    Notes: Abstract The p100 gene coding for the p100 protein of Borrelia burgdorferi strain PKo has been cloned, sequenced and expressed in Escherichia coli. An open reading frame including upstream and downstream sequences with potential translation and transcription signals could be identified. The reading frame consists of 1989 nucleotides corresponding to a protein of 663 amino acids and a calculated molecular mass of 75.8 kDa. The protein has a leader peptide and is processed without modification at the N-terminus. A high percentage of amino acid sequence identity could be found to the high-molecular mass protein p83/p93 of B. burgdorferi strain B31.

    Notes: Abstract The outer surface protein A (OspA) of Borrelia burgdorferi is a major candidate for development of a borrelia vaccine. However, vaccine development may be aggravated by the immunological heterogeneity of OspA. In this respect the knowledge about conserved and variable epitopes is of major interest. In this study truncated proteins derived from two different OspA serotypes of B. burgdorferi were mapped for conserved and specific antibody-binding domains. The OspA fragments were reacted in the Western blot with eight different Osp Aspecific monoclonal antibodies recognizing between one and seven of the seven OspA serotypes previously described. The two broadly reacting antibodies (recognizing all serotypes) react with N-terminal fragments of 93 and 214 amino acids, respectively, whereas antibodies recognizing only one and two to four of the seven serotypes are reactive with C-terminal fragments of amino acid 143–273 and 109–273, respectively. Thus, conserved antibody-binding domains are located nearer to the N terminus than serotype-specific ones. Comparison of the results from western blot mapping with OspA sequence data suggested certain conserved or variable regions as probable candidates for antigenic sites involved in linear or conformationally dependent epitopes. This, however, needs to be confirmed by epitope mapping using the respective synthetic peptides.

    Description / Table of Contents: Zusammenfassung Es wird über die Persistenz vonBorrelia burgdorferi bei sechs Patienten berichtet. Nach dem Zecken- bzw. Insektenstich und Erythema migrans konnteB. burgdorferi noch Wochen nach der Antibiotikatherapie nachgewiesen werden. Serologische Befunde waren außer bei einem Patienten negativ. Diese Ergebnisse bestätigen unsere früheren Beobachtungen und sprechen dafür, dß die Antibiotikabehandlung die Antikörperbildung gegenB. burgdorferi beeinflussen kann. Ferner zeigen diese Ergebnisse und Beobachtungen, daß nicht nur im Frühstadium der Lyme Borreliose, sondern auch in chronischen Stadien bzw. bei Persistenz des Erregers der Nachweis von Antikörpern negativ bleiben kann.
    Notes: Summary The persistence ofBorrelia burgdorferi in patients treated with antibiotics is described. The diagnosis of Lyme disease is based on clinical symptoms, epidemiology and specific IgG and IgM antibody titers toB. burgdorferi in serum. Antibiotic therapy may abrogate the antibody response to the infection as shown in our patients.B. burgdorferi may persist as shown by positive culture in MKP-medium; patients may have subclinical or clinical disease without diagnostic antibody titers toB. burgdorferi. We conclude that early stage of the disease as well as chronic Lyme disease with persistence ofB. burgdorferi after antibiotic therapy cannot be excluded when the serum is negative for antibodies againstB. burgdorferi.

