Summary 1. On the basis of our present knowledge about the genesis of blood circulation troubles the significance of the participation of spasms is overrated. The pains, which originate rather from the tissue with a defective circulation than from the sick vessels themselves, resemble, in the cases of atonic blood circulation troubles with peristasis in the periphery, those occuring in the cases of extreme closing reactions of the vessels. 2. The pain arising from the vessels is traceably tied to an interstitial oedema of the vascular wall. Strong contractions of the wall-musculature do not cause any pains either. 3. In organic diseases of the vesels a peripheral pain-process may be exactly distinguished from a central pains. The symptoms of central pain are: periodicity, pain-kachexia, and sensibility to subliminal irritations.

Résumé 1. Basé sur la connaissance présente de la genèse de troubles circulatoires sanguins, l'importance de la participation de spasmes est surestimée. Les douleurs, naissant plutôt des tissus à circulation défective que des vaisseaux affligés ressemblent, dans les cas de troubles circulatoires atoniques avec une péristase dans la périphérie, à celles qui se passent dans les cas de réactions contractiles extrêmes vasculaires. 2. La douleur prenant naissance dans les vaisseaux est probablement liée à un oedème de la paroi vasculaire. Aussi des contractions marquées de la musculature des parois vasculaire ne causent pas des douleurs. 3. Dans les maladies organiques vasculaires un procès douloureux périphérique peut être distingué exactement d'une fixation centrale des douleurs. Les symptômes de la douleur centrale sont: périodicité, kachexie douloureuse et sensibilité à des irritations sousliminaires.

Zusammenfassung 1. Nach unserem derzeitigen Wissen über die Genese von Durchblutungsstörungen wird die anteilmäßige Bedeutung von Spasmen überschätzt. Die Schmerzen, welche mehr aus dem minderdurchbluteten Gewebe als von den erkrankten Gefäßen selbst ausgehen, sind bei atonischen Durchblutungsstörungen mit Peristase in der Peripherie gleichartig denen bei überwertigen Verschlußreaktionen der Gefäße. 2. Der von den Gefäßen selbst ausgehende Schmerz ist nachweislich an ein interstitielles Ödem der Gefäßwand gebunden. Auch starke Kontraktionen der Wandmuskulatur bedingen noch keine Schmerzen. 3. Bei organischen Gefäßkrankheiten ist ein peripheres Schmerzgeschehen von zentraler Fixierung der Schmerzen deutlich abzugrenzen. Die Zeichen des zentralen Schmerzes sind: Periodizität, Schmerzkachexie und Empfindlichkeit gegen an sich unterschwellige Reize.
