at the tip electrode was associated with higher energy pacing thresholds (3.65 ± 6.48 μJ; +358.3 ± 219.4%, P = 0.002). Conclusions: Mode 8 OLBI pacing is associated with lower acute energy pacing thresholds when used with bipolar leads in contact with the myocardium. OLBI pacing may increase pacemaker longevity by reducing long-term energy consumption.

    Description / Table of Contents: Zusammenfassung In der vorliegenden Studie werden Methoden zum Nachweis und zur Darstellung der Gc-Subtypen verglichen. Die isoelektrische Fokussierung findet im Agarosegel pH 4–6 und im Polyacrylamidgel pH 4,5–5,4 Anwendung. Die Nachweismethoden der direkten Immunfixation mit Anti-Gc, der Immunfixation mit anschließendem Enzym-Antikörperkomplex, des passiven Blot und des Semi-dry-Electroblot hinsichtlich ihrer Empfindlichkeit und Nachweisgrenzen werden überprüft. Erste Untersuchungen an auf Baumwollstoff angelegten Blutspuren unterschiedlichen Alters schließen sich an. Die Auftrennung des Gc-Proteins im Polyacrylamidgel erweist sich als Methode der Wahl, insbesondere wegen guter Bandenschärfe und genügend großem Korridor zwischen den 1S- und 1F-Banden. Innerhalb der Darstellungstechniken ist eindeutig dem Semi-dry-Electroblot mit anschließender enzymgekoppelter Antigen-Antikörperreaktion der Vorzug zu geben. Hier erfolgt durch eine vertikale Elektrophorese ein annähernd 100%iger Transfer des aufgetrennten Gc-Proteins in eine Nitrozellulosemembran, welche im nachfolgenden 2-Schrittverfahren (Antigen-Antikörperreaktion und gekoppelte Enzym-Substratreaktion) zur Darstellung desselben eingesetzt wird. Durch Kombination dieser Techniken können maximale Serumverdünnungen von über 1/500 bestimmt werden; ferner lassen sich nach ersten Versuchen Blutspuren bis zu einem Mindestalter von 20 Wochen bei Raumtemperaturlagerung eindeutig typisieren.
    Notes: Summary A comparison of separation and detection techniques has been carried out to determine the most suitable combination for use in Gc-subtyping. The best results (i.e., high sensitivity, distinct bands, especially with reference to the 1S and 1F separation) were achieved using isoelectric focusing in polyacrylamide gel (pH 4.5–5.4) followed by transfer to nitrocellulose membrane by electroblotting and finally detection with enzyme-linked antibody complex.

    Notes: Abstract Environmental tritium concentrations will be measured near the Tokamak Fusion Test Reactor (TFTR) to help validate dynamic models of tritium transport in the environment. For model validation the database must contain sequential measurements of tritium concentrations in key environmental compartments. Since complete containment of tritium is an operational goal, the supplementary monitoring program should be able to glean useful data from an unscheduled acute release. Portable air samplers will be used to take samples automatically every 4 hours for a week after an acute release, thus obtaining the time resolution needed for code validation. Samples of soil, vegetation, and foodstuffs will be gathered daily at the same locations as the active air monitors. The database may help validate the plant/soil/air part of tritium transport models and enhance environmental tritium transport understanding for the International Thermonuclear Experimental Reactor (ITER).

    Description: Cardiac resynchronization therapy (CRT) is a proven treatment for heart failure but ~30% of patients appear to not benefit from the therapy. Left ventricular (LV) endocardial and multisite epicardial [triventricular (TriV)] pacing have been proposed as alternatives to traditional LV transvenous epicardial pacing, but no study has directly compared the hemodynamic effects of these approaches. Left bundle branch block ablation and repeated microembolizations were performed in dogs to induce electrical dysynchrony and to reduce LV ejection fraction to 〈35%. LVdP/d t max and other hemodynamic indexes were measured with a conductance catheter during LV epicardial, LV endocardial, biventricular (BiV) epicardial, BiV endocardial, and TriV pacing performed at three atrioventricular delays. LV endocardial pacing was obtained with a clinically available pacing system. The optimal site was defined as the site that increased dP/d t max by the largest percentage. Implantation of the endocardial lead was feasible in all canines ( n = 8) without increased mitral regurgitation seen with transesophageal echocardiography and with full access to the different LV endocardial pacing sites. BiV endocardial pacing increased dP/d t max more than BiV epicardial and TriV pacing on average ( P 〈 0.01) and at the optimal site ( P 〈 0.01). There were no significant differences between BiV epicardial and TriV pacing. BiV endocardial pacing was superior to BiV epicardial and to TriV pacing in terms of acute hemodynamic response. Further investigation is needed to confirm the chronic benefit of this approach in humans.

    Description: Aims The long-term clinical value of the optimization of atrioventricular (AVD) and interventricular (VVD) delays in cardiac resynchronization therapy (CRT) remains controversial. We studied retrospectively the association between the frequency of AVD and VVD optimization and 1-year clinical outcomes in the 199 CRT patients who completed the Clinical Evaluation on Advanced Resynchronization study. Methods and results From the 199 patients assigned to CRT-pacemaker (CRT-P) (New York Heart Association, NYHA, class III/IV, left ventricular ejection fraction 〈35%), two groups were retrospectively composed a posteriori on the basis of the frequency of their AVD and VVD optimization: Group 1 ( n = 66) was composed of patients ‘systematically’ optimized at implant, at 3 and 6 months; Group 2 ( n = 133) was composed of all other patients optimized ‘non-systematically’ (less than three times) during the 1 year study. The primary endpoint was a composite of all-cause mortality, heart failure-related hospitalization, NYHA functional class, and Quality of Life score, at 1 year. Systematic CRT optimization was associated with a higher percentage of improved patients based on the composite endpoint (85% in Group 1 vs . 61% in Group 2, P 〈 0.001), with fewer deaths (3% in Group 1 vs. 14% in Group 2, P = 0.014) and fewer hospitalizations (8% in Group 1 vs. 23% in Group 2, P = 0.007), at 1 year. Conclusion These results further suggest that AVD and VVD frequent optimization (at implant, at 3 and 6 months) is associated with improved long-term clinical response in CRT-P patients.
