    Notes: Background: Children frequently experience harmful whealing and delayed papules from mosquito bites. Whealing is mediated by antisaliva IgE antibodies and histamine, but the effect of antihistamines on mosquito-bite symptoms has not been evaluated in children. Methods: The effect of loratadine (0.3 mg/kg) was examined in 28 mosquito-bite-sensitive children (aged 2–11 years). The double-blind, placebo-controlled, crossover study was performed with exposure to Aedes aegypti laboratorymosquitoes. The size of the bite lesion and the intensity of pruritus (visual analog scale) were measured at 15 min and at 2, 6, and 24 h. Results: Loratadine decreased the size of the wheals by 45% (P〈0.001, 25 children) and accompanying pruritus by 78% (P=0.011, 12 children) at 15 min compared to placebo. The size of the 24-h delayed bite lesion also decreased significantly (P=0.004), but there was no change at 2 or 6 h. Loratadine was well tolerated and no marked side-effects were recorded. Conclusions: This study in children shows that prophylactically given loratadine decreases significantly the whealing and pruritus caused by mosquito bites and also reduces the size of the 24-h bite lesions. Therefore, the therapeutic profile of loratadine extends from immediate to delayed allergic symptoms in mosquito-bite-sensitive children.

    Notes: Backround: Patients allergic to natural rubber latex (NRL) frequently show positive skin prick tests (SPT) and hypersensitivity reactions to various fruits, such as avocado, banana, and kiwi, as well as to vegetables such as potato. Methods: Hev b 7 was purified from NRL “C-serum” and Sol t 1 from potato extract, and they were detected by immunoblotting. IgE antibodies to Hev b 7 and Sol t 1 were measured with ELISA in sera from 35 adults and 35 children allergic to NRL. ELISA inhibition and immunoblotting were used to study allergen cross-reactivity. The in vivo reactivity of Hev b 7 and Sol t 1 were demonstrated in the SPT. Results: Seventeen (49%) of the 35 NRL-allergic adults had IgE antibodies to Hev b 7, in contrast to none of the 35 NRL-allergic children. Fifteen (43%) of the NRL-allergic adults and 23 (66%) of the NRL-allergic children had IgE antibodies to Sol t 1. Ten (29%) of the adult sera showed IgE binding to both Sol t 1and Hev b 7, and crosswise inhibition tests with pooled sera revealed marked cross-reactivity. In the SPT, both natural Hev b 7 and Sol t 1 were able to produce a wheal and flare reaction. Conclusions: One-half of the NRL-allergic adults, but none of the NRL-allergic children, had IgE antibodies to natural Hev b 7. These results suggest that Hev b 7 is an important NRL allergen for adults, but not for children. Elucidation of the clinical importance of the observed cross-reactivity between Hev b 7 and Sol t 1 requires further studies.

    Notes: Twelve patients with dermatitis herpetiformis whose skin condition responded to a gluten-free diet (GFD) were re-examined after diet treatment. The findings were compared with those in matched patients on a normal diet. Jejunal histology revealed morphological improvement in every patient on a GFD whereas all patients on a normal diet continued to have villous atrophy. Intra-epithelial lymphocyte counts were normal in 8 patients on a GFD in contrast to one on a normal diet. Immunofluorescence examination of the jejunal mucosa revealed that the numbers of cells containing IgA and IgM were increased significantly in the normal diet group. The figures were lower in the GFD group but these also exceeded the values in the controls.IgA deposits were found in the uninvolved skin of every patient irrespective of the diet treatment, but the fluorescence seemed to be less intense in patients on a GFD. A clear difference was found in the occurrence of C3 deposits in the uninvolved skin. Three patients on a GFD had C3 deposits; two of these did not follow a strict diet. However C3 was found in 8 patients on a normal diet.Circulating dietary and auto-antibodies were found in two patients on a GFD and in 9 on a normal diet. Serum immunoglobulin (IgG, IgA, IgM) and complement (C3, C4) levels were within normal limits in both patient groups.

    Notes: A mother with herpes gestationis and her child without cutaneous lesions had analogous immunohistological findings. In the skin they had deposits of IgG and C3 component of complement at the basement membrane zone. Circulating IgG antibody to the basement membrane was also demonstrated in both. The mother also had a high titre of anti-HLA antibody against a histocompatibility antigen (HLA-B8) present in the child. A possible role of this HLA antibody in the pathomechanism of herpes gestationis is discussed.

    Notes: Cell infiltrates of biopsy specimens from nodular or papulovesicular skin lesions of seven patients with scabies were characterized by α-naphthyl acetate esterase (ANAE) staining and immunoperoxidase labelling. T lymphocytes were the dominant cells in the dermal inflammatory infiltrates of both nodular and papulovesicular lesions. Their mean proportion in the deeper dermal infiltrates was 64% and in the papillary infiltrates 42%. However, they were few in number around the burrows in the epidermis. In contrast, macrophages were frequent in the epidermis and papillary dermis, where they accounted for 47% and 25% respectively of all mononuclear cells.Immunoperoxidase staining revealed immunoglobulin-positivc plasma cells (from 1% to 10%, of all mononuclear cells) only in the dermal infiltrates of nodular lesions. IgE-positive plasma cells were encountered in all four specimens. These results emphasize the importance of macrophages in scabies infestation and suggest that a local IgE-mediated antibody response may occur in nodular lesions of scabies.

    Notes: Sixty-one patients with dermatitis herpetiformis (DH) were studied. Of these fifty-three (87%) had the histocompatibility (HL-A) antigen HL-A8 as compared with 17% of a control population; the difference is significant. This incidence of HL-A8 among patients with DH is higher than that reported earlier for such patients and is similar to that reported for patients with adult coeliac disease. The incidence of HL-A1 was also significantly greater in the patient group, and was attributed to the increased frequency of haplotype HL-A1, 8. The frequency of HL-A8 was about equal in patients in whom DH was associated with jejunal abnormalities and in those in whom it was not. When correlated with rate of acetylation, there was no significant difference in the occurrence of various HL-A antigens.

    Notes:   Topical Calcineurin Inhibitors (TCIs) used for the treatment of atopic eczema modify the immune regulatory function of the skin and may have the potential to enhance immunosuppressive ultraviolet (UV) effects. Current recommendations on UV protection in eczema patients treated with PCIs are inconsistent and have given rise to uncertainty and anxiety in patients. Therefore, the European Dermatology Forum (EDF) developed a position statement which reviews critically the available data with regard to the problem, especially analysing and commenting the limitations of rodent models for the human situation. There is no conclusive evidence from rodent trials to indicate that long-term application of TCIs is photococarcinogenic. There is a need for further studies to investigate the validity of mouse models as well as long-term cohort studies in patients using TCIs. Available data suggest that long-term application of TCIs is safe, that there is no evidence of increased skin cancer risk and that it is ethical to treat patients with TCIs when indicated.

    Notes: Basophil histamine release and lymphocyte proliferation tests were examined with latex allergen prepared from surgical gloves in 15 patients with latex contact urticaria. The basophil histamine release test (BHRT) yielded positive results in 13/14 (93%) patients, whereas commercial latex RAST was positive in only 9/15 (60%) patients. Lymphocyte proliferation test (LPT) was positive in 3/15 (20%) patients, suggesting that cell-mediated immune reactions may also occur in latex allergy. However, patch tests to latex were negative and neither were epidermal Langerhans cells able to present latex antigen to T lymphocytes in vitro.