    Description / Table of Contents: Zusammenfassung In einer prospektiven, randomisierten Multizenter-Studie zur Therapie des Erythema migrans wurden 40 Patienten mit täglich 1 g Ceftriaxon intramuskulär über fünf Tage und 33 Patienten mit täglich 3 × 1 Mill. I. E. Phenoxymethylpenicillin über 12 Tage behandelt. Die Nachbeobachtung betrug durchschnittlich 10±5 Monate. Acht mit Penicillin (24%) und sechs mit Ceftriaxon behandelte Patienten (15%) entwickelten leichtere spätere Manifestationen. Zwei mit Ceftriaxon und ein mit Penicillin Behandelter wiesen erhöhte IgG Antikörper-Titer 10 bis 20 Monate nach Therapie auf.Borrelia burgdorferi konnte bei 29 von 56 Patienten (52%) vor und bei einem Patienten der Penicillingruppe, aber bei keinem Patienten der Ceftriaxongruppe nach Behandlung aus dem Erythema migrans isoliert werden. Ceftriaxon war dem Oral-Penicillin bei einer Untergruppe von Patienten mit mehr als einem Symptom vor Therapie überlegen (p〈0,01), aber nicht bei der Gesamtauswertung klinischer, serologischer und bakteriologischer Erfolgsdaten. Ceftriaxon sollte dem oralen Penicillin bei Patienten mit schwererer früher Lyme-Borreliose vorgezogen werden.
    Notes: Summary In a prospective randomized multicenter trial for the therapy of erythema migrans, 40 patients received ceftriaxone 1 g daily for 5 days and 33 patients obtained phenoxymethylpenicillin, 1 million units 3 times daily, for 12 days. Follow-up was for a mean of 10±5 months. Eight oral penicillin recipients (24%) and six ceftriaxone recipients (15%) developed minor consecutive manifestations. Two ceftriaxone and one penicillin recipient(s) still had elevated IgG antibody titers 10 to 20 months after therapy.Borrelia burgdorferi could be isolated from the erythema migrans in 29 out of 56 patients (52%) before therapy and in one oral penicillin recipient but none of 24 other patients after therapy. Ceftriaxone was superior to oral penicillin in a subgroup of patients with more than one symptom prior to therapy (p〈0.01), but not in the overall evaluation of clinical, serological and bacteriological outcome data. Ceftriaxone ought to be preferred to oral penicillin in patients with more severe early Lyme borreliosis.

    Notes: Abstract The complete coding regions of the chromosomally encoded p83/100 protein of four Borrelia garinii strains and one Borrelia burgdorferi sensu stricto strain have been amplified by the polymerase chain reaction (PCR), cloned and sequenced. From alignment studies with the deduced amino acid sequences presented here, and five other published p83/100 sequences, the most heterologous region of the p83/100 molecule was identified to be located between amino acid position 390–540. To study the structure of this heterogeneous region, and internal fragment of the p83/100 genes from 11 additional B. burgdorferi sensu lato strains was amplified by PCR. The PCR products were analyzed by DNA sequencing and restriction enzyme analysis. These internal p83/100 fragments varied in size and sequence. Cluster analysis of internal p83/100 fragments, as well as restriction enzyme analysis, revealed three major groups in accordance with grouping into the three species causing Lyme disease. Strains within the same species (six B. burgdorferi sensu stricto and six B. afzelii strains) showed similar p83/100 partial structures. Nevertheless, nine B. garinii strains showed more sequence variations and could be further divided into two major subgroups. One group is represented by OspA serotype 4 strains, the other more heterogeneous group is represented by OspA serotypes 3, 5, 6 and 7 strains. Phenotypic analysis with four p83/100-specific monoclonal antibodies revealed four distinct reactivity patterns. Antibody L100 1B4 recognized a common epitope of B. burgdorferi sensu stricto and B. afzelii. Antibodies L100 17D3 and L100 18B4 were reactive with an epitope shared by strains of all three species. The broadest reactivity was shown by L100 18B4 which, in constrast to L100 17D3, additionally recognized the relapsing fever borreliae B. turicatae and B. hermsii. L100 8B8 detected a subgroup of the B. burgdorferi sensu stricto strains. Since comparison of the p83/100 molecule with sequences from protein databases showed similarities with characteristics of eukaryotic cell structures, the p83/100 might mimic these structures and may, therefore, be involved in the immune escape mechanism of the pathogenic agent of Lyme disease.

    Notes: Abstract   Neuroborreliosis is the most frequent manifestation of the second stage of Lyme borreliosis in Europe. However, only few isolates from the cerobrospinal fluid (CSF) have been characterized with controversial results. A large panel of 36 CSF isolates isolated over a 10-year period in Munich has now been analyzed for their OspA and OspC type, resulting in at least eight different types, respectively. Representatives of the different types cultivated from CSF in Munich have also been isolated from other geographical regions in Europe from CSF or ticks, suggesting a widespread distribution of pathogenic strains. A certain OspA type (type 4) was frequently observed in adults but rarely in children or ticks. Since OspA and OspC are the most promising candidates for a Borrelia vaccine, the considerable heterogeneity found among CSF isolates has important implications for development of a vaccine in Europe.
